Introduction and objectivesThis article presents the data corresponding to automated implantable cardioverter-defibrillator (ICD) implants in Spain reported to the Spanish Registry in 2018.MethodsThe data in this registry include both primary implants and generator replacements and were gathered from a data collection sheet voluntarily completed by implantation centers.ResultsIn 2018, 6421 implant sheets were received compared with 7077 reported by Eucomed (European Confederation of Medical Suppliers Associations). This represents data on 90.7% of the devices implanted in Spain. Compliance ranged between 99.6% for the field “name of the implanting hospital” and 12.4% for “population of residence”. A total of 173 hospitals reported their data to the registry, representing a slight decrease compared with hospitals participating in 2017 (n = 181).ConclusionsAfter the reduction in ICD implants in 2017, the number of implants increased in 2018, with the highest number of ICDs implanted in Spain. The total number of implants remains much lower than the European Union average, with substantial differences between autonomous communities.  相似文献

To be truly useful, an index must be applicable universally without modification. The data presented in this article support the use of the standard DAI without modification to assess need for orthodontic treatment in both developing and industrialized countries. When students from Thailand, Australia, the German Democratic Republic, and Native Americans residing on Indian reservations rated the same 25 occlusal stimuli (photographs of dental configurations) for dental aesthetics, their ratings of these stimuli when arranged in rank order were highly correlated with those of US students. These 25 occlusal stimuli were a subset of the original 200 occlusal stimuli used in deriving the standard DAI regression equation. This article presents data indicating that when a Native American DAI was derived based on Native American ratings of the same 200 occlusal stimuli used in deriving the standard DAI, the Native American DAI and the standard DAI had the same ten components (orthodontic measurements) and strikingly similar regression coefficients (weights). Further analysis indicated that the two equations were sufficiently similar to justify using the standard DAI to obtain DAI scores on Native Americans and to estimate the relative social acceptability of their dental aesthetics. If ratings for dental aesthetics for the full set of 200 stimuli were available for each of the countries where only 25 were rated, we expect that, as in the case of Native Americans, a country-specific DAI, if computed, would be sufficiently similar to the standard DAI that no modification would be needed to allow use of the standard DAI in that country.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

目的探讨大鼠脑弥漫性轴索损伤(diffuseaxonalinjuries,DAI)后不同时期脑皮层组织中热休克蛋白(heatshockproteins,HSP)70的表达,为临床治疗脑损伤寻求有效手段。方法制作大鼠DAI模型,分别在伤后6、24、48、72h,7d以免疫组织化学方法检测大鼠脑皮层成纤维细胞生长因子(bFGF)的表达,分析其含量变化特点。结果DAI后24hHSP表达增加,48h达高峰,7d后仍维持较高水平。结论bFGF在各时间点表达有明显差异,提示其参与脑损伤后神经元保护及修复过程。  相似文献

This report discusses a selection of the most relevant articles on cardiac arrhythmias and pacing published in 2013. The first section discusses arrhythmias, classified as regular paroxysmal supraventricular tachyarrhythmias, atrial fibrillation, and ventricular arrhythmias, together with their treatment by means of an implantable cardioverter defibrillator. 目的探讨猴头菌提取物颗粒联合美沙拉秦肠溶片治疗溃疡性结肠炎的临床疗效。方法选取2016年4月—2018年4月咸阳市第一人民医院收治的120例溃疡性结肠炎患者作为研究对象,将所有患者根据随机数字表法分为对照组和治疗组,每组各60例。对照组患者口服美沙拉秦肠溶片,1片/次,3次/d;治疗组患者在对照组治疗的基础上口服猴头菌提取物颗粒,3g/次,3次/d。两组患者均持续治疗4周。观察两组的临床疗效,比较两组的血清炎性因子水平、疾病活动指数(DAI)评分、临床症状改善时间。结果治疗后,对照组和治疗组的总有效率分别为81.67%、95.00%,两组比较差异有统计学意义(P0.05)。治疗后,两组患者白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平均显著降低,同组治疗前后比较差异有统计学意义(P0.05);并且治疗组血清炎性因子水平明显低于对照组,两组比较差异有统计学意义(P0.05)。治疗后,两组患者DAI评分显著降低,同组治疗前后比较差异有统计学意义(P0.05);并且治疗组DAI评分明显低于对照组,两组比较差异有统计学意义(P0.05)。治疗后,治疗组患者治愈时间、黏液脓血便消失时间均短于对照组,两组比较差异有统计学意义(P0.05)。结论猴头菌提取物颗粒联合美沙拉秦肠溶片治疗溃疡性结肠炎具有较好的治疗效果,可改善患者临床症状,降低血清炎性因子水平,具有一定临床推广应用价值。  相似文献

Introduction

Traumatic brain injury (TBI) affects cardiac electrical function, and several extra-cerebral factors, including intra-abdominal pressure (IAP), might further modulate this brain-heart interaction. The purpose of this study was to investigate the impact of TBI, and of increased IAP during TBI, on cardiac electrical function as measured by vectorcardiographic (VCG) variables.

Methods

Survival, IAP and changes in VCG variables including spatial QRS-T angle and QTc interval were measured in consecutive adult patients with either isolated TBI (iTBI), or with TBI accompanied by polytrauma to the abdomen and/or limbs (pTBI). For all patients, observations were performed just after the admission to the ICU (baseline) and at 24, 48, 72 and 96 h after admission.

