Post-induction hypotension is common and associated with postoperative complications. We hypothesised that pneumatic leg compression reduces post-induction hypotension in elderly patients undergoing robot-assisted laparoscopic prostatectomy. In this double-blind randomised study, patients were allocated randomly to the pneumatic leg compression group (n = 50) or control (n = 50). In the intervention group, pneumatic leg compression was initiated before induction of anaesthesia. In the control group, pneumatic leg compression was initiated 20 min after anaesthesia induction. The primary outcome was the incidence of post-induction hypotension in these groups. Post-induction hypotension was defined as systolic blood pressure < 90 mmHg during the first 20 min after induction. Haemodynamic variables and area under the curve of post-induction systolic blood pressure over time were assessed. Complications associated with pneumatic leg compression were recorded, including: peripheral neuropathy; compartment syndrome; extensive bullae beneath the leg sleeves; and pulmonary thromboembolism. The incidence of post-induction hypotension decreased in the pneumatic leg compression group compared with that in the control group; 5 (10%) vs. 29 (58%), respectively, p < 0.001. In the pneumatic leg compression group, the lowest systolic, diastolic and mean blood pressures 20 min after induction of anaesthesia were significantly greater than the control group. Pneumatic leg compression resulted in an increased area under the curve of systolic blood pressure in the first 20 min after induction, p = 0.001. There were no pneumatic leg compression-related complications. Pneumatic leg compression reduced post-induction hypotension in elderly patients undergoing robot-assisted laparoscopic prostatectomy, suggesting that it is an effective and safe intervention to prevent post-induction hypotension among elderly patients undergoing general anaesthesia.  相似文献

目的探讨不同直腿抬高扳腿法对推拿治疗腰椎间盘突出症临床疗效的影响。方法选择2018年5月—2019年1月在广西中医药大学第一附属医院推拿科治疗的腰椎间盘突出症患者100例,随机分为对照组与观察组各50例。2组均采用相同的松解类推拿手法治疗,治疗过程中观察组采用改进的直腿抬高扳腿法,对照组采用常规的直腿抬高扳腿法。2组均以治疗6次为1个疗程,疗程之间间隔1 d,共治疗4个疗程。观察2组治疗效果及治疗前后患侧直腿抬高角度、疼痛视觉模拟评分(VAS评分)、日本骨科学会腰椎功能量表评分(JOA评分)变化和治疗过程中可能发生的不良反应。结果观察组与对照组愈显率分别为74%(37/50)、54%(27/50),总有效率分别为96%(48/50)、94%(47/50),观察组愈显率明显高于对照组(P<0.05),2组总有效率比较差异无统计学意义(P>0.05)。与治疗前比较,治疗后2组患者患侧直腿抬高角度均显著增大(P均<0.05),VAS评分均显著降低(P均<0.05),JOA评分均显著升高(P均<0.05);治疗后2组比较,观察组患侧直腿抬高角度显著大于对照组(P<0.05),VAS评分显著低于对照组(P<0.05),JOA评分显著高于对照组(P<0.05)。观察组患者均未出现疼痛症状加重等手法反应,对照组有7例患者出现不同程度的腰臀疼痛症状加重。结论在采用推拿手法治疗腰椎间盘突出症时,改进的直腿抬高扳腿法较常规的直腿抬高扳腿法疗效更为显著,可能发生的不良反应更少。  相似文献

The aim of this study was to investigate the acute repeated bouts of aerobic exercise decrease leg arterial stiffness. However, the influence of repeated bouts of aerobic exercise on arterial stiffness after glucose ingestion is unknown. The present study investigates the acute effects of repeated bouts of aerobic exercise on arterial stiffness after the 75-g oral glucose tolerance test (OGTT). Ten healthy young men (age, 23.2 ± 0.9 years) performed repeated bouts of aerobic exercise trial (RE, 65% peak oxygen uptake; two 15 min bouts of cycling performed 20 min apart) and control trial (CON, seated and resting in a quiet room) at 80 min before the 75-g OGTT on separate days in a randomized, controlled crossover fashion. Carotid-femoral (aortic) and femoral-ankle (leg) pulse wave velocity, carotid augmentation index, brachial and ankle blood pressure, heart rate and blood glucose and insulin levels were measured before (baseline) and 30, 60 and 120 min after the 75-g OGTT. Leg pulse wave velocity, ankle systolic blood pressure and blood glucose levels increased from baseline after the 75-g OGTT in the CON trial, but not in the RE trial. The present findings indicate that acute repeated bouts of aerobic exercise before glucose ingestion suppress increases in leg arterial stiffness following glucose ingestion.

