Background  

Ankylosing spondylitis is a chronic inflammatory rheumatic disorder which usually begins in early adulthood. The diagnosis is often delayed by many years. MR imaging has become the preferred imaging method for detection of early inflammation of the axial skeleton in ankylosing spondylitis.  相似文献

The value of routinely used MR sequences in the detection of focal changes of femorotibial articular cartilage was studied. T1-weighted, proton density, and T2-weighted SE as well as gradient echo images were acquired in 20 cadaveric knees (56-88 years old, mean 73.8 years). Three hundred five coronal and sagittal (3 mm) anatomic sections were prepared, and 82 areas of cartilage defects were identified. Initially, in an unblinded fashion, correlation of MR scans and anatomic sections was performed. Fifty-nine lesions (72.0%) were detectable on T1-weighted images, 57 (70.0%) with meniscal windowing, 49 (60.0%) on proton density images, 56 (68.3%) on T2-weighted images, and 54 (65.9%) on gradient echo images. Sixty-eight (83.0%) were visible on at least one type of imaging sequence. Most defects presented as a focus of abnormal signal. Subsequently, images of a subset of 35 pathologic and 35 normal cartilage surfaces were blindly evaluated by two osteoradiology fellows. Sensitivity was 71.4% for the detection of focal cartilage changes, specificity was 68.6%, and accuracy was 70.0%. We conclude that the value of those MR sequences that are routinely used in the analysis of internal derangements of the knee in the detection of focal defects of the hyaline cartilage is limited.  相似文献

OBJECTIVE. Using anatomic and histologic correlation, we investigated the reasons for differences in the MR signal intensity between the normal anterior and posterior cruciate ligaments and for the focally increased signal intensity within these ligaments occasionally found in elderly patients. MATERIALS AND METHODS. The MR images of 18 anterior and 20 posterior cruciate ligaments obtained from cadavers (age at death, 56-88 years old; mean, 74 years) were evaluated blindly. Their overall signal intensity and focal regions of increased signal were noted. The findings then were correlated with corresponding anatomic and histologic sections. RESULTS. The anterior cruciate ligament had greater signal intensity than did the posterior cruciate ligament in 15 of 18 knees and had comparable signal intensity in three knees. No histologic basis to explain the different MR appearances of the anterior and posterior cruciate ligaments was found. Rather, differences in macroscopic anatomy appeared to be the best explanation. In the posterior cruciate ligament, the fibers were parallel, whereas they were diverging and twisted in the anterior cruciate ligament, causing volume-averaging artifacts. A focal area of signal increase was found in 29 of the 38 ligaments. In 17 of these 29 ligaments, mucoid and/or eosinophilic degeneration was found that appeared to have caused the focal MR signal change. In nine ligaments without focally increased signal on the MR images, only one had histologic evidence of degeneration. The predictive value of an abnormal finding on MR was 59%, and the predictive value of a normal finding on MR was 89%. The relationship between increased signal on MR and histologic degeneration was statistically significant (chi 2 test, p = .0126). CONCLUSION. The differences in the MR appearances of the anterior and posterior cruciate ligaments probably are due to differences in gross architecture. Increased MR signal within the cruciate ligaments occurring in elderly patients often can be attributed to degenerative changes.  相似文献

An anthropomorphic airway tree phantom was imaged with both hyperpolarized (HP) 3He MRI using a dynamic projection scan and computed tomography (CT). Airway diameter measurements from the HP 3He MR images obtained using a newly developed model-based algorithm were compared against their corresponding CT values quantified with a well-established method. Of the 45 airway segments that could be evaluated with CT, only 14 airway segments (31%) could be evaluated using HP 3He MRI. No airway segments smaller than approximately 4 mm in diameter and distal to the fourth generation were adequate for analysis in MRI. For the 14 airway segments measured, only two airway segments yielded a non-equivalent comparison between the two imaging modalities, while eight more had inconclusive comparison results, leaving only four airway segments (29%) that satisfied the designed equivalence criteria. Some of the potential problems in airway diameter quantification described in the formulation of the model-based algorithm were observed in this study. These results suggest that dynamic projection HP 3He MRI may have limited utility for measuring airway segment diameters, particularly those of the central airways.  相似文献

