外侧块螺钉内固定术治疗颈椎骨折脱位

摘    要:采用外侧块螺钉内固定术治疗颈椎骨折脱位。受伤部位:C_3损伤2例,C_4损伤6例,C_5损伤7例,C_6损伤5例,C_7损伤3例。按Frankel神经功能分类法,A级11例,B级3例,C级4例,D级3例,E级2例。随访时间7个月~4年7个月,平均2年9个月。结果23例中优良6例,显效13例,差4例,有效率82.6%,植骨融合率100%。术中无损伤神经血管、术后感染、内固定物断裂等并发症。外侧块螺钉内固定治疗颈椎骨折脱位的优点是:在进行后方减压的同时能进行坚强的内固定;适用于伴有后方结构损伤的中下颈椎骨折脱位的内固定治疗;有利于促进脊髓功能的恢复。

Abstract:The article reported 23 patients with fracture - dislocation of cervical spine which were operated by lateral mass screw fixation and bone graft.Two cases were injured in C3, six cases in C4, seven cases in C5, five cases in C6. Follow - up period was from 7 months to 4 years and 7 months with an average of 2 years and 9 months. According to Frankel' s neurological function classification, 11 cases were graded as degree A, 3 cases as degree B, 4 cases as degree C, 3 cases as degree D, and 2 cases as degree E.Six cases gained excellent or good results, 13 cases fair results and 4 cases poor re-sults. The rate of improvement of their neurological functions was 82.6% .The rate of bone fusion was 100% .There were no major complications such as neural or vascular injuries , breakage of implants or infection.This method has the advan-tage of rigid internal fixation, even after posterior decompression is performed, compared with wire internal fixation. It is useful for management of fracture - dislocation of middle or lower cervical spine complicated with posterior element injury and facilitate the recovery of injuried spinal function.
