Introduction
Better diagnosis and early referral due to increased health care coverage have increased the cesarean deliveries at tertiary-care hospitals of India. Improvements in the health care system raise many concerns and need of cross-checking system in place to counter the problems pertaining to patient education and participation of patient. While most of the cesarean sections are done in good faith for the patient, it does not escape the purview of consumer awareness and protection.Materials and methods
This cross-sectional study was undertaken at a tertiary level government institution to understand the level of awareness of 220 patients regarding the various aspects of cesarean delivery which are essential for women to know before giving an informed consent.Results
71 % of the women had knowledge about the indication and need to do cesarean delivery. Of these, only one-third (25 % of total women) were properly explained about procedure and complications. Other demographic and social characteristics were also evaluated.Discussion
While the health care schemes have had their improved results, the onus lies upon the caregivers to improve and maintain the quality of health care in these tertiary-care government hospitals in proportion to the increase in patient load. The results of this study highlight the need for proper counseling of patients regarding complications of cesarean section. The fact that only 25 % of total cases were explained proper procedure and complication as opposed to 71 % of patients having proper knowledge about the indication of cesarean section points out the lack of information in seemingly “informed” consent.The way forward
To bring about awareness about the risks and complications of cesarean section, there is a need that patients be counseled during the antenatal visits, specifically when patients visit near term for antenatal check up. 相似文献6.

BACKGROUND: A recent clinical study suggested that restrictive tenosynovitis of the flexor hallucis longus (FHL) may play an important causative role in hallux rigidus. The goals of this research were to assess normal function of the FHL and the effect of restricted FHL gliding on the loading of the hallux metatarsophalangeal joint (MTPJ). We hypothesized that proximal displacement of the FHL would increase the forces carried by the FHL tendon, causing increased loading of the first metatarsal and MTPJ. MATERIALS AND METHODS: Dynamic simulations of the stance phase of walking were created by loading 5 non-embalmed, non-paired cadaver lower extremities (2 male, 3 female; mean age, 69; range, 56 to 75) in the Robotic Dynamic Activity Simulator (RDAS). The RDAS recreates lower leg motion and muscle activity based on kinematic and electromyographic data of healthy subjects. FHL tendon forces and excursions were measured for the entire stance phase of gait. Bone strains in the mid-shaft of the first metatarsal were recorded and used to derive the in situ forces and moments imposed on the metatarsal and the hallux MTPJ. RESULTS: Under force-feedback control, FHL excursion averaged 6.57 (+/-3.13) mm during gait. When the FHL was held 2-, 4-, and 6-mm proximal to the mid-point of normal excursion to simulate progressive stenosis, the forces in the FHL tendon, first metatarsal and first MTP joint were progressively, and significantly, increased (p < 0.05). CONCLUSION: These findings support the hypothesis that progressive fibrosis at the FHL myotendinous junction can cause increased loading of the hallux MTPJ, thereby leading to hallux rigidus. 相似文献

Although cyclooxygenase-2 (COX-2) inhibitors, such as the late stage development drug apricoxib, exhibit antitumor activity, their mechanisms of action have not been fully defined. In this study, we characterized the mechanisms of action of apricoxib in HT29 colorectal carcinoma. Apricoxib was weakly cytotoxic toward naive HT29 cells in vitro but inhibited tumor growth markedly in vivo. Pharmacokinetic analyses revealed that in vivo drug levels peaked at 2-4 μM and remained sufficient to completely inhibit prostaglandin E(2) production, but failed to reach concentrations cytotoxic for HT29 cells in monolayer culture. Despite this, apricoxib significantly inhibited tumor cell proliferation and induced apoptosis without affecting blood vessel density, although it did promote vascular normalization. Strikingly, apricoxib treatment induced a dose-dependent reversal of epithelial-mesenchymal transition (EMT), as shown by robust upregulation of E-cadherin and the virtual disappearance of vimentin and ZEB1 protein expression. In vitro, either anchorage-independent growth conditions or forced EMT sensitized HT29 and non-small cell lung cancer cells to apricoxib by 50-fold, suggesting that the occurrence of EMT may actually increase the dependence of colon and lung carcinoma cells on COX-2. Taken together, these data suggest that acquisition of mesenchymal characteristics sensitizes carcinoma cells to apricoxib resulting in significant single-agent antitumor activity. 相似文献
