BackgroundAs the malignant potential of main duct (MD-) type intraductal papillary mucinous neoplasm (IPMN) has been discussed together with Mixed-type in most previous studies, the malignant potential of pure MD-type IPMN remains unclear. This study evaluated the specific characteristics and predictors of high-grade dysplasia (HGD) and invasive intraductal papillary mucinous carcinoma (IPMC) for pure MD-type IPMN.MethodsFrom 1,100 patients with IPMN, this study includes 387 patients that underwent surgery. We evaluated the specific characteristics of pure MD-type IPMN by comparing clinicopathological factors between MD-type (n = 79) and branch duct (BD-) type (n = 146) or Mixed-type IPMN (n = 162), and predictors of HGD/invasive IPMC in pure MD-type IPMN.ResultsThe rate of HGD/invasive IPMC was significantly higher in MD-type than in BD-type (70.9 vs. 48.6%, P = 0.001), although there was no difference between MD-type and Mixed-type IPMNs (P = 0.343). Recurrence-free survival (RFS) and disease-specific survival (DSS) of patients with MD-type were better than those of patients with Mixed-type (P = 0.008 and P = 0.009, respectively). There were no significant differences in RFS, overall survival, and DSS between patients with MD-type and patients with BD-type IPMNs. Multivariate analysis showed two independent predictors of HGD/invasive IPMC in MD-type IPMN; mural nodule height ≥5 mm (P = 0.025, odds ratio [OR]; 16.949) and carcinoembryonic antigen (CEA) level in the pancreatic juice obtained by preoperative endoscopic retrograde pancreatography ≥50 ng/ml (P = 0.039, OR; 9.091).ConclusionsMeasurement of mural nodule height and CEA in the pancreatic juice might be useful in determining surgical indication for pure MD-type IPMN, although further studies for confirmation are essential.  相似文献

Cassava (Manihot esculenta Crantz) is a plant that contains neurotoxins such as linamarin and lotaustraline. Its long-term consumption is associated with neuronal damage and contributes to the development of motor impairment in humans and rats. We investigated the effects of the consumption of cassava juice on renal and hepatic function and motor impairments in male rats. The rats received the vehicle, non-toxic and toxic doses of cassava juice, or linamarin as a pharmacological control, over 35 consecutive days. The effects were evaluated in an open field test, rotarod, and swim test. The toxic cassava dose and linamarin resulted in motor impairments in the rotarod and swim test from day 7 of treatment. The toxic cassava dose and linamarin increased the parameters that indicate renal and hepatic damage, with the exception of total protein and albumin levels. Behavioral variables that show motor incoordination (i.e., latency to fall in the rotarod) were negatively correlated with biochemical parameters of renal and kidney damage, whereas spin behavior was positively correlated. Our data indicate that chronic oral consumption of cassava juice caused renal and hepatic damage that was correlated with motor coordination impairment in rats, similarly to their principal neurotoxic compound, linamarin.  相似文献

目的:考察黑豆汁炖制时间对制何首乌外观性状及各类有效成分含量变化规律的影响。方法:采用HPLC同时测定不同炮制时间制何首乌样品中二苯乙烯苷、大黄素、大黄素甲醚、大黄素-8-O-β-D-葡萄糖苷、大黄素甲醚-8-O-葡萄糖苷的含量,选择Agilent ZORBAX Extend-C_(18)色谱柱(4. 6 mm×250 mm,5μm),甲醇(A)-水(B)为流动相梯度洗脱(0~30 min,5%~100%A; 30~40 min,100%A),流速1. 0 mL·min~(-1),柱温35℃,检测波长280 nm。结果:随炖制时间的增加,二苯乙烯苷含量逐渐降低,与8 h时相比,至64 h时含量降低76%;大黄素-8-O-β-D-葡萄糖苷和大黄素甲醚-8-O-葡萄糖苷2种蒽醌苷含量先上升后下降,至24 h时达最高值,炖制40 h时降至与8 h近似的水平,之后略有波动;大黄素、大黄素甲醚2种蒽醌苷元含量先上升后下降,炖制32 h时达最大值,之后缓慢降低并趋于稳定。结论:炖制时间对制何首乌中各类成分含量的影响显著,且变化趋势不尽相同,应规范制何首乌饮片的炮制时间;同时,仅以二苯乙烯苷及蒽醌类成分作为制何首乌的指标性成分依据不够充分,应考虑增加多糖类等质量控制指标。  相似文献

