目的探讨高荧光细胞(HFC)和血清腹水白蛋白梯度(SAAG)诊断恶性腹水的价值。方法选取202例出现腹水的患者,依据脱落细胞学检查结果分为恶性腹水组(36例)和良性腹水组(166例),分析HFC和SAAG在良、恶性腹水组中的分布差异。采用受试者工作特征(ROC)曲线评价各项指标单项及联合检测诊断恶性腹水的效能。结果恶性腹水组高荧光细胞绝对值(HF#)和高荧光细胞百分比(HF%)均显著高于良性腹水组(P=0.0000、P=0.0001),SAAG水平显著低于良性腹水组(P=0.0000)。HF#、HF%和SAAG诊断恶性腹水的曲线下面积分别为0.797、0.704、0.770。单项指标分析时,HF#和SAAG的敏感性、特异性、准确性分别为83.3%、66.9%、69.8%和88.9%、62.0%、66.8%,HF#和SAAG串联检测的敏感性、特异性、准确性分别为77.8%、80.1%、79.7%,HF#和SAAG并联检测的敏感性、特异性、准确性分别为94.4%、48.8%、56.9%。结论HF和SAAG联合检测诊断恶性腹水的敏感性有所下降,但特异性和准确性均明显升高,更有利于恶性腹水的诊断。  相似文献

PurposeTo assess clinical outcomes and patency after transjugular intrahepatic portosystemic shunt (TIPS) reduction for overshunting adverse events.Materials and MethodsThis multicenter, retrospective observational study included 33 patients (male-to-female ratio, 20:13; mean age, 59 years; mean Model for End-Stage Liver Disease [MELD] score, 15) who underwent TIPS reduction between 2007 and 2020. Procedure indications included medically refractory hepatic encephalopathy (HE) (85%), post-TIPS hepatic insufficiency (HI) (12%), and heart failure (3%). The measured outcomes included improvement in HE (classified using the West Haven system) and HI, patency of reduced TIPS, and transplant-free survival (TFS).ResultsTIPS reductions were successfully performed using parallel stent (94%) or other (6%) techniques at a median of 120 days after TIPS creation (HE, median, 164 days; HI, median, 5 days). The portosystemic pressure gradient increased from a mean of 10 to 17 mm Hg (P < .001). The overall HE rate after TIPS reduction was 54%; HE was persistent, improved, and resolved in 21%, 32%, and 46% cases, respectively. In patients with HI, the MELD score increased from a mean of 22 before TIPS to 34 after TIPS (P = .061), but without improvement (0%) in HI after TIPS reduction (mean MELD score, 30; P = .266). Recurrent ascites occurred in 14% of the patients. The median shunt patency was 961 days (95% confidence interval, 476–1,447). The 30-day, 6-month, 1-year, and 3-year shunt patency rates were 92%, 81%, 74%, and 37%, respectively. The median TFS was not reached. The 30-day, 6-month, 1-year, and 3-year survival rates were 97%, 90%, 81%, and 60%, respectively.ConclusionsAlthough TIPS reduction may be an effective and durable approach to treat post-TIPS medically refractory HE, shunt reduction may not achieve meaningful benefit for HI.  相似文献

Objectives

This study investigated the prognostic value of first-phase ejection fraction (EF1) in patients with aortic stenosis (AS), a condition in which left ventricular dysfunction as measured by conventional indices is an indication for valve replacement.

Background

EF1, the ejection fraction up to the time of maximal ventricular contraction may be more sensitive than existing markers in detecting early systolic dysfunction.

Methods

The predictive value of EF1 compared to that of conventional echocardiographic indices for outcomes was assessed in 218 asymptomatic patients with at least moderate AS, including 73 with moderate, 50 with severe, and 96 with “discordant” (aortic area <1.0 cm2 and gradient <40 mm Hg) AS, all with preserved EF, followed for at least 2 years. EF1 was measured retrospectively from archived echocardiographic images by wall tracking of the endocardium. The primary outcome was a combination of aortic valve intervention, hospitalization for heart failure, and death from any cause.

