体外受精-胚胎移植在多囊卵巢综合征不育患者中的应用

目的　评价体外受精 胚胎移植(IVF ET)或卵胞浆内单精子注射(ICSI)对多囊卵巢综合征(PCOS)不育妇女的治疗效果。　方法　对2000年1月至2003年12月在本中心行 IVF ET或 ICSI的PCOS不育患者作回顾性分析,A组为55例PCOS不育患者72周期,对照组(B组)为输卵管阻塞性不育患者95例共100周期,比较两组妊娠率、流产率及卵巢过度刺激综合征(OHSS)发生率。　结果　临床妊娠率A组为40%,B组为49%,流产率分别为7%及 6%,两组比较差异无显著意义(P>0.05), OHSS发生率 A组 10%、B组 3%,两组比较差异有显著性(P<0.01)。　结论　对于经常规治疗无效的PCOS不育患者, IVF ET不失为有效的治疗方法之一,但OHSS发生率较非PCOS不育妇女高。

The application of in vitro fertilization and embryo transfer in the infertile patients with polycystic ovary syndrome

Objective: To evaluate the effect of in vitro fertilization-embryo transfer (IVF-ET) or intracytoplasmic sperm injection (ICSI) in the infertile patients with polycystic ovarian syndrome (PCOS). Methods: The infertile patients with PCOS who underwent IVF-ET or ICSI from January 2000 to December 2003 were retrospectively studied. 55 PCOS patients (72 cycles) served as group A and 95 non-PCOS patients with infertility due to tubal occlusion (100 cycles) as control (group B). The clinical pregnancy rates, miscarriage rates, and the occurrence of ovarian hyperstimulation syndrome (OHSS) were compared between the two groups. Results: The clinical pregnancy rate in group A and group B was 40% and 49% respectively, and the miscarriage rate in group A and group B was 7% and 6% respectively. No significant differences of these rates were observed between the two groups, while the incidence of OHSS was significantly higher in group A than in group B (10% vs. 3%, P<0.01). Conclusion: This study demonstrated that IVF-ET is an alternative for the infertile patient with PCOS after the conventional methods failed, but the PCOS patients tend to have more OHSS than non-PCOS infertile patients.