The aim of the present study was to determine the protective effects of pomegranate juice in tris‐based extender on semen parameters, computer‐assisted sperm analysis (CASA) motion characteristics and field fertility of post‐thawed Nili Ravi buffalo (Bubalus bubalis) bull spermatozoa. Two consecutive ejaculates/collection from each of the five adult Nili Ravi buffalo bulls were collected with artificial vagina at 42°C for a period of 7 weeks, diluted in extender containing different concentrations of pomegranate juice (0.0%, 2.5%, 5%, 7.5% and 10%). Diluted samples were packed and frozen in 0.54 ml French straws. The addition of 10% pomegranate juice in extender significantly improved post‐thaw sperm morphology (%), motilities (CASA total motility, progressive motility (%) as well as VAP, VSL, VCL, STR, DAP, DSL) compared to the control group (p < 0.05). Plasma membrane, acrosome membrane and DNA integrity were significantly higher in extender with 10% pomegranate juice than the control group (p < 0.05). Field fertility rate (60.39% vs. 46.53%) was higher (p < 0.05) in extender with 10% pomegranate juice as compared to the control. It is therefore concluded that the addition of 10% pomegranate juice in tris‐based extender improves post‐thaw semen parameters, CASA motion dynamics and field fertility in Nili Ravi buffaloes.  相似文献

ObjectivesThe survival of bacteria in the sports drink and orange juice remaining in and at the mouth of bottles after direct drinking was examined after immediately drinking and incubation at 37 °C for 24 h.MethodsNine healthy participants were asked to drink approximately 100 mL of a plastic bottled sports drink or orange juice. The samples were cultured anaerobically at 37 °C for 7 days. Genomic DNA was extracted from the resulting individual colonies, and bacterial species were identified using 16 S rRNA gene sequencing.ResultsThe mean amount of bacteria in the remaining sports drink and orange juice, immediately after drinking, were (1.6 ± 2.3) × 103 colony-forming units (CFU)/mL and (2.9 ± 3.3) × 103 CFU/mL, respectively. Additionally, bacteria recovered from the mouths of the sports drink and orange juice bottles were (2.5 ± 5.5) × 104 CFU/mL and (5.8 ± 2.4) × 103 CFU/mL, respectively. Oral bacteria, such as Streptococcus, Actinomyces, Neisseria, and Rothia were found to be transferred in the sports drink and orange juice, and the bacteria were scarcely detected after incubation at 37 °C for 24 h.ConclusionsThe bacterial levels differed significantly from the previously reported levels in bottled tea 24 h after drinking, suggesting that remaining drinks with low pH levels can be preserved for a longer period.  相似文献

目的 探讨三种分子诊断技术对儿童肺结核的诊断效能,为儿童肺结核的诊断寻求简单、快速、准确的新方法。方法 搜集哈尔滨市胸科医院儿童结核科、哈尔滨市儿童医院呼吸科、哈尔滨医科大学附属第一医院儿科2016年9月至2018年6月收治的2个月至14周岁的疑似肺结核患儿共186例,最后临床诊断肺结核者119例,其中低龄儿54例(45.4%),年长儿65例(54.6%);非结核病者67例,其中低龄儿31例(42.3%),年长儿36例(53.7%)。应用实时荧光定量PCR(real-time fluorescen quantitative PCR,FQ-PCR)、实时荧光核酸恒温扩增检测技术(simultaneous amplification and testing,SAT)和耐药基因芯片技术分别检测患儿胃液和痰液(年长儿)中的结核分枝杆菌,评价各方法的诊断效能。结果 以临床诊断为标准,FQ-PCR、SAT、耐药基因芯片检测胃液中MTB的敏感度分别为81.51%(97/119)、 78.15%(93/119)、73.11%(87/119),特异度分别为86.57%(58/67)、98.51%(66/67)、92.54%(62/67),Kappa值分别为 0.653、0.709、0.603。3种方法检测年长儿胃液中MTB的敏感度分别为 80.00%(52/65)、73.85%(48/65)、67.69%(44/65),特异度分别为75.00%(27/36)、97.22%(35/36)、88.89%(32/36);检测年长儿痰液中MTB的敏感度分别为 47.69%(31/65)、41.54%(27/65)、36.92%(24/65),特异度分别为88.89%(32/36)、 97.22%(35/36)、91.67%(33/36),各方法检测的敏感度胃液均高于痰液,差异均有统计学意义(χ 2值分别为14.696、13.898、12.334,P值均为0.000),各方法检测的特异度胃液和痰液之间差异均无统计学意义(χ 2值分别为2.347、0.000、0.158,P值分别为0.126、1.000、0.691);胃液总的阳性检出率为87.69%(57/65),痰液总的阳性检出率为58.46%(38/65),两者差异有统计学意义(χ 2=14.114,P=0.000)。 结论 FQ-PCR、SAT、耐药基因芯片检测儿童疑似肺结核患者的胃液和痰液中结核分枝杆菌的敏感度高、特异度强,对儿童肺结核的诊断具有重要价值。  相似文献

