The use of circular frame external fixation in the treatment of ankle/hindfoot Charcot Neuroarthropathy

Charcot Neuroarthropathy (CN) of the ankle/hindfoot is a devastating condition that results in a loss of alignment, bony malleolar protrusions and frequently leads to ulceration, infection, and amputation. Major limb amputations in this patient population has a 5-year mortality rate approximating 39%–68%. The treatment goal for CN of the ankle/hindfoot is to provide stability with a plantigrade foot that is infection free, shoeable and allows independent weight bearing. The use of a circular frame external fixator is often required when treating patients with CN of the ankle/hindfoot because they often present late with deformity, soft tissue compromise and infection which are contraindications to primary internal fixation. These patients require urgent surgical attention to salvage the limb or risk amputation. In this narrative review article we will discuss the indications, management options, surgical technique, evidence and describe our experience in the use of circular frame external fixation in patients with ankle/hindfoot Charcot Neuroarthropathy.     

新疆低温奶消费情况调研报告

本文旨在探讨影响新疆低温乳消费市场的限制性因素及相应的对策。调研通过网络调查问卷和实地调研走访相结合的方式,从乳制品消费结构、消费能力,低温奶认知、低温奶购买意愿、购买便利性、购买偏好及健康饮奶习惯等方面分析影响新疆低温乳消费市场的因素。结果显示:32.93%的消费者不了解乳中生物活性物质(如乳铁蛋白)的热敏性,39.97%的消费者不能区分各类液态乳产品,47.64%的消费者有煮沸巴氏杀菌乳后饮用的习惯;43.21%的消费者"担心未消毒彻底,存在细菌",36.30%的消费者认为"加热后可增加营养价值和香味";72.64%的消费者曾感到"购买巴氏杀菌乳不方便";54.85%的消费者认为本地品牌"更新鲜、更健康"。结果启示:本地乳企应该充分利用在当地的良好品牌信誉度和牧场资源,尽早布局低温奶市场,通过研发高品质低温乳制品、普及科学饮奶理念、改善冷链物流能力、完善购物体验和精细化服务能力等手段,提高生产竞争力,抵御进口乳制品和常温乳对当地乳品市场的冲击,占领区域低温奶消费市场。     

Fracture mechanics of circular discs with a V-notch subjected to wedging

ObjectiveA method proposed for determining the fracture toughness (FT) of dental materials involves a ‘roller’ wedging open a V-notch in a cylindrical specimen. There are a number of problems with the design of this test and its mechanical analysis, and thus with the validity of the results obtained, were it to be used. Firstly, friction is ignored in calculating the horizontal wedging force. Secondly, the test specimen does not make use of a pre-crack at the notch tip. The aim of this study was to evaluate the effects of these factors on the FT calculated.MethodsAn analytical solution for the mode-I stress intensity factor (K_I) of the compact tension specimen, which bears some similarities, is taken to be applicable. The mechanics of the specimen has been reanalysed, with a finite-element study of the resultant stresses, and compared with the compact-tension test.ResultsThe assumed analytical solution can provide accurate estimates for K_I for the V notched specimen. However, the apparent agreement is due to the fortuitous combination of an overestimated horizontal wedging force and an underestimated stress singularity at the crack tip. In any case, ignoring friction will lead to an overestimate of FT.SignificanceIt is concluded that the test as presented is invalid.     

食管癌切除食管胃T形吻合与圆形吻合安全性的meta分析

目的比较食管癌切除食管胃吻合术中T形吻合与圆形吻合的安全性。 方法计算机检索PubMed、EMbase、The Cochrane Library、中国知网、万方数据库及维普数据库中收录的有关食管癌切除食管胃T形吻合与圆形吻合的比较研究，检索时间为数据库建库至2020年2月1日，采用Stata12.0软件进行meta分析。 结果11篇文献共纳入987例食管癌手术患者，其中T形吻合组551例，圆形吻合组436例。Meta分析结果显示：相比较于圆形吻合组，T形吻合组的吻合口瘘[OR(95%CI)＝0.48(0.27～0.87)，P＝0.015]，吻合口狭窄[OR(95%CI)＝0.14(0.08～0.23)，P<0.001]及胃食管反流[OR(95%CI)＝0.54(0.35～0.84)，P＝0.006]的发生风险较低。 结论食管癌手术使用T形吻合方式可以降低术后吻合口瘘、吻合口狭窄及胃食管反流的发生率。     

中药资源产业副产品循环利用模式研究

"健康中国"建设战略的提出促使我国中药资源产业必须走一条高质量发展之路。但目前我国中药资源产业还未摆脱粗放、污染和孤岛式的发展模式。通过分析中药资源产业生产过程中产生的副产品及废弃物现象及原因、传统处理方式的不足,基于循环经济和超循环经济理论,研究如何高质量绿色发展,创新中药副产品循环利用的模式,构建中药资源高效利用的超循环经济模式体系,最后提出了促进我国中药资源产业高质量发展的策略建议。     