Results

74 patients aged 45 ± 18 were studied. 44 were treated for iTBI and 30 for pTBI. In all patients, spatial QRS-T angle and QTc interval increased after TBI (p < 0.001), relatively more so in patients with pTBI. Compared to survivors, non-survivors also ultimately had greater widening of the spatial QRS-T angle (p < 0.001), most notably just before foraminal herniation. Wider spatial QRS-T angle and longer QTc interval were also noted in patients with IAP > 12 mmHg (p < 0.001), and with right compared to left hemispheric injury (p < 0.001). ST segment level at the J point decreased 24 and 48 h after TBI in leads I, II, III, aVR, aVF, V1, V2, V3 and V6, and increased in lead V1, especially in non-survivors.

Conclusions

Spatial QRS-T angle and QTc interval increase after TBI. If foraminal herniation complicates TBI, further widening of the spatial QRS-T angle typically precedes it, followed by notable narrowing thereafter. Increased IAP also intensifies TBI-associated increases in spatial QRS-T angle and QTc interval.  相似文献

Introduction and objectives

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is an inherited cardiomyopathy characterized by progressive fibrofatty replacement of predominantly right ventricular myocardium. This cardiomyopathy is a frequent cause of sudden cardiac death in young people and athletes. The aim of our study was to determine the incidence of pathological or likely pathological desmosomal mutations in patients with high-risk definite ARVC.

Methods

This was an observational, retrospective cohort study, which included 36 patients diagnosed with high-risk ARVC in our hospital between January 1998 and January 2015. Genetic analysis was performed using next-generation sequencing.

Results

Most patients were male (28 patients, 78%) with a mean age at diagnosis of 45 ± 18 years. A pathogenic or probably pathogenic desmosomal mutation was detected in 26 of the 35 index cases (74%): 5 nonsense, 14 frameshift, 1 splice, and 6 missense. Novel mutations were found in 15 patients (71%). The presence or absence of desmosomal mutations causing the disease and the type of mutation were not associated with specific electrocardiographic, clinical, arrhythmic, anatomic, or prognostic characteristics.

Conclusions

The incidence of pathological or likely pathological desmosomal mutations in ARVC is very high, with most mutations causing truncation. The presence of desmosomal mutations was not associated with prognosis.Full English text available from:www.revespcardiol.org/en  相似文献

Effect of adiponectin on murine colitis induced by dextran sulfate sodium   总被引:3,自引:0,他引:3  
BACKGROUND & AIMS: Adiponectin, an adipose tissue-derived hormone, exhibits anti-inflammatory properties and has various biological functions, such as increasing insulin sensitivity, reducing hypertension, and suppressing atherosclerosis, liver fibrosis, and tumor growth. The aim of the present study was to determine the effect of adiponectin on intestinal inflammation. METHODS: We investigated the effect of adiponectin on dextran sulfate sodium (DSS)-induced colitis by using adiponectin-knockout (APN-KO) mice and an adenovirus-mediated adiponectin expression system. We also examined the contribution of adiponectin deficiency to trinitrobenzene sulfonic acid (TNBS)-induced colitis. In vitro, we examined the effect of adiponectin on intestinal epithelial cells. RESULTS: After administration of 0.5% DSS for 15 days, APN-KO mice developed much more severe colitis compared with wild-type mice. The messenger RNA expression levels of chemokines were significantly higher in the colonic tissues of DSS-treated APN-KO mice compared with wild-type mice, accompanied by increased cellular infiltration, including macrophages. Adenovirus-mediated supplementation of adiponectin significantly attenuated the severity of colitis, but there were no differences in the severity of TNBS-induced colitis between the 2 groups. Adiponectin receptors were expressed in intestinal epithelial cells, and adiponectin inhibited lipopolysaccharide-induced interleukin-8 production in intestinal epithelial cells. CONCLUSIONS: Adiponectin is protective against DSS-induced murine colitis, probably due to the inhibition of chemokine production in intestinal epithelial cells and the following inflammatory responses, including infiltration of macrophages and release of proinflammatory cytokines.  相似文献

[目的]探析戴德英教授临床治疗不同类型子宫内膜异位症的经验。[方法]通过门诊跟师学习,整理相关病例资料,从子宫内膜异位症的临证特点、戴师创制红藤方的思路及针对不同类型内异症的治疗经验三个方面展开,分析戴德英教授运用红藤方治疗不同类型子宫内膜异位症的经验,并附三则验案以验证。[结果]子宫内膜异位症因其表现之"杂"与复发之"勤"而成为妇科疑难病症。戴德英教授究其病因病机为癥积下焦、瘀久化热、瘀热互结、胞脉阻滞,临证注重审证求因,以经方为本,辨证加减,创制红藤方,并根据不同临床表现,结合不同治法,临床每获良效。本文所附三则验案,分别为子宫内膜肺异位,辨证为瘀热内结、肺气不宣;子宫腺肌病,辨证为腑气不通、湿热内结;经行腹痛,辨证为气滞血瘀、蕴而化热,戴师分别予红藤方加减配合调周治疗,中西医结合及中医内外疗法综合治疗,均获良效。[结论]戴德英教授辩证地看待病和证的关系,临床巧用红藤方结合不同治法治疗不同类型子宫内膜异位症,其经验值得后辈学习借鉴。  相似文献