目的: 建立藏药石榴莲花散中鞣花酸和胡椒碱含量测定方法,以完善其质量标准。方法: 采用高效液相色谱法测定制剂中石榴子和诃子的鞣花酸含量,采用ODS-2 HYPERSIL C18色谱柱(4.6 mm×250mm,5 μm),以乙腈-0.2%磷酸(15:85)为流动相,流速1.0 mL·min-1,检测波长254 nm,柱温30℃;采用高效液相色谱法测定制剂中荜茇的胡椒碱含量,采用ODS-2 HYPERSIL C18色谱柱(4.6mm×250 mm,5μm),以乙腈-水(40:60)为流动相,流速1.0 mL·min-1,检测波长343 nm,柱温30℃。结果: 鞣花酸进样量在5.065~25.325 μg范围内呈良好线性关系(r=0.9999),平均回收率为97.5%,RSD=1.5%(n=6);胡椒碱进样量在10.414~52.071 μg范围内呈良好线性关系(r=1.0000),平均回收率为99.6%,RSD=0.6%(n=6)。结论: 该方法可准确测定石榴莲花散中鞣花酸和胡椒碱的含量,有助于提高石榴莲花散的质量标准。  相似文献

Chronic kidney disease (CKD) patients in dialysis (HD) are considered to be submitted to a continuous oxidative stress. This stress can cause damage on DNA and, consequently, contribute to the high levels of DNA damage observed in these patients. Due to the well-known role of polyphenols as antioxidant agents we proposed its use to reduce the levels of genotoxicity present in HD-CKD patients. The objective of this study was to evaluate the antigenotoxic effects of unfermented grape juice (UGJ) on HD-CKD patients. The levels of DNA damage were analyzed using different biomarkers, such as breaks and oxidized DNA bases by the comet assay, chromosome damage by the micronucleus test. In addition, TEAC (Trolox equivalent antioxidant capacity) was also evaluated. Thirty-nine patients were followed for six months, of whom 25 were supplemented by UGJ and 14 were not supplemented. The obtained results showed a significant decrease in the underlying levels of oxidative DNA damage, in the supplemented group. Regarding the clinical parameters, LDL and cholesterol, were significantly reduced in the patients studied after the supplementation period, although cholesterol was also decreased in the non-supplemented patients. In conclusion, in our studied group the supplementation with UGJ reduced the levels of oxidative DNA damage of HD-CKD patients.  相似文献

Background: The daily consumption of dietary fiber is frequently below suggested recommendations. Using a double‐blind, controlled, randomized study, we assessed the efficiency and tolerance of a fiber‐enriched orange juice to supplement fiber intake in women. Materials and Methods: After 1 week of noninterventional observation, 192 healthy adult women ingested 400 mL of orange juice for 21 days, which either was not (placebo group) or was enriched with fiber (fiber group). Orange juice ingestion was registered daily and controlled for each week during the study period. Macronutrient, fiber, and energy intake were determined using a 3‐day food record, validated food chemical composition databases, and the “Pro Diet” software. Gastrointestinal symptoms were self‐evaluated daily by scoring 4 grades of symptom intensity and using a visual analog scale to grade pain severity. Results: No changes were observed for macronutrient and energy ingestion. For the placebo group (n = 97), the total fiber intake record was under the daily recommended value. In contrast, the fiber group (n = 95) displayed higher comparative values of total and soluble fiber consumption (P ≤ .001), achieving the daily recommended values of fiber intake. Both groups reported an increased frequency of slight bloating and rumbles over time (P ≤ .05). The fiber group also experienced a higher frequency of slight flatulence over time (P = .002). Conclusion: Consumption of fiber‐enriched orange juice was efficient to achieve the daily fiber intake recommendation for women, was not accompanied by intense adverse events, and may represent a suitable method to supplement fiber intake in woman.  相似文献
