OBJECTIVE: In recent years, platelet-rich plasma combined with graft materials has been used for periodontal regeneration. The individual role of blood products with guided tissue regeneration in periodontal regenerative therapy is unclear and needs to be elucidated. The purpose of this study was to compare the clinical and radiological effectiveness of platelet pellet/guided tissue regeneration (PP/GTR) and bioactive glass/GTR (BG/GTR) treatments in patients with periodontal disease. MATERIAL AND METHODS: Using a split mouth design, 15 chronic periodontitis patients with pocket depths > or = 6 mm following periodontal initial therapy were randomly assigned to treatment with a combination of PP/GTR or BG/GTR in contralateral dentition areas. An absorbable membrane of polylactic acid was used GTR. The criteria for the comparative study were preoperative and postoperative 6 months pocket depth, clinical attachment level, and radiological alveolar bone level. RESULTS: Both treatment modalities resulted in significant pocket depth reduction and gain in clinical attachment and alveolar bone level compared to the preoperative values (p < 0.01). Reduction in pocket depth, gain in clinical attachment and alveolar bone level were 4(3-6), 4.1+/-0.7, 4.9+/-1.4 mm in the PP/GTR group and 4(3-7), 4.1+/-1.2, 5.9+/-1.7 mm in the BG/GTR group, respectively. The differences between the two groups were not statistically significant (p > 0.05). CONCLUSIONS: Within the limits of this study, it was concluded that PP may be effective as a bioactive glass graft material and used as a graft material for treating intrabony defects. PP thus appears to be a suitable alternative in the regenerative treatment of intrabony periodontal defects.  相似文献

The surface of zirconia may be damaged during grinding, influencing the mechanical properties of the material. The purpose of this study was to compare the flexural strength of zirconia after different grinding procedures. Twenty bar-type zirconia specimens (21 x 5 x 2 mm) were divided into 4 groups and ground using a high-speed handpiece or a low-speed straight handpiece until the bars were reduced 1 mm using two different grinding times: continuous grinding and short-time grinding (n=5). Control specimens (n=5) were analyzed without grinding. The flexural strengths of the bars were determined by using 3-point bending test in a universal testing machine at a crosshead speed of 0.5 mm/min. The fracture load (N) was recorded, and the data were analyzed statistically by the Kruskal Wallis test at a significance level of 0.05. In the test groups, high-speed handpiece grinding for a short time had produced the highest mean flexural strength (878.5 ± 194.8 MPa), while micromotor continuous grinding produced the lowest mean flexural strength (733.8 ± 94.2 MPa). The control group was the strongest group (928.4 ± 186.5 MPa). However, there was no statistically significant differences among the groups (p>0.05). Within the limitations of the study, there was no difference in flexural strength of zirconia specimens ground with different procedures.  相似文献

Functional limitations for the vitamin K cycle, caused either by mutations in gamma-glutamyl carboxylase or vitamin K epoxide reductase genes, result in hereditary deficiency of vitamin K-dependent coagulation factors (VKCFD1 and VKCFD2, respectively). Patients suffering from VKCFD often share several other anatomical irregularities which are not related to haemostasis. Here we report on nine patients, eight of them previously unreported, who presented with VKCFD1. All were examined with special attention to vitamin K-dependent coagulation factors as well as to bone and heart development and to other anatomical signs of embryonal vitamin K deficiency. In total, we detected ten mutations in the gamma-glutamyl carboxylase gene of which seven have not been previously reported. Most interestingly, additional non-bleeding phenotypes were observed in all patients including midfacial hypoplasia, premature osteoporosis, cochlear hearing loss, heart valve defects, pulmonary stenosis, or pseudoxanthoma elasticum-like phenotype. Undercarboxylated matrix Gla protein, osteocalcin, and periostin appear to be responsible for these defects which are also observed in cases of fetal warfarin syndrome.  相似文献

Introduction

We compared the effects of 6 different rotary systems on transportation, canal curvature, centering ratio, surface area, and volumetric changes of curved mesial root canals of mandibular molar via cone-beam computed tomographic (CBCT) imaging.

Methods

Mesiobuccal root canals of 120 mandibular first molars with an angle of curvature ranging from 20°–40° were divided into 6 groups of 20 canals. Based on CBCT images taken before instrumentation, the groups were balanced with respect to the angle and radius of canal curvature. Root canals were shaped with the following systems with an apical size of 25: OneShape (OS) (MicroMega, Besancon, France), ProTaper Universal (PU) F2 (Dentsply Maillefer, Ballaigues, Switzerland), ProTaper Next X2 (Dentsply Maillefer), Reciproc (R) R25 (VDW, Munich, Germany), Twisted File Adaptive (TFA) SM2 (SybronEndo, Orange, CA), and WaveOne primary (Dentsply Tulsa Dental Specialties, Tulsa, OK). After root canal preparation, changes were assessed with CBCT imaging. The significance level was set at P = .05.

Results

The R system removed a significantly higher amount of dentin than the OS, PU, and TFA systems (P < .05). There was no significant difference among the 6 groups in transportation, canal curvature, changes of surface area, and centering ratio after instrumentation.

