实时荧光定量PCR检测急性髓系白血病患者WT1基因表达及其临床意义

摘    要:目的:应用实时荧光定量PCR(real time quantitative PCR,RQ-PCR)检测WT1基因表达水平,探讨其在监测白血病微小残留病(minimal residual disease,MRD)中的临床意义。方法:采用RQ-PCR检测70例急性髓系白血病(AML)患者、20例正常对照者骨髓细胞WTl基因拷贝数及内参基因(ABL)拷贝数,以NQ比值=WTl基因拷贝数/内参基因拷贝数计算基因表达水平。结果:初诊组与复发组WT1表达水平分别与完全缓解组及对照组比较差异均有统计学意义(P〈0.01),而对照组与完全缓解组之间比较及初诊与复发组之间比较差异无统计学意义(P〉0.05),即初诊急性白血病患者WT1表达水平明显高于完全缓解组及对照组,复发时同样高于完全缓解组及对照组。在初诊的各种白血病亚型中,M3亚型WT1表达水平最高(平均值为3.44),M5亚型WT1表达水平最低(平均值为0.25)。4例短期随访提示在达到完全缓解时WT1表达水平下降,而复发时再次升高。结论:用RQ-PCR测定WT1基因的表达,可以作为白血病MRD的一项监测指标。
