目的:评价“通督调神法”针刺加重复经颅磁刺激(rTMS)治疗脑卒中后失眠的临床疗效。方法:将72例脑卒中后失眠患者随机分为2组,每组36例。对照组采用频率1 Hz、运动阈值90%的rTMS治疗,观察组在对照组rTMS治疗基础上加“通督调神法”针刺治疗。穴位取百会、神庭、印堂、风府、四神聪、神门、三阴交、申脉和照海。每日治疗1次,每周治疗5d后休息2d,共治疗4周。现察两组治疗后匹兹堡睡眠质量指数(PSQI)、焦虑自测量表(SAS)和抑部自测量表(SDS)评分改善情况,并进行两组临床疗效比较。结果:治疗4周后,两组PSQK SAS及SDS评分均降低,组内差异均有统计学意义(均P<0.001);观察组治疗后三项评分均低于对照组,组间差异均有统计学意义(P<0.05,P<0.05,P<0.001),现察组和对照组临床疗效差异有统计学意义(P<0.05)。结论:“通督调神法”针刺加rTMS治疗脑卒中后失眠疗效优于单独rTMS治疗,且能更好地改善患者焦虑、抑耶情绪。  相似文献

Individuals with schizophrenia spectrum disorders (SSD) experience frequent sleep disturbances in addition to enduring cognitive impairments. The purpose of the present review was to systematically summarise our current understanding of the association between sleep disturbances and cognition in SSD. Through this, it was aimed to identify features of disturbed sleep that are reliably associated with cognitive deficits in SSD and identify the gaps within the current literature that require future investigation. Eighteen relevant studies were identified following a two-stage screening process. Following a structured narrative synthesis of key study components, no clear and consistent pattern emerged. Considerable methodological variability was present amongst the reviewed studies. Although some broad consistencies were identified, such as associations between sleep spindle density and sleep-dependent memory consolidation, the overall pattern of results lacked a cohesive composition due to the diverse list of sleep parameters and cognitive domains investigated, as well as a lack of replication. Additional research is needed before more definitive remarks can be made regarding the influence of sleep disturbances on cognitive function in SSD.  相似文献

This study seeks to understand whether people substitute between recreational cannabis and conventional over-the-counter (OTC) sleep medications. UPC-level grocery store scanner data in a multivariable panel regression design were used to compare the change in the monthly market share of sleep aids with varying dispensary-based recreational cannabis access (existence, sales, and count) in Colorado counties between 12/2013 and 12/2014. We measured annually-differenced market shares for sleep aids as a portion of the overall OTC medication market, thus accounting for store-level demand shifts in OTC medication markets and seasonality, and used the monthly changes in stores’ sleep aid market share to control for short-term trends. Relative to the overall OTC medication market, sleep aid market shares were growing prior to recreational cannabis availability. The trend reverses (a 236% decrease) with dispensary entry (−0.33 percentage points, 95% CI −0.43 to −0.24, p < 0.01) from a mean market share growth of 0.14 ± 0.97. The magnitude of the market share decline increases as more dispensaries enter a county and with higher county-level cannabis sales. The negative associations are driven by diphenhydramine- and doxylamine-based sleep aids rather than herbal sleep aids and melatonin. These findings support survey evidence that many individuals use cannabis to treat insomnia, although sleep disturbances are not a specific qualifying condition under any U.S. state-level medical cannabis law. Investigations designed to measure the relative effectiveness and side effect profiles of conventional OTC sleep aids and cannabis-based products are urgently needed to improve treatment of sleep disturbances while minimizing potentially serious negative side effects.  相似文献