BackgroundVenous leg ulcers (VLUs) often take a very long time to heal. Timolol maleate has been reported as displaying efficacy in healing of VLUs.ObjectivesTo evaluate the efficacy of timolol maleate gel in the management of hard-to-heal VLUs and to assess its safety as a topical agent during 12 weeks of use in combination with conventional treatment.MethodsA prospective, phase-II randomised-controlled trial with a sample size based on Fleming's one-stage design (P0 = 0.25, P1 = 0.45, alpha = 0.1, beta = 0.2) was planned. Patients with VLUs present for ≥ 24 weeks and with ≥ 50% granulation tissue were included. One drop of sustained-release timolol gel (Timoptol® LP 0.5%, Santen, Tampere, Finland) per 6 cm2 VLU area was applied every 2 days for 12 weeks in timolol-treated patients, as adjuvant therapy to the standard care protocol (interface dressing and multilayer venous compression). Controls received standard care alone. The primary endpoint was to obtain ≥ 40% reduction in ulcer area at week 12 (W12).ResultsForty-three patients were randomised to the study, with 40 receiving at least one treatment and included in the analysis: 21 timolol-treated patients and 19 controls (females: 70%; median age: 72.5 [range 35–93] years). At W12, ≥ 40% ulcer-area reduction was achieved in 14/21 (67%) timolol-treated patients vs. 6/19 (32%) controls. No serious adverse events occurred. Local wound infections not requiring systemic antibiotics occurred in 5 cases in the timolol group and in one case in the controls.ConclusionsThese results support the benefit and safety of using timolol maleate to manage hard-to-heal VLUs, but confirmation is required in a larger multicentre randomised phase-III study.  相似文献

BackgroundThe goals and approaches to fluid therapy vary through different stages of resuscitation. This pilot study was designed to test the safety and feasibility of a fluid therapy protocol for the second or optimisation stage of resuscitation in patients with predicted severe acute pancreatitis (SAP).MethodsSpontaneously breathing patients with predicted SAP were admitted after initial resuscitation and studied over a 24-h period in a tertiary hospital ward. Objective clinical assessment (OCA; heart rate, mean arterial pressure, urine output, and haematocrit) was done at 0, 4, 8, 12, 18–20, and 24 h. All patients had mini-fluid challenge (MFC; 250 ml intravenous normal saline within 10 min) at 0 h and repeated at 4 and 8 h if OCA score ≥2. Patients who were fluid responsive (>10% change in stroke volume after MFC) received 5–10 ml/kg/h, otherwise 1–3 ml/kg/h until the next time point. Passive leg raising test (PLRT) was done at each time point and compared with OCA for assessing volume status and predicting fluid responsiveness.ResultsThis fluid therapy protocol based on OCA, MFC, and PLRT and designed for the second stage of resuscitation was safe and feasible in spontaneously breathing predicted SAP patients. The PLRT was superior to OCA (at 0 and 8 h) for predicting fluid responsiveness and guiding fluid therapy.ConclusionsThis pilot study found that a protocol for intravenous fluid therapy specifically for the second stage of resuscitation in patients with predicted SAP was safe, feasible, and warrants further investigation.  相似文献

BackgroundCurved periacetabular osteotomy (CPO) is one of the periacetabular osteotomies for the treatment of acetabular dysplasia. Several complications have been described after CPO, however, there have been no reports on the leg length change (LLC). This study aimed to investigate the LLC after CPO and its impact on the clinical outcomes.MethodsThis study was a retrospective review of 70 consecutive hips in 67 patients with symptomatic acetabular dysplasia who underwent CPO between March 2016 and April 2019. Preoperative and postoperative leg lengths were measured using anteroposterior radiographs, and the clinical outcomes were evaluated based on the Harris hip score (HHS) and Medical Outcomes Survey 36-item Short Form Health Survey (SF-36).ResultsThe mean LLC (and standard deviation) after CPO was −0.08 ± 3.10 mm. The mean HHS significantly improved from 73.5 points to 91.9 points (P < .001). The physical component and role component scores of SF-36 significantly improved from 35.1 to 46.1 (P < .001) and from 39.5 to 47.0 (P < .001), respectively. No significant differences were found between the preoperative and postoperative mental component scores of SF-36. In addition, among 70 hips, 35 hips exhibited leg length elongation (0 to plus 6.82 mm) after CPO, whereas 35 hips exhibited leg length shortening (0 to minus 6.23 mm). No significant differences were found in HHS and SF-36 between the leg elongation group and leg shortening group.ConclusionThe mean LLC after CPO was −0.08 ± 3.10 mm, and this change does not affect the postoperative clinical outcomes.  相似文献

目的探究Most Care/PRAM系统监测下不停跳冠状动脉旁路移植术(OPCABG)患者术中血流动力学变化情况和预后分析。方法纳入2016年10月至2017年1月安贞医院89例OPCABG患者,其中男53例、女36例,年龄(60.50±8.40)岁。记录术中血流动力学变化情况。按是否发生心肌梗死、低心排血量等严重循环不良事件,分为平稳组和严重循环不良事件组,进行相关分析。结果手术全程监测完整血流动力学数据患者65例,开胸前和关胸后被动抬高试验的每搏量(SV)升高均值分别为23.00%±3.20%和29.40%±3.70%。麻醉、开胸、应用肝素时、搭桥中、应用鱼精蛋白时、关胸和术毕7个时间段,SV明显下降,外周血管阻力指数(SVRI)持续显著增加,最大压力梯度(d P/d T)和心脏循环效率(CCE)在麻醉后明显下降,搭桥时下降到最低,其后逐渐升高;每搏量变异率(SVV)和脉压变异率(PPV)在麻醉后下降,开胸后一直升高。89例患者发生严重循环不良事件共9例,其中4例死亡。严重循环不良事件组术中基础SVRI、SVV和PPV均显著高于平稳组(P<0.05),CCE、d P/d T和SV差异无统计学意义(P>0.05)。术中基础SVRI、CCE、d P/d T、SVV、PPV和SV均值与预后指标均无明显相关性。结论OPCABG术中易出现血流动力学的改变,因此,OPCABG术中宜应用Most Care/PRAM仪进行血流动力学监测,并及时纠正血流动力学异常。  相似文献

