Pre-operative counselling is an important part of the care that we give to our patients undergoing surgery. It ensures that the patients understand about their disease and the procedure that they are undergoing. This study was done with the objective of finding whether or not the patients do actually understand what has been explained to them. This is a prospective study done in the form of a survey with a set questionnaire in two hospitals in Kathmandu namely Patan hospital and Tribhuvan University Teaching Hospital. The patients surveyed were those who were admitted for elective surgery after counselling has been done and consent forms signed. The result showed that the level of understanding was poor in 11%, unsatisfactory in 33%, satisfactory in 37% and excellent in 19%. The level of understanding was affected by the education level of the patients. To conclude, the level of understanding was unsatisfactory in nearly half the patients. Only 19% had optimal understanding. This may reflect the education level of the many of our patients but it clearly points out that the medical profession needs to make more effort to make the patients understand more about their disease process. This study determined the effects of a high-fat meal on circulating endotoxin and cardiometabolic indices in adult Arab women. The cohort consisted of 92 consenting Saudi women (18 non-diabetic (ND)) control subjects; Age 24.4 ± 7.9 year; body mass index (BMI) 22.2 ± 2.2 Kg/m2), 24 overweight/obese (referred to as overweight-plus (overweight+)) subjects (Age 32.0 ± 7.8 year; BMI 28.5 ± 1.5 Kg/m2) and 50 type 2 diabetes mellitus (T2DM) patients (Age 41.5 ± 6.2 year; BMI 35.2 ± 7.7 Kg/m2). All were given a high-fat meal (standardized meal: 75 g fat, 5 g carbohydrate, 6 g protein) after an overnight fast of 12–14 h. Anthropometrics were obtained and fasting blood glucose, lipids, and endotoxin were serially measured for four consecutive postprandial hours. Endotoxin levels were significantly elevated prior to a high-fat meal in the overweight+ and T2DM than the controls (p < 0.05). Furthermore, the postprandial cardiometabolic changes led to a more detrimental risk profile in T2DM subjects than other groups, with serial changes most notable in glucose, triglycerides, high density lipoprotein-cholesterol (HDL-cholesterol), and insulin levels (p-values < 0.05). The same single meal given to subjects with different metabolic states had varying impacts on cardiometabolic health. Endotoxemia is exacerbated by a high-fat meal in Arab subjects with T2DM, accompanied by a parallel increase in cardiometabolic risk profile, suggesting disparity in disease pathogenesis of those with or without T2DM through the altered cardiometabolic risk profile rather than variance in metabolic endotoxinaemia with a high-fat meal. 相似文献

BackgroundNasolabial flap is reliable flap in the reconstruction of oral defects over a period of time. Still there is scanty literature available of using this flap for reconstruction of isolated defects of tongue. We carried out this study in our patients to assess the role of pedicled nasolabial flap in reconstruction of isolated tongue defects.MethodsIn total, 11 patients with T1 and T2 tongue cancer were selected for the study. The functional improvement in the form of speech and swallowing was evaluated postoperatively.ResultsThe flap was successfully taken in all patients except for marginal or tip loss. This is a locally available flap with minimal operating time and does not require microvascular skills. The results of speech and swallowing after reconstruction were comparable.ConclusionNasolabial flap is an excellent locally available flap for the reconstruction of the anterior two-thirds of the tongue and with very minor, if any postoperative cosmetic defect. 相似文献

4D printed objects are indexed under additive manufacturing (AM) objects. The 4D printed materials are stimulus-responsive and have shape-changing features. However, the manufacturing of such objects is still a challenging task. For this, the designing space has to be explored in the initial stages, which is lagging so far. This paper encompasses two recent approaches to explore the conceptual design of 4D printed objects in detail: (a) an application-based modeling and simulation approach for phytomimetic structures and (b) a voxel-based modeling and simulation approach. The voxel-based modeling and simulation approach has the enhanced features for the rapid testing (prior to moving into design procedures) of the given distribution of such 4D printed smart materials (SMs) while checking for behaviors, particularly when these intelligent materials are exposed to a stimulus. The voxel-based modeling and simulation approach is further modified using bi-exponential expressions to encode the time-dependent behavior of the bio-inspired 4D printed materials. The shape-changing materials are inspired from biological objects, such as flowers, which are temperature-sensitive or touch-sensitive, and can be 4D printed in such a way that they are encrypted with a decentralized, anisotropic enlargement feature under a restrained alignment of cellulose fibers as in the case of composite hydrogels. Such plant-inspired architectures can change shapes when immersed in water. This paper also outlines a review of the 4D printing of (a) smart photocurable and biocompatible scaffolds with renewable plant oils, which can be a better alternative to traditional polyethylene glycol diacrylate (PEGDA) to support human bone marrow mesenchymal stem cells (hMSCs), and (b) a biomimetic dual shape-changing tube having applications in biomedical engineering as a bioimplant. Although rare, atrial myxoma is the most common primary tumour of the heart. Its relation to immunosuppression in solid organ transplant is presently debateable. We report the case of a 71-year-old male patient who underwent renal transplant 17 years prior. Since that time he continued high dose immunosuppression without physician consultation and presented to us with atrial myxoma and its complications raising the question of any association between immunosuppression and the development of atrial myxoma. 相似文献
