A 70‐year‐old man with a history of coronary artery bypass grafting 15 years back and arteriovenous (AV) fistula creation in the left arm 1 month back presented with acute coronary syndrome (ACS). He had not received dialysis before his referral. We felt the most likely etiology for these complaints was increased cardiac oxygen demand from an increased cardiac output related to the newly formed left AV fistula. Coronary angiography was done to detect any significant stenosis in the native or grafted vessels. This revealed that the left subclavian artery was totally occluded in the ostioproximal segment and the coronary arteries did not have occlusions to explain the ACS setting. CT angiography confirmed the angiographic findings of the totally occluded left subclavian artery followed by a well‐developed and patent left internal mammary artery to left anterior descending artery. This led to the consideration of a steal syndrome from the coronary artery by the subclavian artery distal to the occlusion. A successful percutaneous endovascular intervention on the left subclavian artery occlusion was performed. Subsequently, the patient became asymptomatic and experienced a dramatic increase in left ventricular ejection fraction. 相似文献

The connection between pancreatic cancer and venous thrombosis has been discussed for almost 150 years. The exact pathophysiological mechanisms are still partly understood, but it is known that pancreatic cancer induces a prothrombotic and hypercoagulable state and genetic events involved in neoplastic transformation (e.g., KRAS, c-MET, p53), procoagulant factors [e.g., tissue factor (TF), platelet factor 4 (PF4), plasminogen activator inhibitor type 1 (PAI-1)], mucin production (e.g., through activation of P- and L-selectin) and pro-inflammatory factors [e.g., cytokines, cyclooxygenase-2 (COX-2)] may be implicated. Also pancreatitis, both acute and chronic, is associated with increased risk of venous thrombosis, but in this circumstance a direct inflammatory process may be more important. This article discusses the incidence, treatment and outcome of venous thromboembolism (VTE) complicating pancreatic disease, with special emphasis on new knowledge obtained during the last fifteen years. 相似文献

Dietary supplements and foods can interact with various drugs, leading to possible clinical concerns. This study aimed to investigate the effect of orally administered sinapic acid (SA) on the pharmacokinetics of aripiprazole (APZ) in rats and its possible modulatory effects on hepatic cytochrome P450 (CYP3A2 and CYP2D6) expression in the liver tissues. Single dose and multiple dose parallel groups of wistar rats were categorized into six groups (n = 6 each) which abstained from food for 12 h prior to the experiment, while water was allowed ad libitum. The investigation was carried out for single dose: Group I was treated with normal saline orally for 15 days (normal control). Group II was administered normal saline orally for 15 days and received APZ (3 mg/kg p.o.) on day 15. Group III received SA (20 mg/kg p.o.) for 15 days and received APZ (3 mg/kg p.o.) on day 15. Group IV was treated with SA (20 mg/kg p.o.) for 15 days. For the multiple dose study, Group I was treated with normal saline orally for 15 days (normal control); Group II received APZ (3 mg/kg p.o.) daily for 15 days; Group III was administered with SA (20 mg/kg p.o.) and APZ (3 mg/kg p.o.) for 15 days and Group IV received SA (20 mg/kg p.o.) for 15 days. The group I and IV were kept common in single and multiple dose groups. After last APZ dose, plasma samples were collected and APZ concentrations were determined using an UPLC-MS/MS technique. The pharmacokinetic parameters were calculated using a non-compartmental analysis. The concomitant administration of APZ with SA (as single or multiple dose) resulted in an increase in APZ absorption and a decrease on its systemic clearance. This was associated with a reduction in CYP3A2 and CYP2D6 protein expressions by 33–43% and -71–68% after the single and multiple co-administration, which are two enzymes responsible of the metabolism of APZ. Therefore, a reduction in the metabolic clearance appears to be the mechanism underlying the drug interaction of dietary supplement containing SA with APZ. Therefore, the concomitant administration of SA and APZ should be carefully viewed. Further investigations are required to assess the clinical significance of such observations in humans. 相似文献

BACKGROUND: The retention and growth of cariogenic microorganisms on toothbrushes pose a threat of recontamination. The authors to studied three species of oral microorganisms found at different places on toothbrush filaments. METHODS: The authors tested on patients 30 toothbrushes each of three different toothbrush types made by a single manufacturer. The toothbrushes were divided into three groups by type of construction: staple-set tufting (toothbrush A); in-mold tufting (toothbrush B); individual in-mold placement of filaments (toothbrush C). Subjects used the toothbrushes once under standardized conditions; the authors subsequently examined the brushes for the presence of Streptococcus mutans, lactobacilli and Candida species. The inspection was carried out at three time intervals after use. RESULTS: S. mutans was retained to the highest degree, followed by lactobacilli and finally by C. albicans. The authors found that the number of microorganisms on toothbrush types A and B did not reveal a significant difference either on examination immediately after use or after the toothbrushes had been dried for two hours or eight hours. The technique of individual in-mold placement of filaments made retention of microorganisms more difficult. The difference between the number of germs retained on toothbrush types A and C, as well as that between the number of germs retained on types B and C, was significant or even highly significant. CONCLUSIONS: The results show that toothbrushes made with the technique of individual in-mold placement of filaments appear to retain the least amount of microbial material. CLINICAL IMPLICATIONS: Owing to the fact that toothbrushes always are a possible source of microbial reinfection, the arrangement of the filaments within the head of the toothbrush is of great importance with regard to hygiene. 相似文献

The clonal relationship between oral and fecal Candida albicans isolated from children of pre‐school age was examined using RAPD analysis. Significantly higher levels of C. albicans were found in saliva, dental plaque, carious specimens and stools of 56 patients with severe caries as compared to 52 healthy control subjects. The highest prevalence was found in carious specimens and a strong correlation was observed between its presence in saliva, dental plaque, carious specimen and feces. RAPD analysis of isolates from 23 patients with simultaneous oral and fecal C. albicans revealed clonal counterparts present in both oral and stool samples in 15 cases; five patients harbored closely related strains; and three patients harbored unrelated strains. Our results demonstrate a strong correlation between oral and gastrointestinal C. albicans colonization. We assume that carious teeth may constitute an ecologic niche for C. albicans potentially responsible for recurrent oral and non‐oral candidiasis. 相似文献

Effects of one-year storage in water on bond strength of self-etching adhesives to enamel and dentin

The aim of this study was to compare the bond strengths of three self-etching materials during one year of storage. Clearfil SE Bond (SE), Clearfil Protect Bond (PB), and Clearfil Tri-S Bond (TS) were used for bonding to dentin and enamel according to manufacturer's instructions. Microshear bond strength values were measured after 24 hours, six months, and one year. Two-way ANOVA showed that the interaction of material type and storage time was significant for dentin. At baseline, SE had the highest bond strength to dentin. There were no significant changes in bond strength for each material during the storage period, except for PB which showed increased bond strength to dentin after one year. All materials performed reliably after one year. However, the antibacterial and fluoride-releasing effects of PB would further contribute to the long-term clinical benefits of this material. 相似文献
