Objectives:To evaluate, in an in vitro study, the quantity of artefacts generated by two different restorative materials, and to determine the diagnostic accuracy of cone beam CT (CBCT) scans and periapical radiographs in identifying gaps in prosthetic crowns.Methods:A total of 30 teeth restored with metal-ceramic (n = 15) and all-ceramic (n = 15) crowns, properly adapted and with 0.30- and 0.50 mm gaps, underwent CBCT exams (with voxel sizes of 0.25 and 0.30 mm) and periapical radiographs. The artefacts generated by two different crowns were quantified and compared by the Mann–Whitney test. In addition, five examiners evaluated the presence or absence of gaps in periapical radiographs and CBCT images. The accuracy of tests was determined by the area under the receiver operatring characteristic curve and these values were compared by using the Kruskal–Wallis test.Results:There was no significant difference in artefact values between the different restorative materials and the different resolutions of CBCT images. Regarding the accuracy of the tests evaluated, periapical radiography and CBCT with voxel size 0.25 mm showed the best performance for smaller gaps (0.30 mm). For larger gaps (0.50 mm), all exams tested showed the same performance.Conclusions:Periapical radiography was still the most cost-beneficial method for the diagnosis of maladaptation in dental restorations. CBCT exams did not improve accuracy in detecting gaps in prosthetic crowns.  相似文献

Purpose

Purpose of this study was to evaluate the accuracy of fit of cemented polymer infiltrated ceramic network (PICN) material crowns manufactured after digital and conventional impression techniques using micro computed tomography (CT). Furthermore to determine the cement space volume and porosities in the cement layer.

Methods

A molar typodont tooth was prepared for PICN material crowns and replicated thirty times. The dies were randomly divided into three groups of 10 specimens each according to the impression technique: 3M True Definition Scanner (TDS), cara TRIOS (Trios) and Impregum Penta Soft (Impregum). PICN material crowns were milled for each specimen from Vita Enamic blocks and cemented on their respective dies. The absolute marginal discrepancy (AMD), internal fit (IG), total cement space volume (TVC) and marginal porosities (VP) were measured using Micro-CT.

Results

Mean and standard deviations values in μm for the AMD were: TDS 140.1 (28.4); Trios 253.7 (56.8); Impregum 220.2 (101.1). IG values in μm: TDS 173.1 (27.7); Trios 222.2 (22.4); Impregum 211.6 (55.9). TVC in mm3: TDS 19.82 (2.9); Trios 23.67 (2.01); Impregum 23.77 (5.09). VP in mm3: TDS 0.38 (0.09); Trios 0.36 (0.10); Impregum 0.51 (0.31).

Conclusions

TDS group showed significantly better marginal and internal fit than the Trios group. No difference of the parameters was detected between the Impregum and both digital groups which implies that the digital impression technique is suitable in the manufacturing process of PICN material crowns.  相似文献

