人工关节置换术后感染的Ⅱ期翻修术治疗

人工关节置换术后由于机体全身和局部抵抗力降低，容易并发感染。在此过程中，细菌的数量、毒力和机体的抵抗力是三个最重要的因素，而细菌粘附是决定性起始步骤。Ⅱ期翻修术治疗是人工关节置换术后感染常用的治疗方法，即第一期对感染彻底清创，取出人工关节假体、骨水泥等异物，间隔一定时间后，第二期再植新假体。其主要优点在于感染清除率高，如在翻修间隔期使用暂时性关节替代系统如PROSTALAC等，再植新假体后关节功能可较好地保留。选择合适的抗生素以合适的方式给药是Ⅱ期翻修术治疗的重要组成部分，尤其是局部抗生素缓释系统的应用可显提高感染清除率。但是，Ⅱ期翻修术治疗的时机即间隔期的长短尚无一致意见，原因在于缺乏实验依据和理论指导，根本在于尚未建立有效的动物模型。
