介绍李国安主任医师运用针灸治疗遗尿、艾灸治疗霰粒肿和针药结合治疗垂体生长素腺瘤验案。     

Amblyopia due to inflamed chalazion in a 13-month old infant

Meibomian gland cysts (chalazia) are very common lesions in childhood and are not thought of as visually threatening lesions. A case is reported of dense amblyopia and secondary exotropia resulting from an upper eyelid chalazion.     

Clinical and cytopathologic correlation in chronic inflammations of the orbit and ocular adnexa: A review of 55 cases

Fine-needle aspiration cytology has been used for the diagnosis of neoplastic lesions of the orbit and ocular adnexa with some success. However, inflammatory conditions have not received much attention. This is a retrospective study of fine-needle aspirations of chronic inflammations of the orbit and ocular adnexa over a 15-year period from January 1988 through December 2002. Clinical records and stained smears were reviewed. There were 55 chronic inflammations, presenting as masses, cysts, abscesses, discharging sinuses and dermal plaques and nodules. Forty-two were granulomatous inflammations comprising chalazion (18), tuberculosis (5), Cysticercus cellulosae (3), ruptured epidermal cysts (2), actinomycosis (1) and leprosy (13). Thirteen patients had chronic non-specific inflammations. Fine-needle aspiration cytology is a useful diagnostic procedure in chronic inflammations of the ocular adnexa. Specific diagnosis is possible in many granulomatous inflammations, allowing appropriate, individualized therapy to be initiated.     

复发性霰粒肿患者眼部蠕形螨感染调查分析

目的：了解复发性霰粒肿患者人体蠕形螨的感染情况及引起霰粒肿复发的可能相关因素。

方法：收集2019-01/05于我院眼表和角膜病科门诊就诊确诊“霰粒肿”的患者，A组32眼为复发性霰粒肿组，B组30眼为初发性霰粒肿组，C组35眼为无眼病的正常对照组。生物光学显微镜观察蠕形螨睫毛根部阳性感染率，活体激光扫描共聚焦显微镜检查睫毛根部、睑板腺管开口蠕形螨感染情况。

结果：生物光学显微镜观察情况：A组患者睫毛的蠕形螨检出率为78%，显著高于B组(57%)和C组(34%)。共聚焦显微镜观察情况：A组患者睫毛的蠕形螨检出率为88%，显著高于B组(67%)和C组(37%)； A组患者睑板腺开口的蠕形螨检出率为69%，显著高于B组(23%)和C组(14%)。

结论：复发性霰粒肿患者蠕形螨感染率明显较高，蠕形螨感染可能是顽固性霰粒肿致病原因之一。     

Keep an Eye on Chalazion

Nurse practitioners routinely encounter patients presenting with chalazion or meibomian cyst in primary care. This report describes the pathogenesis, assessment, and treatment of chalazion. Differential diagnosis and management strategies are discussed, with attention to more serious diagnoses, such as sebaceous cell carcinoma. Many malignancies can conceal as benign or inflammatory lesions, making diagnoses difficult and leading to higher morbidity and mortality. A vigilant history and physical examination will aid in early assessment and referral of clients with new or suspicious lesions warranting further investigation.     

Cytopathology of chalazia

Chalazion is a common inflammatory condition of the eyelid, usually treated on the basis of clinical diagnosis alone. Preoperative exclusion of malignancy in chalazia with atypical clinical presentation could prevent unwarranted surgery. This is a retrospective study of aspirates from 16 patients with chalazia having an atypical clinical presentation. Smears were stained with May-Grünwald Giemsa. Two broad patterns of granulomatous inflammation reflecting the spectrum of changes in the course of the disease were seen. Nine smears had mixed-cell granulomas consisting of neutrophils, lymphocytes, plasma cells, macrophages, giant cells, and granulation tissue. Seven smears had suppurating granulomas characterized by epithelioid cell granulomas with numerous neutrophils in a proteinaceous background. Fine-needle aspiration cytology of chalazia with atypical clinical presentation provides a rapid, safe, and reliable means of documenting the diagnosis and excluding malignancy.     

石炭酸治疗睑板腺囊肿的临床观察

目的 为提高睑板腺囊肿手术的成功率，探讨减少睑板腺囊肿复发的简易手术方法。方法 对950例睑板腺囊肿采用石炭酸涂睑板腺囊肿的囊壁，观察其疗效。结果 950例睑板腺囊肿均能达到破坏其囊壁的完整性，术后随访7天-6个月，无复发，达到根治目的。结论 石炭酸治疗睑板腺囊肿方法简易、并发症少，是较为理想的手术方法。
