We report a rare case of renal carcinoma metastasis involving a lumbar nerve root. Metastases to nerve roots are rare occurrences, and to our knowledge, only six cases have been reported so far in the literature. The patient in this report presented with weakness in the right lower limb and intractable pain irradiating along the L5 dermatome. MRI findings revealed a right-sided L5 nerve root mass, suggestive of a schwannoma, involving the spinal ganglion and its extraforaminal region. Complete macroscopic resection of this mass was performed, and histopathologic analysis confirmed the lesion to be a metastasis of a renal clear cell carcinoma. Local radiotherapy was given and tyrosine kinase inhibitors administered. At 5 months, the patient was pain-free and his right limb weakness had completely resolved. A tumoral recurrence could be observed on the control MRI 5 months after surgery. This report presents the first case of a patient with a renal clear cell carcinoma metastasis to a L5 nerve root, as well as a brief review of previous cases of metastases to peripheral nerve roots. 相似文献

The purpose of this study is to examine the projectile penetration resistance of the base metal and heat-affected zones of armor steel weldments. To ensure the proper quality of armor steel welded joints and associated ballistic protection, it is important to find the optimum heat input for armor steel welding. A total of two armor steel weldments made at heat inputs of 1.29 kJ/mm and 1.55 kJ/mm were tested for ballistic protection performance. The GMAW welding carried out employing a robot-controlled process. Owing to a higher ballistic limit, the heat-affected zone (HAZ) of the 1.29 kJ/mm weldment was found to be more resistant to projectile penetration than that of the 1.55 kJ/mm weldment. The ballistic performance of the weldments was determined by analyzing the microstructure of weldment heat-affected zones, the hardness gradients across the weldments and the thermal history of the welding heat inputs considered. The result showed that the ballistic resistance of heat affected zone exist as the heat input was decreased on 1.29 kJ/mm. It was found that 1.55 kJ/mm does not have ballistic resistance. 相似文献

Cell-free (cf) DNA in the plasma of cancer patients offers an easily obtainable source of biologic material for mutation analysis. Plasma samples from 157 patients with advanced cancers who progressed on systemic therapy were tested for 21 mutations in BRAF, EGFR, KRAS, and PIK3CA using the BEAMing method and results were compared to mutation analysis of archival tumor tissue from a CLIA-certified laboratory obtained as standard of care from diagnostic or therapeutic procedures. Results were concordant for archival tissue and plasma cfDNA in 91% cases for BRAF mutations (kappa = 0.75, 95% confidence interval [CI] 0.63 – 0.88), in 99% cases for EGFR mutations (kappa = 0.90, 95% CI 0.71– 1.00), in 83% cases for KRAS mutations (kappa = 0.67, 95% CI 0.54 – 0.80) and in 91% cases for PIK3CA mutations (kappa = 0.65, 95% CI 0.46 – 0.85). Patients (n = 41) with > 1% of KRAS mutant cfDNA had a shorter median survival compared to 20 patients with </= 1% of KRAS mutant DNA (4.8 vs. 7.3 months, p = 0.008). Similarly, 67 patients with > 1% of mutant cfDNA (BRAF, EGFR, KRAS, or PIK3CA) had a shorter median survival compared to 33 patients with </= 1% of mutant cfDNA (5.5 vs. 9.8 months, p = 0.001), which was confirmed in multivariable analysis. 相似文献

Background
Augmented reality technology has been used for intraoperative image guidance through the overlay of virtual images, from preoperative imaging studies, onto the real-world surgical field. Although setups based on augmented reality have been used for various neurosurgical pathologies, very few cases have been reported for the surgery of arteriovenous malformations (AVM). We present our experience with AVM surgery using a system designed for image injection of virtual images into the operating microscope’s eyepiece, and discuss why augmented reality may be less appealing in this form of surgery.Methods
N?=?5 patients underwent AVM resection assisted by augmented reality. Virtual three-dimensional models of patients’ heads, skulls, AVM nidi, and feeder and drainage vessels were selectively segmented and injected into the microscope’s eyepiece for intraoperative image guidance, and their usefulness was assessed in each case.Results
Although the setup helped in performing tailored craniotomies, in guiding dissection and in localizing drainage veins, it did not provide the surgeon with useful information concerning feeder arteries, due to the complexity of AVM angioarchitecture.Conclusion
The difficulty in intraoperatively conveying useful information on feeder vessels may make augmented reality a less engaging tool in this form of surgery, and might explain its underrepresentation in the literature. Integrating an AVM’s hemodynamic characteristics into the augmented rendering could make it more suited to AVM surgery. 相似文献8.