Results

EF1 was the most powerful predictor of events in the total population and all subgroups. A cutoff value of 25% (or EF1 of <25% compared to ≥25%) gave hazard ratios of 27.7 (95% confidence interval [CI]: 13.1 to 58.7; p < 0.001) unadjusted and 24.4 (95% CI: 11.3 to 52.7; p < 0.001) adjusted for other echocardiographic measurements including global longitudinal strain, for events at 2 years in all patients with asymptomatic AS. Corresponding hazard ratios for all-cause mortality in the total population were 17.5 (95% CI: 5.7 to 53.3) and 17.4 (95% CI: 5.5 to 55.2) unadjusted and adjusted, respectively.

Conclusions

EF1 may be potentially valuable in the clinical management of patients with AS and other conditions in which there is progression from early to late systolic dysfunction.  相似文献

Atherosclerotic renal artery stenosis is the leading cause of secondary hypertension and may lead to resistant (refractory) hypertension, progressive decline in renal function, and cardiac destabilization syndromes (pulmonary edema, recurrent heart failure, or acute coronary syndromes) despite guideline-directed medical therapy. Although randomized controlled trials comparing medical therapy with medical therapy and renal artery stenting have failed to show a benefit for renal artery stenting, according to comparative effectiveness reviews by the Agency for Healthcare Research and Quality, the trials may not have enrolled patients with the most severe atherosclerotic renal artery stenosis, who would be more likely to benefit from renal stenting. Because of limitations of conventional angiography, it is critical that the hemodynamic severity of moderately severe (50% to 70%) atherosclerotic renal artery stenosis lesions be confirmed on hemodynamic measurement. The authors review techniques to optimize patient selection, to minimize procedural complications, and to facilitate durable patency of renal stenting. The authors also review the current American College of Cardiology and American Heart Association guidelines and the Society for Cardiovascular Angiography and Interventions appropriate use criteria as they relate to renal stenting.  相似文献

目的通过与温度梯度灌注技术进行比较,探讨低温间断灌注技术在经皮椎体后凸成形术治疗椎体周壁破裂型骨质疏松性骨折中的临床价值。方法回顾性分析我院2015年1月至2018年12月期间共60例周壁破损型骨质疏松性椎体骨折患者,均为单椎体骨折,根据骨水泥灌注方法的不同分为温度梯度灌注组30例和低温间断灌注组30例。温度梯度灌注组男6例,女24例;平均年龄为(71.9±6.9)岁;低温间断灌注组男8例,女22例;平均年龄(69.7±7.2)岁。比较两组术前、术后2 d及末次随访时疼痛视觉模拟评分(visual analogue scale,VAS)和Oswestry功能障碍指数(Oswestry disability index,ODI),椎体后凸角,伤椎椎体前缘相对高度,手术时间,骨水泥注射量,骨水泥分布面积比,骨水泥可推注时间(从开始推注到所有骨水泥至团块期晚期不可推注的时间),骨水泥渗漏率。结果温度梯度灌注组术后随访时间平均(15.3±8.4)个月;低温间断灌注组术后随访时间平均(18.6±8.2)个月,两组间比较差异无统计学意义(P>0.05)。两组内术后2 d和末次随访时VAS评分、ODI评分较术前差异均有统计学意义(P<0.05),两组内末次随访时与术后2d相比差异无统计学意义(P>0.05)。术后2 d和末次随访时,两组组间的VAS评分比较差异有统计学意义(P<0.05),其余时间点VAS评分和ODI比较差异无统计学意义(P>0.05)。手术前后及末次随访时,两组组间的伤椎前缘相对高度和伤椎椎体后凸角比较差异均无统计学意义(P>0.05)。两组间手术时间差异无统计学意义(P>0.05),其中低温间断灌注组的骨水泥可推注时间(12.1±1.0)min,显著长于温度梯度灌注组(5.3±1.1)min(P<0.05)。低温间断灌注组的骨水泥灌注量(3.9±0.79)mL,骨水泥分布面积比(0.46±0.10),均优于温度梯度灌注组(3.20±0.67)mL和(0.31±0.07),差异有统计学意义(P<0.05)。低温间断灌注组有2例患者发生骨水泥渗漏,渗漏率6.7%,显著低于温度梯度灌注组的骨水泥渗漏率26.7%(P<0.05)。结论低温间断灌注技术可以有效延长骨水泥可推注时间,有利于提高骨水泥在椎体内的注射量和分布面积,降低骨水泥渗漏率的同时可获得更好的临床疗效。  相似文献
