Since 2013, H5N6 highly pathogenic avian influenza viruses (HPAIVs) have been responsible for outbreaks in poultry and wild birds around Asia. H5N6 HPAIV is also a public concern due to sporadic human infections being reported in China. In the current study, we isolated an H5N6 HPAIV strain (A/Muscovy duck/Long An/AI470/2018; AI470) from an outbreak at a Muscovy duck farm in Long An Province in Southern Vietnam in July 2018 and genetically characterized it. Basic Local Alignment Search Tool (BLAST) analysis revealed that the eight genomic segments of AI470 were most closely related (99.6%–99.9%) to A/common gull/Saratov/1676/2018 (H5N6), which was isolated in October 2018 in Russia. Furthermore, AI470 also shared 99.4%–99.9% homology with A/Guangxi/32797/2018, an H5N6 HPAIV strain that infected humans in China in 2018. Phylogenetic analyses of the entire genome showed that AI470 was directly derived from H5N6 HPAIVs that were in South China from 2015 to 2018 and clustered with four H5N6 HPAIV strains of human origin in South China from 2017 to 2018. This indicated that AI470 was introduced into Vietnam from China. In addition, molecular characteristics related to mammalian adaptation among the recent human H5N6 HPAIV viruses, except PB2 E627K, were shared by AI470. These findings are cause for concern since H5N6 HPAIV strains that possess a risk of human infection have crossed the Chinese border.  相似文献

Background

Highly cross-linked polyethylene (HXLPE) liners in total hip arthroplasty (THA) have demonstrated decreased wear rates, resilience to cup orientation, and reduced osteolysis compared to conventional polyethylene. Sequential irradiation and annealing below the melting temperature is unique compared to most HXLPE which is irradiated and remelted. This study purpose is to provide minimum 5-year femoral head penetration rates of sequentially annealed HXLPE in primary THA.

Methods

A retrospective review of a prospectively collected database identified 198 consecutive, cementless primary THAs utilizing sequentially annealed HXLPE (X3; Stryker, Mahwah, NJ). Operative technique was standardized. Radiographs were analyzed utilizing the Martell method with minimum 5-year and 1-year radiographs as baseline to minimize the initial bedding-in period.

Results

Seventy-seven hips with minimum 5-year follow-up were analyzed. Mean steady state linear and volumetric head penetration rates were 0.095 mm/y and 76 mm3/y, respectively. Volumetric head penetration was significantly less for 32-mm compared to 36-mm (P = .028). In addition, less head penetration was observed for ceramic 32-mm heads at nearly half the rate compared to cobalt-chromium 36-mm heads (P ≥ .092). No correlations existed between penetration rates and age, body mass index, University of California Los Angeles Activity Level, polyethylene thickness, cup inclination, or anteversion (P ≥ .10). No radiographic osteolysis was observed.

Conclusion

Surprisingly, linear head penetration rates of sequentially annealed HXLPE were nearly identical to the osteolysis threshold for conventional polyethylene and greater than reports of irradiated and remelted HXLPE. Furthermore, these data corroborate reports that HXLPE is resilient to cup orientation and demographic variables. Longer term follow-up is recommended.  相似文献

依据《江苏省重点职业病监测与职业健康风险评估工作实施方案》要求,分析泰州市职业健康检查、职业病诊断与鉴定、职业病患者工伤保险待遇落实及职业病报告信息。结果显示,2017年职业健康检查共16759人,检出职业病10例,疑似职业病23例,职业禁忌证22人。职业病患者均已享受工伤保险待遇。泰州市重点职业病危害因素为电焊烟尘、矽尘和噪声,应进一步加强职业健康监护工作。  相似文献

