综合疗法治疗膝关节滑膜炎随机平行对照研究

目的]观察综合疗法治疗膝关节滑膜炎疗效。方法]使用随机平行对照方法，将200例门诊患者按随机数字表分为两组。对照组100例膝关节穿刺联合注射玻璃酸钠注射液，支具及石膏固定并卧床休息。治疗组100例①塌渍：牛膝、防己、木瓜、威灵仙、乳香、没药、续断、骨碎补各20g，制川乌、制草乌各5g，红花30g，1剂／d，水煎100mL，塌渍膝关节肿胀处，lh／次，2；K／d。②推拿：患者仰卧位，按摩医师点按髀关、伏兔、三阴交、阴陵泉，足三里，解溪、委中等穴位，30min/d。穴位按摩后行膝关节拔伸屈曲练习。③运动疗法：膝关节股四头肌等长、等张收缩练习、胭绳肌、内收肌及踝泵功能训练。④红光治疗仪，30min／次，2；L／d。⑤中药内服：活血通络、利水消肿。利水渗湿汤，苍术、黄柏、防己、威灵仙、苹薜各10g，赤小豆、薏苡仁、黄芪各30g，甘草、茯苓各15g，1剂／d，水煎300mL，早晚口服；基础治疗同对照组。连续治疗2周为1疗程。观测临床症状、关节功能、浮髌试验、不良反应。连续治疗3疗程，判定疗效。结果]治疗组痊愈56例，显效27例，有效10例，无效7例，总有效率92，70％。对照组痊愈42例，显效38例，有效10例，无效10例，总有效率91．30％。治疗组疗效优于对照组（P〈0．05）。随访1年，远期疗效治疗组优于对照组（P〈0．05）。结论]综合疗法治疗膝关节滑膜炎，疗效满意，无副作用，值得推广。

Continuous 3 courses of treatment, the curative effect. Result ] The treatment group cured 56 cases, effective in 27 cases, effective 10 cases, invalid 7 cases, the total efficiency of 92.70%. The control group cured 42 cases, effective in 38 cases, effective 10 cases, invalid 10 cases, the total efficiency of 91.30%. The treatment group was better than control group （P〈 0.05 ） . 1 years of follow-up, the long-term effect of the treatment group than in control group （ P〈 0.05 ） . Conclusion ] Comprehensive therapy in the treatment of knee joint synovitis, satisfactory curative effect, no side effect, is worth promoting .
