Purpose To examine combined treatment of intravitreal triamcinolone acetonide (IVT) and photodynamic therapy (PDT) for retinal angiomatous proliferation (RAP).Methods Ten eyes of eight consecutive patients with RAP underwent a single injection (about 20 mg) of IVT followed 1 month later by PDT. Best-corrected visual acuity (BCVA) was measured by standardized protocol refraction.Results Median BCVA was 20/160, 20/64, 20/98, 20/89, 20/89, 20/125 at baseline and 1, 3, 6, 9 and 12 months, respectively. Of ten eyes, six (60%) improved BCVA of at least 3 lines at 1, 3, 6 and 9 months. Four (40%) eyes improved BCVA of at least 3 lines at 12 months. Three eyes (30%) lost >3 lines at 12 months. In all patients intraocular pressure was well controlled. No endophthalmitis, retinal detachment or vitreous haemorrhage developed.Conclusions Improvement of BCVA suggests that combination treatment with IVT and PDT for RAP merits further investigation.The authors do not have any proprietary interests.  相似文献

Steady-state pattern electroretinogram (PERG) in response to sinusoidal gratings (1.7 c/deg spatial frequency; 9 × 9 deg field size) temporally modulated (sinusoidally) at 8 Hz were recorded in 40 insulin-dependent diabetics and 28 age-matched normal subjects. Visual acuity was 20/20 in all 40 patients; 31 (62 eyes) showed no sign of retinopathy and nine (18 eyes) showed a few microaneurysms on fluorescein angiography. Insulin-dependent diabetics showed a significant reduction in the PERG mean amplitude as compared with age-matched control subjects (one-way analysis of variance: p < 0.0001). Significant differences were observed between normals and diabetics without retinopathy (Scheffé test: p < 0.0001), normals and diabetics with early retinopathy (Scheffé test: p < 0.0001), no retinopathy and early retinopathy patients (Scheffé test: p < 0.05). In diabetics without retinopathy multi-factorial analysis of variance revealed a significant effect of age of onset of the disease (p < 0.01) and an interaction effect between age of onset and duration (p < 0.001) on PERG amplitude. These results suggest a possible use of the steady-state PERG to detect early macular dysfunction in insulin-dependent diabetics.  相似文献

Effect of citicoline on visual acuity in amblyopia: preliminary results   总被引:1,自引:1,他引:8  
Background: Citicoline has been used to improve consciousness levels in cerebral trauma and as a complement for levodopa in Parkinson's disease. It has also been shown that severe glaucomatous visual field deficits improve for at least 4 months with the use of citicoline. In this paper, preliminary results are presented of an open and a double-blind study on the effect of citicoline in amblyopia.Methods: The open study was started in 1991. Fifty patients with amblyopia were treated with citicoline (1000 mg i.m. daily for 15 days). They were selected from an age group beyond the plastic period of the visual system. Occlusion or other types of anti-amblyopic treatment were never used at the same time as citicoline. A double-blind study was conducted on 10 more patients, randomly assigned to a treatment or placebo group and followed for 6 months. A statistical test for repeated measures was performed on all the results.Results: A statistically significant improvement in visual acuity was found both for the amblyopic and the sound eye in 46 of the 50 patients (92%). The behaviour was different for normal and amblyopic eyes. The improvement remained stable for at least 4 months. These results have been substantiated by the double-blind study. No side effects were observed.Conclusion: Citicoline improves visual acuity, at least temporarily, in amblyopic patients beyond the plastic period of the visual system. Our results suggest that trials of citicoline as a medical treatment of amblyopia are warranted.  相似文献

Background Preliminary data show that nerve growth factor (NGF) may improve tear production in humans. We evaluated the efficacy of topical NGF treatment in dogs who developed dry eye after the excision of the third eyelid lacrimal gland.Methods English Bulldogs (2- to 6-year-old males and females) that had undergone the surgical removal of the prolapsed lacrimal gland of the third eyelid in both eyes at the age of 3–6 months developed chronic keratoconjunctivitis sicca associated with a decrease of Schirmer tear test I values after at least 1 year. One eye, randomly selected, of each dog was treated twice daily with 100 l of NGF ointment for 1 month, while the fellow eye was used as control and treated with the ointment vehicle only. At baseline and after 1 month of NGF treatment the following examinations were performed: corneal evaluation by slit lamp, fluorescein staining, Schirmer tear test I, tear ferning test, corneal esthesiometry by cotton swab and conjunctival impression cytology.Results Topical application of NGF caused a significant improvement of all the evaluated parameters compared with baseline values. In contrast, in the control eyes there was no significant difference between the values measured before and after treatment. In particular, after NGF treatment superficial punctate keratopathy was resolved, corneal haze was reduced from stage 4 to stage 2 and Schirmer test values increased (17.2±1.7 mm/min vs 4.5±1.3 mm/min; p<0.05), as did the tear mucous component (as demonstrated by ferning test: 2.0±0.0 vs 4.0±0.0; p<0.05); conjunctival impression cytology evaluation demonstrated the presence of numerous mucous filaments and a significant increase in conjunctival goblet cell density (102.7±68.3 vs. 18.2±14.3 cell × field; p<0.05). Topical NGF treatment improved corneal sensitivity in two of three eyes.Conclusions This open study suggests that topical application of NGF may enhance the production and functional characteristics in tear film, with an improvement of ocular surface signs in dogs with surgically induced dry eye. These results suggest the possibility of performing further, larger, controlled studies.  相似文献

