人工股骨头置换术治疗高龄股骨转子间骨折的效果观察

目的探讨人工股骨头置换术治疗高龄股骨转子间骨折的效果。方法选取本院2009年1月~2012年12月收治的60例不稳定性高龄股骨转子间骨折患者,随机分为研究组和对照组,研究组采用人工股骨头置换术进行治疗,对照组应用动力髋螺钉(DHS)内固定治疗,比较两组患者的治疗效果。结果经研究两组患者治疗过程中的手术时间、出血量、并发症发生率差异有统计学意义(P<0.05),髋关节功能评分差异无统计学意义(P>0.05)。结论 DHS内固定术与人工股骨头置换术治疗老年股骨转子间骨折均具有明显作用,但是人工股骨头置换术过程中出血量少、并发症少、可及时恢复,在高龄、骨质疏松、不稳定股骨转子间骨折患者中,具有更为明显的效果,但无法完全代替内固定手术,在临床应用中需根据患者的实际情况予以选择。

Observation on the efficacy of artificial femoral head replacement on treatment of elderly patients with intertrochanteric fracture

Objective To explore the efficacy of artificial femoral head replacement on treatment of elderly patients with intertrochanteric fracture. Methods 60 elderly patients with unstable intertrochanteric fracture treated in our hos- pital from January 2009 to December 2012 were selected and randomly assigned to the research group and the control group.The research group was treated with artificial femoral head replacement while the control group was treated with DHS internal fixation.The therapeutic effects were compared between the two groups. Results The study found that the two groups had statistical difference with regard to duration of operation,hemorrhage volume and complications （P〈 0.05）,but no statistical difference with regard to hip joint function score （P〉0.05） during treatment. Conclusion Both DHS internal fixation and artificial femoral head replacement have obvious efficacy on treatment of elderly patients with intertrochanteric fracture.Even though artificial femoral head replacement demonstrates better efficacy on elderly osteo- porotic patients with unstable intertrochanteric fracture because of less hemorrhage volume,fewer complications and prompt recovery,it still cannot completely replace internal fixation.Therefore,the treatment option should be determined based on patients＇ actual conditions in clinical practice.