Plasma circular RNA panel acts as a novel diagnostic biomarker for colorectal cancer

BackgroundColorectal cancer (CRC) is one of the most common cancers worldwide, and emerging lines of evidence have implicated circular RNAs (circRNAs), a novel class of endogenous noncoding RNAs, in CRC development. However, whether plasma circRNAs might be novel diagnostic biomarkers for CRC remains unclear.MethodsWe investigated the plasma levels of selected circRNAs by quantitative real-time PCR (qRT-PCR). The presence of the candidate circRNAs was confirmed through RNase R assays, qRT-PCR and DNA sequencing, and their diagnostic value was evaluated using a receiver operating characteristic (ROC) curve.ResultsThe plasma levels of three circRNAs (circ-CCDC66, circ-ABCC1 and circ-STIL) were significantly decreased in CRC patients (n = 45) compared with healthy controls (n = 61). The ROC curve analysis showed that the area under the ROC curve (AUC) of the three-circRNA panel was 0.780, which is higher than that of traditional protein biomarkers, such as carcinoembryonic antigen (CEA) and carbohydrate antigen 19–9 (CA19-9). Combining the circRNA panel with CEA and CA19-9 might improve the ability to diagnose CRC (AUC = 0.855). In addition, the plasma circ-ABCC1 level was related to tumor growth and progression, and the plasma circ-CCDC66 and circ-ABCC1 levels were decreased in precursor lesions of CRC, including colon adenomas and adenomatous polyps. More importantly, circ-CCDC66 and circ-STIL were found to be useful for diagnosing early-stage CRC, and the three-circRNA panel improved the ability to diagnose CEA-negative and CA19-9-negative CRC.ConclusionOur study provides the first identification of a panel of three plasma circRNAs that could serve as a novel and independent diagnostic biomarker for CRC.     

circCDYL Acts as a Tumor Suppressor in Triple Negative Breast Cancer by Sponging miR-190a-3p and Upregulating TP53INP1

BackgroundBreast cancer is the most commonly diagnosed cancer and the leading cause of cancer death among females. Circular RNAs (circRNAs) have been implicated in the initiation and development of cancer. Here, we explored the biological role and regulatory mechanism of circCDYL in breast cancer.Materials and MethodsThe expression and correlation of circCDYL/miR-190a-3p/TP53INP1 axis in breast cancer tissues and cells were determined by quantitative polymerase chain reaction and Western blot. Cell-counting Kit-8, colony formation, cell migration, and invasion assays were applied to investigate the biological roles of circCDYL in breast cancer development and progression.ResultsCircCDYL were down-regulated in breast cancer tissues and cells, the expression of which positively correlated with patients’ survival rate. CircCDYL worked as a “sponge,” binding to miR-190a-3p directly, which inhibited the expression of miR-190a-3p and relieved the inhibition of tumor suppressor gene TP53INP1.ConclusionCircCDYL promotes apoptosis and inhibits proliferation of the malignant phenotype of breast cancer through regulating miR-190a-3p/TP53INP1 axis, which suggests that circCDYL is a potential therapeutic target for breast cancer.     

Circular saw-related fatalities: A rare case report,review of the literature,and forensic implications

Fatalities attributed to powered circular saws appear to be vanishingly rare events with highly wounding and rapidly incapacitating effects. When they do occur, they are mainly self-inflicted in nature. We report the suicide committed by a 79-year-old man using a self-made circular table saw. Autopsy confirmed that the man received multiple heterogeneously distributed saw-type impacts to the head and neck resulting in complete amputation of the upper skull and partial beheading. Homemade or modified commercial sawing instruments and the resultant injuries pose a number of forensic challenges starting from the death scene investigation, continuing with technical examinations, and concluding with determining the manner of death. As with all deaths due to sharp force injuries, fatalities involving power tools such as chainsaws, circular, and band saws warrant a high degree of suspicion of criminal activity and require diligence during all phases of the death investigation.     

ciRS-7/miR-7轴调控肿瘤生长转移的研究进展

抑癌因子miRNA-7（miR-7）在肿瘤组织发生中表达下降,但机制未明。最近发现环状RNA-7（ciRS-7）具有miR-7海绵样吸附作用,从而抑制miR-7的功能,该发现为探索miR-7在肿瘤中表达降低的原因开辟了新的途径。目前认为ciRS-7/miR-7轴不仅具有重要的生物学作用,同时参与机体多种疾病的发生发展。笔者就ciRS-7/miR-7轴调控肿瘤生长转移的研究进展进行综述。
