目的分析重症监护病房(ICU)中重症肺炎婴儿的死亡危险因素。方法收集医院信息系统中儿科和新生儿科ICU 2012年1月1日至2017年1月1日收治的年龄不超过2个月的重症肺炎患儿的病历信息。采用SAS 9.4统计学软件分析数据,以患儿的一般资料、药物治疗方案、合并用药、合并疾病、临床结局等指标进行单因素分析,采用多因素Logistic回归分析法筛选出与患儿死亡相关的独立危险因素。结果共纳入604例重症肺炎患儿,其中死亡49例(8.11%)。多因素Logistic回归分析结果显示,出生体质量低于1.8 kg[OR=3.92,95%CI(1.50,10.23),P=0.002],极危重肺炎[OR=3.55,95%CI(1.39,9.09),P=0.008],进行机械通气[OR=5.06,95%CI(1.97,12.95),P<0.001],合并贫血[OR=5.61,95%CI(2.36,13.35),P<0.001],合并新生儿窒息[OR=6.03,95%CI(1.57,23.12),P=0.009],合并消化道出血[OR=3.73,95%CI(1.21,11.48),P=0.021],合用镇静催眠药[OR=4.32,95%CI(1.76,10.61),P<0.001]均为重症肺炎婴幼儿死亡的独立危险因素;合用微生态制剂为保护性因素,可显著降低患儿的死亡率[OR=0.24,95%CI(0.10,0.54),P<0.001]。结论重症肺炎疾病的严重程度、患儿的营养状态、合并疾病、临床治疗方案、合用药物等均可对重症肺炎婴儿的临床结局产生较大影响,医师和药师在临床治疗过程中应早期识别死亡危险因素,并建立相应的预防措施,以降低死亡率,提高合理用药水平和医疗质量。  相似文献

目的:研究早期肠内营养(EEN)耐受性分级干预对重症患者的应用效果。方法:将我院2018年1月1日—2021年1月31日期间收治的87例重症患者纳入研究。以电脑随机数字表法将其分成分级组43例和对照组44例。对照组开展常规肠内营养干预;分级组则开展EEN耐受性分级干预方案进行干预。分析2组热卡达标情况,干预前后营养指标变化情况,喂养不耐受症状情况,干预前后免疫状况以及胃肠激素水平变化情况等方面的差异。结果:分级组干预7 d后、14 d后的热卡达标例数占比均高于对照组(均P<0.05)。干预后分级血清白蛋白以及总蛋白水平均高于对照组(均P<0.05)。分级组喂养不耐受症状总发生率低于对照组(P<0.05)。干预后分级组IgA以及IgG水平均高于对照组(均P<0.05)。分级组干预后血清胃动素(MTL)水平高于对照组,而血管活性肽(VIP)水平低于对照组(均P<0.05)。结论:EEN耐受性分级干预对重症患者的应用效果显著,可明显改善患者的营养状况,减少喂养不耐受症状的发生,促进免疫功能的增强,值得临床推广应用。  相似文献

A recent study reported that patients with interstitial lung disease (ILD) are at increased risk of death from coronavirus disease 2019 (COVID-19). However, there are no studies on the outcome of COVID-19 patients with preexisting ILD treated with corticosteroids or antiviral drugs. We extracted 26 patients with preexisting ILD by medical records and HRCT pattern. Of 503 patients with COVID-19, we selected 52 patients as control matched for age and sex. Twenty out of the 26 ILD patients (76.9%) received corticosteroid therapy, and 23 patients (88.5%) also received antiviral treatment with remdesivir or favipiravir. Although no statistical difference was found, the proportion of severe patients in ILD group tended to be higher than in non-ILD group (23.1% vs. 42.3%; p = 0.114). Also, mortality rate in ILD group tended to be higher than in non-ILD patients (11.5% vs. 3.8%; p = 0.326). In univariate analysis to evaluate risk factors for severe condition, diagnosis of idiopathic pulmonary fibrosis, usual interstitial pneumonia pattern, and honeycomb lung were not risk factors of severe disease. Treatment with corticosteroids, antiviral drugs, and immunosuppressive agents may affect the outcome of COVID-19 patients with ILD.  相似文献

