ObjectiveTo examine published research which explores the perception and acceptability of microneedle technology for immunisation and to investigate the suitability of this technology for paediatric use.MethodsA series of keywords and their synonyms were combined in various combinations and permutations using Boolean operators to sequentially search four databases (PubMed, Web of Science, Embase and CINAHL). Following removal of duplications and irrelevant results, 12 research articles were included in the final literature review.ResultsThe opinions of patients, parents, children and healthcare professionals (HCP) were collated. A positive perception and a high level of acceptability predominated.ConclusionMicroneedle technology research has been focussed on demonstrating efficacy with minimal focus on determining HCP/public perception and acceptability for paediatric use, exemplified by the paucity of studies presented in this review. Commercial viability will depend on HCP/public acceptability of microneedle technology. An effort must be made to identify the barriers to acceptance and to overcome them by increasing awareness and education in stakeholder groups pertaining to the paediatric population.  相似文献

We report on the use of degradable polymer capsules as carriers for the delivery of oligopeptide antigens to professional antigen presenting cells (APCs). To achieve encapsulation, oligopeptide sequences were covalently linked to a negatively charged carrier polymer via biodegradable linkages and the resulting conjugate was then adsorbed onto amine-functionalized silica particles. These peptide-coated particles were then used as templates for the layer-by-layer (LbL) deposition of thiolated poly(methacrylic acid) (PMASH) and poly(vinylpyrrolidone) (PVPON) multilayers. Removal of the silica core and disruption of the hydrogen bonding between PMASH and PVPON by altering the solution pH yielded disulfide-stabilized PMA capsules that retain the encapsulated cargo in an oxidative environment. In the presence of a natural reducing agent, glutathione, cleavage of the disulfide bonds causes release of the peptide from the capsules. The developed strategy provides control over peptide loading into polymer capsules and yields colloidally stable micron- and submicron-sized carriers with uniform size and peptide loading. The conjugation and encapsulation procedures were proven to be non-degrading to the peptide vaccines. The peptide-loaded capsules were successfully used to deliver their cargo to APCs and activate CD8 T lymphocytes in a non-human primate model of SIV infection ex vivo. The reported approach represents a novel paradigm in the delivery of peptide vaccines and other therapeutic agents.  相似文献

High rates of physical and sexual abuse have been found among Alaska Native women entering a residential treatment program in Fairbanks, Alaska. Little information, however, has been available that describes the nature of such abuse and its relationship to treatment outcome. This article describes the extent of such abuse and reviews factors involved in its onset. The implications of the findings are discussed with an emphasis on how they can be applied to enhance treatment outcome.  相似文献

Venous thromboembolism (VTE) is recognized as a serious complication in both surgical and non-surgical patients. Cancer, particularly adenocarcinoma developing in the digestive organs, is one of the most potent risk factors for VTE, as first described by Trousseau in 1865. Cell membrane microparticles shedding from cancer cells play a critical role in venous clot formation through tissue factor (TF)-mediated activation of the coagulation system. Since recent prospective studies have demonstrated that VTE risk in Japanese surgical patients is comparable to that in Caucasians, perioperative thromboprophylaxis with anticoagulants is now considered essential in patients undergoing abdominal cancer surgery, in addition to mechanical thromboprophylaxis. Recent multi-center studies have also shown the efficacy and safety of enoxaparin, a low molecular weight heparin (LMWH), and fondaparinux, a synthetic Xa inhibitor, in VTE prevention after abdominal cancer surgery. Anticoagulants like LMWH are expected to exert not only thromboprophylactic but also antineoplastic effects.  相似文献

Summary During TUR, high frequency currents and voltages applied for cutting and coagulation may interfere with video imaging systems. To minimize this effect, all industrial video systems are shielded against electromagnetic radiation. In many cases, however, this shielding is insufficient. Unfortunately, it is not possible to shield all components of the high frequency circuit in order to avoid the remaining distortions. Sometimes, a careful arrangement of the video system, the high frequency unit, and the connection lines to the resectoscope and the patient plate may help. A very effective reduction of interference can be achieved by minimizing the high frequency power applied to the patient. Respective high frequency units have been developed primarily to reduce the well-known hazards for the patient. As a positive side effect, these units decrease the interference to video systems considerably.  相似文献

BACKGROUND: Human herpesvirus 8 (HHV-8) infection is common in Africa, but prevalence varies geographically. Studies in Europe and America suggest spread through homosexual contact, but evidence of heterosexual spread is inconsistent. We examined the association between HHV-8 and markers of risky sexual activity in Nigeria. METHODS: The study subjects included an adult referent population at relatively low risk of HIV infection, patients attending a sexually transmitted disease (STD) clinic, and female commercial sex workers (CSW). Sera were collected between 1991 and 1994 to study the epidemiology of retroviruses and STD in Lagos, Nigeria. Residual samples were tested for HHV-8 antibodies using a K8.1 enzyme immunoassay and for antibodies to syphilis, chancroid, herpes simplex virus 2, HIV-1/2, and HTLV-1. Associations were sought using chi square tests and logistic regression. RESULTS: Overall, HHV-8 prevalence was 26.5% in 2002 study subjects, being higher among CSW and STD patients (31% in each) than in the referent population (19%). HHV-8 prevalence in women was approximately half that in men in both the referent and the STD populations. Increasing age and STD were each associated with HHV-8-seropositivity in men and women, and among women being a CSW was also a risk factor. HHV-8 antibodies were more frequently detected in those with laboratory evidence of STD in each group. Having at least one STD was associated with having HHV-8 antibodies. CONCLUSION: The higher prevalence of HHV-8 antibody in groups with multiple sexual partners and the association with STD among individuals both support the sexual transmission of HHV-8 in African adults.  相似文献

There are two types of bacterial communication systems, those in which the signal produced by bacteria is directed only at other organisms, and those where the signal is detected by others and self. The latter is involved in adaptation to the environment. The adaptation signals are autoinducers, the response is population density-dependent and has been termed "quorum sensing". Our current knowledge of bacterial signaling systems indicates that Gram positive bacteria use small peptides for both types of signaling, whereas Gram negative organisms use homoserine lactones as autoinducers. Gram- negative bacteria internalize the signals which act upon an intracellular receptor. Gram-positive bacteria use the signals as ligands for an extracellular receptor of a two-component signaling system. Inhibitors of quorum sensing compounds for both Gram positive and Gram negative bacteria are being explored. Signal inhibitors could be potentially effective in impeding biofilm formation, which might prolong the utility of the currently available antibiotics in this era of antibiotic resistance. In this review, we will explore both bacteria-host and bacteria-bacteria communication systems, with an emphasis on inhibitors of these systems both natural and synthetic.  相似文献