Conclusions

The 6 different file systems straightened root canal curvature similarly and produced similar canal transportation in the preparation of mesial canals of mandibular molars. R instrumentation exhibited superior performance compared with the OS, TFA, and PU systems with respect to volumetric change.  相似文献

Introduction

The aim of the present study was to evaluate crack formation after flaring root canals with Gates Glidden drills and ProTaper Universal (SX; Dentsply Maillefer, Ballaigues, Switzerland), Endoflare (MicroMega, Besançon, France), Revo-S (MicroMega), and HyFlex (Coltene-Whaledent, Allstetten, Switzerland) flaring instruments.

Methods

One-hundred eight mandibular molars were selected. Eighteen teeth were left unprepared to serve as negative controls; the experimental groups consisted of the mesiobuccal and mesiolingual root canals of the remaining 90 teeth, which were instrumented with the following coronal flaring instruments: Gates Glidden drills and ProTaper Universal SX, Endoflare, Revo-S SC1, and HyFlex 25.08 instruments. All roots were then sectioned perpendicular to the long axis at 1, 2, 3, 4, 6, and 8 mm from the cementoenamel junction. The sections were inspected under a stereomicroscope, and any crack formations were recorded. The data were analyzed using the chi-square test (P = .05).

Results

The Gates Glidden drills resulted in a higher rate of crack formation than that noted in the control group (P < .05). Flaring of the root canals using the ProTaper Universal, Endoflare, Revo-S, and HyFlex instruments resulted in crack formation similar to that of the control group (P > .05).

Conclusions

The use of the Gates Glidden drills resulted in the formation of the most cracks. However, the results for the ProTaper Universal, Endoflare, Revo-S, and HyFlex flaring instruments were similar to those of the control group in terms of crack formation.  相似文献

Introduction

The purpose of this study was to evaluate the fracture strength of roots instrumented with the Self-Adjusting File (SAF; ReDent-Nova, Ra’anana, Israel) and the ProTaper system (Dentsply Maillefer, Ballaigues, Switzerland) and filled with the cold lateral compaction technique.

Methods

In total, 50 mandibular premolar teeth were sectioned at or below the cementoenamel junction to obtain roots 13 mm in length. The roots were balanced with respect to buccolingual-mesiodistal diameters and weight. They were distributed into 4 experimental groups and 1 control group (n = 10): no instrumentation (control group), instrumentation with ProTaper rotary files but no filling (PT), instrumentation with ProTaper rotary files and filling with cold lateral compaction (PT filling), instrumentation with SAF but no filling (SAF), and instrumentation with SAF and filling with cold lateral compaction (SAF filling). AH Plus sealer (Dentsply DeTrey, Konstanz, Germany) was used along with gutta-percha points. One week later, a vertical load was applied to the specimen’s canal until fracture occurred. Data were statistically analyzed using 1-way analysis of variance (P = .05).

Results

The mean fracture load was 244 ± 51 N for the control group, 226 ± 21 N for the PT, 242 ± 45 N for PT filling, 233 ± 14 N for the SAF, and 271 ± 44 N for SAF filling. However, the differences were not statistically significant (P > .05).

Conclusions

Instrumentation with the SAF or the ProTaper rotary system did not change the fracture strength of standardized roots with respect to cross-sectional diameter and weight.  相似文献

The papillary muscles of the heart are conical projections into the respective chamber of myocardium covered by endocardium. Functionally, the papillary muscles are important part of the respective valve complex. They prevent the cusps of a valve from being everted when the ventricle contracts. Our study was conducted on papillary muscles in left ventricle of 135 normal adult hearts (20 humans, 25 dogs, 60 sheep and 30 goats). The length (L), width (W), head number (HN), angle with ventricular wall (A degrees ), distance with annulus (AnD) and apex (ApD), and the shapes of the anterior and posterior papillary muscles were observed. The tendinous chords (CTN) attached to each papillary muscle were counted at their origin. The values of both anterior and posterior papillary muscles were not statistically significant (p > 0.05) in measurements of L, W, HN, A degrees and AnD of all species, but those of CTN and ApD were significant (p < 0.05) between human and animal hearts, while the difference was not significant (p > 0.05) between sheep and goat hearts. Knowledge of morphology of papillary muscles in different species will be of much use to clinical investigators in their studies.  相似文献

Background

Until now there have been no prospective studies describing the results of using the superior clavicle plate with lateral extension in patients with displaced lateral clavicle fractures (Neer type 2). The purpose of this study was to evaluate the results of applying this plate for this specific type of fracture.

Materials and methods

In this prospective study, seven patients (mean age 43, M:F; 6:1) with a fresh displaced lateral clavicle fracture were evaluated with a mean follow-up of 10 months. Analysis included functional and subjective outcome, time until union, time until return to work, and complications.

Results

All patients achieved clinical and radiological union within 6–12 weeks. Full range of motion as well as a return to work was achieved in most cases within 2 weeks. The mean Constant score was 98 (range 90–100), the DASH score was 3.6 (range 0–11.4), and the Shoulder Rating Questionnaire score was 97 (range 96–100). No major complications were encountered. Three patients required plate removal: two because of a prominent and subcutaneous plate and one because of an intra-articular screw.

Conclusions

In this study, use of the superior clavicle plate with lateral extension yielded excellent results in the treatment of this difficult fracture. In particular, patients acquired full range of motion within 2 weeks, reflecting the stability of the osteosynthesis achieved with this implant.  相似文献