目的:观察D-半乳糖联合对氯苯丙氨酸(PCPA)致肾不藏志不寐大鼠下丘脑神经递质和凋亡基因的变化。方法:40只SD大鼠随机分为对照组、老年组、失眠组、肾不藏志不寐组(模型组),每组10只。老年组大鼠给予D-半乳糖皮下注射,失眠组给予对PCPA腹腔注射,肾不藏志不寐组给予D-半乳糖皮下注射联合PCPA腹腔注射建立模型,对照组给予等剂量生理盐水注射。造模结束后测量各组大鼠体质量、采用水迷宫观察大鼠定位航行探索运动轨迹、采用RT-PCR法检测各组大鼠下丘脑GABAARα1、GABAARβ2、GABAARγ2、Bax、Bcl-2 mRNA的相对表达量。结果:与空白组比较,模型组体质量显著降低(P<0.01);定位航行探索运动轨迹显著延长;模型组下丘脑GABAARα1、GABAARβ2、GABAARγ2、Bcl-2mRNA相对表达量均显著性降低(P<0.05,P<0.05,P<0.01,P<0.05);下丘脑Bax mRNA相对表达量显著性增加(P<0.05)。结论:D-半乳糖联合PCPA致肾不藏志不寐大鼠下丘脑睡眠相关的神经递质和凋亡基因表达发生改变。  相似文献

目的基于文献研究,探讨失眠症的中医证候要素分布规律。方法通过检索中国知网数据库(CNKI)、万方数据库、维普全文期刊数据库2009年1月至2018年12月中医药期刊文献所报道的治疗失眠症相关文献,收集统计失眠症中医证候并提取证素信息,利用SPSS 23.0软件进行统计描述分析。结果(1)共检出文献653篇,符合要求317篇,整理后规范证名得到44种中医证候,其中以心脾两虚证、痰热内扰证、肝郁化火证、阴虚火旺证、心肾不交证等频次较高。(2)证素组合规律中以两证素、三证素、四证素组合为主。(3)在单一病性证素中以热(火)、气虚、血虚、阴虚等频次较高;病位证素以肝、心、脾、肾等频次较高。(4)在证素组合时以热(火)+肝、气虚+血虚+心+脾、热(火)+痰、热(火)+阴虚等频次较高。结论失眠症的主要病理因素以热(火)、气虚、血虚、阴虚为主,病位主要以肝、心、脾、肾为主。  相似文献

目的采用网络药理学方法寻找黄连阿胶汤的有效成分、治疗靶标。方法根据药代动力学参数筛选药物成分,通过网络药理学方法挖掘药物和疾病靶标并构建药物-靶点网络,研究黄连阿胶汤成分的作用机制。结果发现黄连阿胶汤中的氢化小檗碱、β-谷甾醇、山姜素、山柰酚、小檗浸碱、荠苎黄酮等19个成分可能通过调控5-羟色胺、多巴胺、γ氨基丁酸、阿片受体、乙酰胆碱等25个靶点作用于失眠症治疗。结论黄连阿胶汤可能主要通过多个靶点调控多种神经递质发挥对失眠症的治疗作用。  相似文献

BackgroundThe relationship between insomnia and lung cancer is scanty. The Mendelian randomization approach provides the rationale for evaluating the potential causality between genetically-predicted insomnia and lung cancer risk.MethodsWe extracted 148 insomnia-related single-nucleotide polymorphisms (SNPs) as instrumental variables (IVs) from published genome-wide association studies (GWASs). Summary data of individual-level genetic information of participants were obtained from the International Lung Cancer Consortium (ILCCO) (29,266 cases and 56,450 controls). MR analyses were performed using the inverse-variance-weighted approach, MR pleiotropy residual sum and outlier (MR-PRESSO) test, weighted median estimator, and MR-Egger regression. Sensitivity analyses were further performed using Egger intercept analysis, leave-one-out analysis, MR-PRESSO global test, and Cochran's Q test to verify the robustness of our findings.ResultsThe results of the MR analysis indicated an increased risk of lung cancer in insomnia patients (OR = 1.1671; 95% CI 1.0754–1.2666, p = 0.0002). The subgroup analyses showed increased risks of lung adenocarcinoma (OR = 1.1878; 95% CI 1.0594–1.3317, p = 0.0032) and squamous cell lung cancer (OR = 1.1595; 95% CI 1.0248–1.3119, p = 0.0188).ConclusionOur study indicated that insomnia is a causal risk factor in the development of lung cancer. Due to the lack of evidence on both the epidemiology and the mechanism level, more studies are needed to better elucidate the results of the study.  相似文献

