The aim of this study was to investigate knowledge of PMTCT and to describe potential barriers that might affect acceptability of interventions for PMTCT in a resource poor setting in South Africa. The sample included 1534 pregnant women recruited at first antenatal care visit from 5 clinics implementing PMTCT (61%) and from 5 communities around the 5 clinic areas (39%). In addition, the mothers or mothers-in-law (70.9%) and husbands or partners (58.2%) of the pregnant women were interviewed at their homes. Results indicate that major potential barriers in implementing PMTCT programmes in a resource poor setting included physical access to the health facility, PMTCT knowledge, stigma and support, HIV testing, and delivery preference.  相似文献

BackgroundMany of the markets in Nigeria are open, where foodstuffs are laid bare on flat trays and open baskets, directly exposing them to environmental contaminants. This study aimed at determining whether some food items on sale around an industrialized area of Ogun State are contaminated with trace metals.MethodsSeven different food items – Clarias gariepinus (roasted, fresh and smoked) Bos taurus (dried and fresh beef), zobo leaf (Hibiscus sabdariffa) and crayfish (Astacus leptodactylus) were sampled from three major markets, namely: Lusada, Atan and Ota in Ado-Odo/Ota LGA of Ogun State. They were analyzed for Lead, Cadmium, Nickel, Manganese and Zinc using Atomic Absorption Spectroscopy.ResultsCd was not detected in most of the food items across the markets. Pb, Ni and Mn were detected in very high concentrations above the maximum allowable limits by international regulatory agencies. Zn was the only metal that was generally below regulatory limits in food items across the three markets. The Estimated Daily Intake (EDI) and Target Hazard Quotient (THQ) values were generally higher than values acceptable in food.ConclusionSome of the food items consumed in this area are not entirely safe from metal toxicity and this may have serious health consequences.  相似文献

AIDS stigmas interfere with HIV prevention, diagnosis, and treatment and can become internalized by people living with HIV/AIDS. However, the effects of internalized AIDS stigmas have not been investigated in Africa, home to two-thirds of the more than 40 million people living with AIDS in the world. The current study examined the prevalence of discrimination experiences and internalized stigmas among 420 HIV-positive men and 643 HIV-positive women recruited from AIDS services in Cape Town, South Africa. The anonymous surveys found that 40% of persons with HIV/AIDS had experienced discrimination resulting from having HIV infection and one in five had lost a place to stay or a job because of their HIV status. More than one in three participants indicated feeling dirty, ashamed, or guilty because of their HIV status. A hierarchical regression model that included demographic characteristics, health and treatment status, social support, substance use, and internalized stigma significantly predicted cognitive-affective depression. Internalized stigma accounted for 4.8% of the variance in cognitive-affective depression scores over and above the other variables. These results indicate an urgent need for social reform to reduce AIDS stigmas and the design of interventions to assist people living with HIV/AIDS to adjust and adapt to the social conditions of AIDS in South Africa.  相似文献

Multiple HIV-1-specific cytokine and proliferative responses by CD4(+) T cells have not been studied in acutely infected infants. Using an intracellular cytokine staining assay, 34 untreated clade C HIV-1-infected infants (2-102 days old) were assessed for IFN-gamma, 28/34 for IL-2, and 26/34 for TNF-alpha responses to all HIV-1 proteins. Responses were detected in 29%, 36%, and 15% of infants, respectively. Twelve of the original 34 infants were then studied longitudinally for 14 months to determine the effect of viral load on IFN-gamma Gag-specific responses: seven infants were treated for 1 year, stopped treatment, and resumed when CD4% was < 20 and five infants were treated only when the CD4% was <20. Following treatment cessation, there was an immediate increase in viral load followed by an increase in the magnitude of CD4(+) Gag-specific responses. Despite this, the majority of infants (54%) had to restart treatment by 24 months of age, indicating that the immune responses were antigen driven but not associated with protection. Among untreated infants HIV-specific CD4(+) responses were detected sporadically indicating a dysfunctional immune response in the face of constant exposure to high levels of viremia.  相似文献

Background

Disparities in use of healthcare services between rural and urban areas have been empirically attributed to several factors. This study explores the existence of this disparity and its implication for planning and managing healthcare delivery systems.