The Schizosaccharomyces pombe rad51+ and dmc1+ genes code for homologues of the Escherichia coli recombination protein RecA. Deletion of rad51+ causes slow growth, retardation of cell division and a decrease in viability. rad51 cells have a defect in mating-type switching. The DNA modification at the mating-type locus required for mating-type switching contributes to slow growth in the rad51 mutant. Cell mating is reduced in crosses homozygous for rad51. Ectopic expression of the dmc1+ gene allowed us to demonstrate that the reduction in meiotic recombination in dmc1 mutants is not caused by a disturbance of rad24 expression from the dmc1-rad24 bicistronic RNA. We describe the functional defects of terminally epitope-tagged Dmc1 and Rad51 and discuss it in terms of protein interaction. Presumptive Rad51 and Dmc1 foci were detected on spreads of meiotic chromatin.  相似文献

This article reviews the examination technique of shoulder ultrasound, normal and abnormal ultrasound findings in acute (posttraumatic) and chronic (degenerative) lesions. Moreover, it reviews the effectiveness of ultrasound in relation to magnetic resonance (MR) imaging. Most authors report that full-thickness tears of the supraspinatus can reliably be diagnosed by ultrasound. However, the simple diagnosis of a full-thickness rotator cuff tear is no longer sufficient for surgical management. The precise localization and size of rotator cuff tears as well as the extent of muscle degeneration is important for surgical planning. For this aspect and for partial-thickness tears of the supraspinatus, for subscapularis lesions as well as for lesions of the long biceps tendons there is no consensus regarding the diagnostic value of ultrasound. To the present, ultrasound (contrary to MR imaging) has failed to demonstrate that it consistently influences the clinician's degree of confidence in the clinical diagnosis or the treatment plan. Therefore, some orthopedic surgeons prefer MR imaging to ultrasound in the evaluation of rotator cuff tears and other abnormalities of the glenohumeral joint. Moreover, MR imaging, especially when combined with arthrography, represents a one-step investigation, which not only allows for assessment of rotator cuff lesion but also of lesions of the labrum (Bankart lesions, SLAP lesions), the joint capsule and the biceps tendon. It also demonstrates muscle atrophy, which represents an important predictor of surgical outcome in rotator cuff repair.  相似文献

Storage of energy as triglyceride in large adipose-specific lipid droplets is a fundamental need in all mammals. Efficient sequestration of fat in adipocytes also prevents fatty acid overload in skeletal muscle and liver, which can impair insulin signaling. Here we report that the Cide domain-containing protein Cidea, previously thought to be a mitochondrial protein, colocalizes around lipid droplets with perilipin, a regulator of lipolysis. Cidea-GFP greatly enhances lipid droplet size when ectopically expressed in preadipocytes or COS cells. These results explain previous findings showing that depletion of Cidea with RNAi markedly elevates lipolysis in human adipocytes. Like perilipin, Cidea and the related lipid droplet protein Cidec/FSP27 are controlled by peroxisome proliferator-activated receptor gamma (PPARgamma). Treatment of lean or obese mice with the PPARgamma agonist rosiglitazone markedly up-regulates Cidea expression in white adipose tissue (WAT), increasing lipid deposition. Strikingly, in both omental and s.c. WAT from BMI-matched obese humans, expression of Cidea, Cidec/FSP27, and perilipin correlates positively with insulin sensitivity (HOMA-IR index). Thus, Cidea and other lipid droplet proteins define a novel, highly regulated pathway of triglyceride deposition in human WAT. The data support a model whereby failure of this pathway results in ectopic lipid accumulation, insulin resistance, and its associated comorbidities in humans.  相似文献