目的:采用网状Meta 分析评价6种止吐用药方案预防高致吐风险化疗所引起恶心呕吐(CINV)的疗效。方法:计算机检索中国知网、万方数据、维普医药、PubMed、Embase和Cochrane Library(2019年第8期),检索时限至2019年9月1日,收集5-羟色胺3受体抑制剂(包括第一代5-HT3和第二代5-HT3帕洛诺司琼(PAL))+地塞米松(DEX)、神经激肽1受体抑制剂(NK-1RA)+5-HT3+DEX、奥氮平(OLZ)+5-HT3+DEX、(奈妥吡坦300mg+帕洛诺司琼0.50mg)NEPA+DEX预防高致吐风险CINV的随机对照试验(RCT),由两名评价员独立筛选文献、提取数据,并按照Cochrane偏倚风险评估工具评价文献质量,采用Stata 15.0统计软件进行网状Meta分析。结果:共纳入18项RCT,合计8076例患者。网状Meta分析结果显示:与5-HT3+DEX比较,NEPA+DEX、NK-1RA+5-HT3+DEX、NK-1RA+PAL+DEX和OLZ+PAL+DEX预防高致吐风险CINV的急性CINV完全缓解率高于5-HT3+DEX组,差异有显著差异,概率排序为OLZ+PAL+DEX>NK-1RA+PAL+DEX>NK-1RA+5-HT3+DEX>NEPA+DEX>PAL+DEX = 5-HT3+DEX。与5-HT3+DEX比较,NEPA+DEX、NK-1RA+5-HT3+DEX、NK-1RA+PAL+DEX、OLZ+PAL+DEX和PAL+DEX预防高致吐风险CINV的迟发性CINV完全缓解率高于5-HT3+DEX组,差异有显著差异,概率排序为NK-1RA+PAL+DEX>OLZ+PAL+DEX>NEPA+DEX>NK-1RA+5-HT3+DEX = PAL+DEX = 5-HT3+DEX。与5-HT3+DEX比较,NEPA+DEX、NK-1RA+5-HT3+DEX和NK-1RA+PAL+DEX预防高致吐风险CINV的持续期CINV缓解率高于5-HT3+DEX组,差异有显著差异,概率排序为NK-1RA+PAL+DEX>OLZ+PAL+DEX>NEPA+DEX>NK-1RA+5-HT3+DEX)>PAL+DEX>5-HT3+DEX。结论:通过网状Meta分析发现,OLZ+PAL+DEX是预防高致吐风险急性CINV最有效方案,而NK-1RA+PAL+DEX是预防迟发性和持续期CINV最有效方案。  相似文献

目的 分析河南省疾病预防控制中心2005 - 2017年科技论文发表情况,为疾控机构今后科研工作发展、学科建设和人才队伍建设提供参考依据。方法 运用中国知网、万方数据库、Pubmed、EMbase、Web of Science数据库或搜索引擎,检索2005 - 2017年“第一作者”当时是该中心员工且“第一作者单位”为“该中心”正式发表的科技论文,应用Microsoft Exce 2010录入和整理数据,应用SPSS 17.0软件进行统计分析。结果 2005 - 2017年该机中心发表科技论文2 052篇,年平均发表158篇。刊载在200种期刊上,其中外文期刊37种,中文期刊163种。传染病防控专业和卫生检验专业发表的论文数量占比较高,分别为45.18%和23.73%。CN一般期刊占比最高,为44.44%;其次是中文核心期刊,为40.59%。硕士、博士学历组人均发表论文较多,人均分别为12.09篇、12.82篇;副高和正高级职称组人均发表论文较多,分别为12.36篇、10.76篇,结论 该中心近年年均发表论文数量和质量整体呈上升趋势,在国内同行中处于相对较高水平,说明科研水平在稳步上升,同时也反映出社会影响、政策激励对科学研究的推动作用。  相似文献

Background

There is continued debate regarding retention versus sacrificing of the posterior cruciate ligament (PCL) in total knee arthroplasty (TKA). We sought to determine if there was a difference in range of motion (ROM) after TKA between patients with PCL sacrifice versus PCL retention when using a highly congruent polyethylene insert.

Methods

We conducted an Institutional Review Board approved retrospective study of consecutive patients receiving TKA using the same implant with a highly congruent polyethylene component implanted by one surgeon from November 2013 to January 2016. Patients were placed in 2 groups based on whether the PCL was intact or released at the time of surgery. Patient charts were reviewed for age, body mass index, PCL status at surgery (incompetent, kept intact, or released), and preoperative/postoperative knee ROM.

Results

Both groups were similar in average age (60.5 vs 60.6, respectively) and body mass index (33.3 vs 32.6, respectively). Postoperative tibial slope (5.5° PCL release, 6.6° PCL retained, P = .028) was the only alignment variable reaching significance; all other alignment and motion variables were similar.

Conclusion

Results indicate that the PCL can be successfully retained with the use of a congruent bearing design, with no evident limitation in postoperative ROM or loss of stability due to the bearing in comparison to patients who undergo PCL release.  相似文献