目的了解尿毒症继发不安腿综合征患者患病率、严重程度、危险因素、具体不适部位及临床表现。方法以2015年3~7月于首都医科大学附属北京友谊医院、北京第六医院、北京中兴医院、首都医科大学附属北京朝阳医院血液净化中心尿毒症持续性血液透析患者186例为研究对象,进行横断面研究,采用床旁问卷形式,以国际不安腿综合征研究小组(IRLSS)制定的4个基本标准进行诊断,以IRLSSG量表进行严重程度分级,对患者一般情况、生活习惯、治疗情况及相关实验室检查进行统计,详细记录患者不适部位及具体临床表现。结果入组患者186例,根据是否符合RLS诊断,将其分为RLS(+)组73例和RLS(-)组113例。RLS(+)组:患病率为39.25%,RLS严重程度评分为(16.44±7.36)分,中、重度占68.50%。与RLS(-)组相比,RLS(+)组患者中肾移植失败后透析人数多(42/31 vs.44/69),而应用铁剂及进行血滤治疗的人数少(28/45 vs.61/52,51/22 vs.96/17),差异均有统计学意义(P<0.05)。RLS(+)组患者双下肢受累者占89.04%,单侧下肢受累占5.48%,上肢受累占2.74%,四肢受累占2.74%。肌肉内部不适患者占所有患者的80.82%,肢体表面不适患者占8.22%。结论尿毒症患者继发RLS与肾移植失败、不应用铁剂及血液滤过相关。RLS主要累及双下肢,以肌肉内部不适为主。  相似文献

目的:观察调神针刺法治疗不安腿综合征及其伴发睡眠障碍、焦虑情绪的临床疗效,为调神法在临床上的应用提供理论依据。方法:将60例患者随机分为观察组和对照组,每组30例。对照组予常规针刺治疗,穴取足三里、阳陵泉、悬钟等下肢局部腧穴;观察组在对照组取穴基础上加用百会、神庭、本神。两组均每日治疗1次,每周6d,1周为一疗程,连续治疗3个疗程。观察两组患者治疗前后国际不安腿综合征评定量表(IRLS)、匹兹堡睡眠质量指数(PSQI)和汉密尔顿焦虑量表(HAMA)评分变化。结果:治疗后两组患者IRLS评分、PSQI和HAMA总评分均较治疗前下降(P<0.05),且观察组治疗后各评分改善均优于对照组(P<0.05)。结论:在常规针刺基础上,调神针刺法可明显减轻不安腿综合征患者的下肢不适感,改善其睡眠障碍和焦虑情绪,疗效优于单纯常规针刺。  相似文献

BackgroundDistal one – third trauma of the lower limb is a complex condition to treat. The reverse sural flap is a time tested procedure for reconstruction of such defects especially in patients where free flaps are ruled out due to comorbidities. The purpose of this study is to compare the two modifications of the classical technique of reverse sural flap (adipofascial and fasciocutaneous) which is lacking in the literature.Material & methodsIn this study, the authors conducted a retrospective analysis of 67 patients with lower one-third leg defects reconstructed with either adipofascial reverse sural flap (Group A, n = 37) or two-staged fasciocutaneous reverse sural flap (Group B, n = 30) in a tertiary care hospital in North India between 2015 and 2019. An evaluation of the different flap characteristics of the two variants of the reverse sural flap was done and compared. Mean follow up period was 12 months.ResultsThe adipofascial group showed shorter operative time, was a single-stage and with better reach and aesthetic outcome. The complications did not differ except that for the adipofascial group was associated with unstable skin graft over the flap initially which did not require any treatment.DiscussionLower one-third defect of the lower limb has been a challenge for reconstructive surgeons all over the world. The goal of reconstruction is a functional lower limb. Although free tissue transfer is the preferred modality of treatment of such cases but it may not be possible in all cases due to various reasons. Reverse sural flap is a very lucrative local option for such reconstructions as it is easy to perform, reliable, low profile and bulk, require minimal facilities with less operative time. Adipofascial flaps represent an extremely useful modification of the reverse sural flap which is quick to perform with minimal donor site morbidity.ConclusionAdipofascial reverse sural artery flap is a good option for patients with lower limb trauma with the added advantage of being single-stage and with better donor site cosmesis as compared to the fasciocutaneous reverse sural artery flap.  相似文献