Background  To study the incidence of endophthalmitis after 23-gauge pars plana vitrectomy and to compare it with the endophthalmitis rate after 20-gauge pars plana vitrectomy performed in the same ophthalmology department. Methods  The charts of 4,021 consecutive 20-gauge or 23-gauge pars plana vitrectomies performed at a single institution, between August 1 2003 and April 1 2008, were reviewed to search for the occurrence of postoperative endophthalmitis. This is a retrospective, interventional, comparative cohort study. Results  Endophthalmitis developed in one of 3,078 eyes after 20-gauge vitrectomy (0.03%) and in none of 943 eyes after 23-gauge vitrectomy. Conclusions  We did not find an increased risk of endophthalmitis after 23-gauge sutureless vitrectomy. No financial relationship exists. The orientation disparity field from two orthographic views of an inclined planar surface patch (covered by straight lines) is analyzed, and a new tool to extract the patch orientation is provided: the function coupling the average orientation of each pair of corresponding surface contours with their orientation disparity. This function allows identifying the tilt of the surface, and two indeterminacy functions describing the set of surface inclinations (around the vertical and horizontal axes) over convergence angle values compatible with the orientation disparity field. Results of simulations show that the selection of inclination values matching the difference between the areas below the indeterminacy functions are consistent with some surface orientation effects found in psychophysical and computational experiments, like: the unbiased tilt vs. biased slant estimates, the slant underestimation, the surface orientation anisotropy, and the slant/tilt covariation.  相似文献

Background Previous investigations have shown that vascular factors are involved in pseudoexfoliative glaucoma (XFG). The present study aims at comparing ocular perfusion pressure (OPP), diastolic ocular perfusion pressure (dOPP), and retrobulbar haemodynamics in pseudoexfoliative glaucoma and primary open-angle glaucoma (POAG). Methods Forty-seven XFGs, 41 POAGs, and 38 healthy controls were evaluated. OPP and dOPP were calculated. Peak systolic velocity (PSV), end-diastolic velocity (EDV), and resistivity index (RI) were recorded in ophthalmic artery (OA), central retinal artery (CRA), and short posterior ciliary arteries (SPCAs) by color Doppler imaging (CDI). Correlations between OPP and CDI parameters and between dOPP and CDI parameters were determined. Results OPP and dOPP were significantly lower in XFGs than in POAGs and controls (p < 0.001). CDI investigation revealed decreased EDV of OA, SPCAs, and CRA (p < 0.001) and increased RI of all the three considered vessels in XFGs compared with POAGs and controls (p < 0.001). A negative correlation between OPP and RI of OA and between dOPP and RI of OA was found in XFGs (p = 0.022 and p = 0.015 respectively). Conclusions Ocular perfusion pressure is decreased and retrobulbar haemodynamics are worse in pseudoexfoliative glaucoma patients than in primary open-angle glaucoma patients and healthy controls. An impaired ocular vascular regulation is suggested in pseudoexfoliative glaucoma. Background In vitro and in vivo studies demonstrated the antiviral efficacy of nerve growth factor (NGF) and its cyto-protective effect in herpes simplex virus (HSV)-infected cells. The aims of this study were to evaluate the role of endogenous NGF in HSV corneal infection, and the effects of topical NGF treatment on herpetic keratitis. Methods Herpetic keratitis was induced in 40 rabbits with the HSV-1 McKrae strain. Animals were divided into four groups, and treated with topical neutralizing anti-NGF antibodies, NGF, acyclovir or balanced salt solution (BSS) respectively. The clinical course of HSV keratitis was evaluated and scored by slit-lamp examination. In addition, biochemical (immunohistochemistry for glycoprotein D) and molecular (nested PCR for glycoprotein D) analyses were carried out to estimate viral replication. Results Treatment with anti-NGF antibodies induced a more severe keratitis associated with increased biochemical and molecular markers of active viral replication. Two animals in this group developed lethal HSV encephalitis. Conversely, topical treatment with NGF induced a significant amelioration of clinical and laboratory parameters when compared to the BSS treated group (control). No significant differences were observed between NGF- and acyclovir-treated groups. Conclusions This study demonstrated the crucial role of endogenous NGF in herpetic keratitis. The comparable effects of NGF and acyclovir confirm the antiviral activity of NGF, and indicate a potential use of topical NGF in herpetic keratitis.  相似文献