BackgroundWe performed a proportion meta-analysis of currently available data to determine the prevalence of ceramic fracture for each generation.MethodsA total of 10,571 total hip arthroplasties from 45 studies were included. Proportion meta-analysis with a random-effects model was performed to estimate the prevalence of ceramic fracture. To determine whether the ceramic fractures have a fatigue nature and a risk or latent period for the development, postoperative time lapse, patient age, and body mass index were related with ceramic fracture by metaregression analysis.ResultsAs of postoperative 2.0 to 18.8 years, the rate of ceramic fracture was 0.5% (95% CI, 0.3%-0.8%) in the forte group and 0.2% (95% CI, 0.1%-0.4%) in the delta group (P = .059). The ceramic fracture rate for each component was 0.2% (95% CI, 0.1%-0.3%) for the forte head, 0.1% (95% CI, 0.0%-0.2%) for the delta head (P = .210), 0.2% (95% CI, 0.1%-0.3%) for the forte liner, and 0.2% (95% CI, 0.1%-0.4%) for the delta liner (P = .305). The rate of ceramic fracture per 1000 patient-years was 0.9 (95% CI, 0.5-0.13) in the forte group and 0.5 (95% CI, 0.2-0.8) in the delta group (P = .072). In metaregression analysis, no significant associations were found between prevalence of ceramic fracture and postoperative time lapse, patient age, or body mass index.ConclusionsThe rate of ceramic fracture was 0.9/1000 patient-year in the forte group and 0.5/1000 patient-year in the delta group. BackgroundCeramic-on-ceramic bearings are becoming increasingly popular in primary total hip arthroplasty (THA). To enhance ceramic-on-ceramic liner exchange in case of revision surgery, metal-backed liner systems have been proposed. Little is known about the clinical performance of these implants. The purpose of this study is to evaluate a metal-backed liner implant system for primary THA.MethodsA total of 422 patients (with 468 consecutive THAs) were followed over a mean period of 10 years. All arthroplasties were performed with a cementless stem, a press-fit cup, and a metal-backed liner system. Surgical and clinical data, complications, and revisions were analyzed. The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) before surgery and at 1 and 10 years after surgery were compared.ResultsThe overall 10-year implant survival rate was 93.8%. The survival rate was 97.0% for heads and liners, 97.5% for stem, and 99.3% for acetabular cup. The most common reason for revision was ceramic breakage (2.4%) of the third-generation (BIOLOX forte) acetabular liner. Mean WOMAC score improved significantly from 50.1 before surgery to 13.2 at 1 year after surgery. There was no difference in WOMAC scores between surgical approach and type of bearing at 1 and 10 years after surgery.ConclusionTHA using cementless stem, press-fit cup, and metal-backed liner system provides satisfactory long-term outcomes, with revision rate comparable to that with other systems available in the market. The metal-backed liner system has low risk of mal-seating. Third-generation ceramic liners should be avoided as they seem to be more prone to breakage.  相似文献

Interface strength, damage and fracture properties between ceramic films and metallic substrates affect the service reliability of related parts. The films’ thickness, grain size and residual stress affect the interface properties and fracture behavior, thus related studies attract great attention. In this paper, the interface damage evolution and fracture behavior between ceramic films and metallic substrates were simulated by developing a three dimensional finite element model of alumina films on Ni substrates with cohesive elements in the interfaces. The interface fracture energy as a key parameter in the simulation was firstly determined based on its thermodynamic definition. The simulation results show the Mises stress distribution and damage evolution of the film/substrate structures during uniaxial tensile loading. Specially, when grain size of the films is in nanoscale, the interface strength increases obviously, agreeing with the previous experimental results. The effects of residual stress on interface properties was further simulated. The interface strength was found to decrease with increasing radial residual force and the axial residual pressure increases the interface strength. When the thickness of the films increases, the interface strength keeps a constant but the speed of interface damage becomes faster, that is, the thicker films show catastrophic fracture. The underlying mechanism of damage speed was analyzed. Understanding these size effects and the effects of residual stress is helpful to guide the design of related parts.  相似文献

ObjectivesTo evaluate silane influence on the interfacial fracture toughness (IFT) of composite cement, with the two sub-classes of CAD-CAM composites, polymer-infiltrated ceramic networks (PICN) and dispersed fillers (DF), after hydrofluoric acid etching (HF) or airborne-particle abrasion (AB). A secondary objective was to correlate results with developed interfacial area ratio (Sdr) and surface wettability.MethodsExperimental PICN and DF blocks were cut into equilateral half-prisms, which were treated with HF or AB, then treated with an experimental silane or not and bonded to their counterparts with an experimental light-cure resin cement. After thermocycling, samples (n = 30 per group) were tested for IFT using the notchless triangular prism test in a water bath at 36 °C. Moreover, profilometry and contact angle measurement were performed on rectangular samples of each group. Finally, bonding interface was analysed by SEM.ResultsPICN-HF treated with silane showed the highest IFT significantly. Three-way ANOVA revealed the influence of silane, material class and surface pre-treatment (HF or AB) on IFT (p < 0.05). When silane was used, IFT was correlated with Sdr, while surface wettability was increased. Silane application significantly increased IFT for PICN but not for DF, while PICN performed better with HF and DF with AB.SignificanceSilane increases IFT of composite cement with PICNs, but not with DF materials. Results suggest that silane increases the micromechanical bond by promoting resin cement spreading and penetration in surface roughness. This roughness is significantly higher for pre-treated PICNs than for DF due to their specific honeycomb microstructure when etched, which explains their better bonding properties.  相似文献