Objectives

The objectives determine the relative importance of the various predisposing, enabling, need and health services factors on utilization of health services; similarity between rural and urban areas; and major explanatory variables for utilization

Method

A four-stage model of service utilization was constructed with 31 variables under appropriate model components. Data is collected using cross-sectional sample survey of 1086 potential health services consumers in selected health facilities and resident milieu via questionnaire. Data is analyzed using factor analysis and cross tabulation.

Results

The 4-stage model is validated for the aggregate data and data for the rural areas with 3-stage model for urban areas. The order of importance of the factors is need, enabling, predisposing and health services. 11 variables are found to be powerful predictors of utilization.

Conclusion

Planning of different categories of health care facilities in different locations should be based on utilization rates while proper management of established facilities should aim to improve health seeking behavior of people.Keywords/Phrases: Utilization, healthcare Delivery System, Planning, Model, Factor Analysis, Rural and Urban Areas  相似文献

In this study, bamboo impregnated with nanoscale zero-valent iron (nZVI) and nanoscale manganese (nMn) were prepared by the aqueous phase borohydride reduction method and characterized using scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR) and PIXE analysis. The synthesized nMn-bamboo and nZVI-bamboo composites were subsequently applied to the sorption of methylene blue (MB) dye from aqueous solution. The adsorption of MB dye was investigated under various experimental conditions such as pH, contact time, initial concentration of MB dye and adsorbent dosage. The results showed that the synthesized nZVI-bamboo composite was more effective than nMn-bamboo composite in terms of higher MB dye adsorption capacity of 322.5 mg/g compared to 263.5 mg/g of nMn-bamboo composite. At a concentration of 140 mg/L MB dye, 0.02 g of nZVI-bamboo and nMn-bamboo composites resulted in 79.6% and 78.3% removal, respectively, at 165 rpm, contact time of 120 min and at a solution pH of 7.6. The equilibrium data was best represented by Freundlich isotherm model and the pseudo-second order kinetic model better explained the kinetic data for both nZVI-bamboo and nMn-bamboo composites.  相似文献

In this study, the use of ultra-violet (UV) light with or without iron oxide nanoparticles (IONPs) for the degradation of synthetic petroleum wastewater was investigated. The IONPs was synthesised by sodium borohydride reduction of ferric chloride solution and was characterised by scanning electron microscopy (SEM), transmission electron microscopy (TEM), Fourier transform infrared spectrometry (FTIR), x-ray fluorescence spectrophotometry (XRF), and energy dispersive spectroscopy (EDS). The amount of degradation was evaluated by chemical oxygen demand (COD) determination. Experimental results show that the COD removal from synthetic petroleum wastewater by IONPs/UV system was more effective than they were independently. The combination of UV light at a wavelength of 254 nm, pH of 8, and 1.0 g of IONPs resulted in COD removal from 10.5% up to 95.5%. The photocatalytic degradation of synthetic petroleum wastewater is about 1.3–2.0 times faster in comparison to UV light only. The removal of COD from synthetic petroleum wastewater by UV light and IONPs follows the pseudo-first-order kinetic model with rate constant k ranging from 0.0133 min−1 to 0.0269 min−1. Consequently, this study has shown that the use of UV light in the presence of IONPs is favourable and effective for the removal of organic pollutants from petroleum refinery wastewater.  相似文献

Mobile phones work by transmitting and receiving radio frequency microwave radiation. The radio frequency (RF) emitted by mobile phones is stronger than FM radio signal which are known to cause cancer. Though research and evidence available on the risk of cancer by mobile phones does not provide a clear and direct support that mobile phones cause cancers. Evidence does not also support an association between exposure to radio frequency and microwave radiation from mobile phones and direct effects on health. It is however clear that lack of available evidence of cancer as regards the use of mobile phone should not be interpreted as proof of absence of cancer risk, so that excessive use of mobile phones should be taken very seriously and with caution to prevent cancer.  相似文献