ObjectivesThis pilot study aimed to examine the efficacy of integrated cognitive behavioral therapy (CBT) and acupressure in treating insomnia and its daytime impairments in a Chinese adult population.Methods40 eligible participants with insomnia were randomly assigned to either the integrated CBT and acupressure (CBTAcup) group (n = 14), the CBT group (n = 13), or the waitlist control (WL) group (n = 13). Participants in the CBTAcup group attended a 2-hour integrated CBT and self-administered acupressure group treatment once per week for six consecutive weeks, while participants in the CBT group attended six weekly 2-hour CBT for insomnia. Sleep, mood, daytime impairments, quality of life, and treatment credibility and adherence were assessed at baseline, immediate post-treatment (Week 7), and 4-week post-treatment (Week 11).ResultsLinear mixed-effects models showed that both the CBTAcup and CBT groups had significantly lower insomnia severity (d = −1.74 and d = −2.61), dysfunctional beliefs related to sleep (d = −2.17 and −2.76), and mental fatigue (d = −1.43 and −1.60) compared with the WL group at Week 7. The CBTAcup group provided additional benefits in reducing total fatigue (d = −1.43) and physical fatigue (d = −1.45). Treatment credibility was found to be improved in the CBTAcup group from baseline to Week 7.ConclusionsIntegrated CBT and acupressure demonstrated comparable efficacy to pure CBT in treating insomnia symptoms, with additional advantages to improve fatigue symptoms and acceptability in the Chinese population. Further methodologically rigorous studies on a larger scale and longer follow-up are warranted to confirm these findings.  相似文献

There is considerable concern in the mental health community about the psychological consequences of the coronavirus pandemic and who may be most vulnerable. Obsessive-compulsive disorder (OCD) symptoms may be particularly sensitive to the context of the pandemic. Previous research suggests insomnia symptoms may contribute to increased OCD symptoms over time, particularly during times of stress, such as the pandemic. The present study examined pre-coronavirus outbreak insomnia symptoms as a predictor of post-coronavirus outbreak OCD symptoms in a sample of community adults who completed a 2016 survey study and were re-contacted on April 1, 2020 (N = 369). Results revealed a small significant increase in OCD symptoms following the coronavirus outbreak and a small significant decrease in insomnia symptoms. Pre-coronavirus outbreak insomnia symptoms significantly predicted increases in post-coronavirus outbreak OCD symptoms. Similar results were found for specific OCD symptom facets with the exception of washing and hoarding symptoms, which were unrelated to pre-coronavirus insomnia symptoms. There was no evidence for a reverse effect of prior OCD symptoms on insomnia symptoms during the pandemic. These findings suggest those with insomnia symptoms prior to the coronavirus pandemic may be vulnerable to increases in some OCD symptoms during the pandemic. The implications for preventing adverse psychological responses during the coronavirus pandemic are discussed.  相似文献

BackgroundThere is possibly an association between migraine, tension-type headache, anxiety, depression and insomnia. These conditions are prevalent among university students. Our primary objective was to verify whether students with primary headaches (migraine and tension-type headache) have a higher prevalence of insomnia. Our secondary objective was to assess whether the impact of headaches was associated with greater severity of insomnia.MethodsCross-sectional study. 440 students out of 3030 were randomly selected. A semi-structured questionnaire containing information about the characteristics of the headaches, including their frequencies in the last 3 months; the Headache Impact Test (HIT-6); the Hospital Anxiety Depression Scale; and the Insomnia Severity Index were used.Results420 students (95.5%) agreed to participate; 51.4% men; median age of 21 (19, 23); 95 (22.6%) had insomnia; 265 (63.1%), migraine; 152 (36.2%), tension-type headache; 201 (47.9%) suffered from anxiety and 108 (25.7%), from depression. The severe impact of headache (HIT-6>55 points; OR = 3.9; p = 0.003) and anxiety (OR = 3.6; p = 0.003) were associated with insomnia (logistic regression). The severity of insomnia was positively and significantly correlated with the impact (HIT-6 score), with frequency of headache, and with having anxiety (multiple linear regression).ConclusionsThe diagnoses of migraine and tension-type headache are not associated with the presence of insomnia. The severity of insomnia is associated with the impact and the frequency of the headaches.  相似文献