目的:分析在预防重症脑卒中患者发生肺部感染的过程中振动排痰机的临床应用效果。方法:本次实验研究的76例对象随机选取于2019年10月~2020年10月本院收治的重症脑卒中患者,根据掷硬币法实现患者的分组,其中将振动排痰机作为预防肺部感染方法的38例患者作为Ⅰ组,运用传统排痰方式预防肺部感染的38例患者作为Ⅱ组。结果:比较后可知Ⅰ组患者排痰效果与排痰满意度均明显高于Ⅱ组患者,P<0.05;统计学分析后可知,与Ⅱ组患者相比,Ⅰ组患者肺部感染发生率明显较低,P<0.05。结论:在预防重症脑卒中患者出现肺部感染的过程中振动排痰机的运用不仅可以提高排痰的效果与排痰满意度,同时也有效地降低了肺部感染的发生概率,为患者的高效治疗、更好恢复奠定了基础。  相似文献

Background:In recent years, the incidence rate of children with severe Mycoplasma pneumoniae pneumonia (SMPP) is increasing, which poses a great threat to children''s life and safety. There are some limitations in the existing drugs for the treatment of SMPP, and the supplementary and alternative therapy of SMPP plays an irreplaceable role in the treatment of this disease. This study will evaluate the efficacy and safety of various complementary and alternative therapies for SMPP by means of mesh meta-analysis. In order to provide the basis for clinical rational use.Methods:Two researchers will independently and comprehensively searched the Cochrane Central controlled trials registry, Cochrane Library, PubMed, web of science, EMBASE, CNKI, and Wanfang database to collect randomized controlled trials (RCT) studies on complementary and alternative therapies for SMPP. And the relevant references included in the systematic review/meta-analysis are screened. The retrieval time limit is from the establishment of the database to November 2020. We will use Revman 5.3 software for meta-analysis and use grade to grade the quality of evidence in the net meta-analysis (NMA).Results:The aim of this study was to compare the efficacy and safety of different complementary and alternative therapies in the treatment of SMPP, with a view to evaluating and ranking different interventions.Conclusion:The supplement and replacement therapy of SMPP can improve the clinical efficacy, relieve the clinical symptoms, improve the quality of life of children, and reduce adverse reactions, which can provide strong support for the rational use of clinicians.INPLASY registration number:INPLASY2020110079.  相似文献

BackgroundChildren with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are typically asymptomatic but contagious. The authors investigated the positivity rate of asymptomatic SARS-CoV-2 infection in pediatric dental patients.MethodsThe authors reviewed consecutive charts of children younger than 18 years scheduled for elective dental procedures from April 1, 2020, through August 1, 2020. All patients were screened for signs and symptoms of SARS-CoV-2 infection. Asymptomatic patients scheduled for dental procedures underwent polymerase chain reaction (PCR) testing for SARS-CoV-2. Sociodemographic characteristics were abstracted, and positivity rates were calculated. Variables for patients who were SARS-CoV-2 positive and SARS-CoV-2 negative were compared using Fisher exact and Mann-Whitney U tests.ResultsThe sample size was 921. The median age was 6 years, and 50.9% were boys. The overall SARS-CoV-2 positivity rate was 2.3%. Age, insurance status, medical history, and dental diagnosis were comparable in patients who were SARS-CoV-2 positive and SARS-CoV-2 negative. Positivity rates were statistically higher for Hispanic or Latinx patients than other groups (P = .038).ConclusionsAlthough the yield of testing was low, the systematic evaluation of asymptomatic pediatric dental cases via PCR resulted in the identification of SARS-CoV-2 carriers who could have been infectious. In this study, Hispanics or Latinx had a higher positivity rate than other demographic groups.Practical ImplicationsPCR testing for SARS-CoV-2 of asymptomatic patients in pediatric dentistry adds value to the use of screening questionnaires for the identification of infected people who could be contagious.  相似文献
