目的 开展养老机构志愿者预立医疗照护计划培训并评价其效果。方法 应用基于Bloom目标教学理论构建的养老机构志愿者预立医疗照护计划培训内容,结合《预立医疗照护计划》知识教育手册、《我的五个愿望》手册、案例视频等教具,通过角色扮演、案例讨论等方式对53名养老机构志愿者进行培训。培训前后使用养老机构志愿者预立医疗照护计划知信行及自我效能问卷对志愿者进行调查,根据志愿者角色扮演表现评价其预立医疗照护计划沟通技能。结果 培训前后志愿者预立医疗照护计划认知、态度、行为意愿及自我效能差异有统计学意义(P<0.05,P<0.01),培训后75.47%志愿者预立医疗照护计划沟通技能达合格及以上水平。结论 预立医疗照护计划培训能改善养老机构志愿者的预立医疗照护计划知信行及自我效能感,培养其预立医疗照护计划沟通能力。  相似文献

As advanced practice registered nurses (APRNs) open private practices in medically underserved areas, clinical, practice management, and business skills are needed conjunctively to remain operational. Further information is needed to understand APRN business and entrepreneurial skill needs. A convenience sample assessed perceptions of confidence in business and operational skill sets needed for entrepreneurial roles in 102 APRNs, representing diversity of subspecialties and practice locations in 4 southeastern states with provider shortages: Alabama, Florida, Mississippi, and Tennessee. Sustaining practices in underserved areas requires providing innovative educational offerings to address APRN business skills needs.  相似文献

Sharing new medical information that is perceived as seriously effecting people’s lives, i.e., breaking bad news (BBN) is important in caring for patients and relatives and is challenging for healthcare professionals. Optimal BBN requires incorporation and implementation of multiple professional competencies acquired gradually throughout years of training. The BBN encounter has implications for all participants: the patient, family members, their close social environments, and the deliverer of the news. Due to these implications and the accountability involved, medical schools invest educational resources in helping medical students develop this competency.The current paper summarizes literature, research, and teaching experiences while suggesting practical guidelines for designing and teaching a BBN course to undergraduate students.The following principles lie behind the recommendations: stepwise spiral continuity of exposure to and teaching of communication skills in various contexts while focusing on BBN in the advanced clinical years; relating the developing skills to broader humanistic studies; enhancing awareness of self-perspectives and beliefs regarding BBN; connecting to patients’ and family members experiences and needs; providing a BBN protocol and opportunities for structured experiential learning followed by reflection and feedback; using observation and reflection to address gaps between theory and real-life practice; and creating continuity of learning about BBN through undergraduate, graduate, and continuing medical education. Applying this learning process can help enhance the management of these difficult conversations to improve patients’ care during these difficult, life-changing encounters, and physicians’ well-being.  相似文献

BackgroundPrevious validity studies of the Alberta Infant Motor Scale (AIMS), using raw scores and percentile curves for group comparisons, showed that infants in Brazil achieved gross motor milestones at later ages. Validity of the AIMS norms were later reassessed using a logistic regression model that placed the AIMS items on an age scale.ObjectivesOur study examined the validity of the AIMS norms for Brazilian infants using the recommended method for calculating and comparing item locations.MethodsData from 732 Brazilian infants (2009–11), 3 days to 18 months old, were compared to the AIMS normative sample (n = 2202). Logistic regression placed the AIMS items of both samples on age scales representing the age at which 50% of infants passed an item and compared the two datasets. Pearson correlation coefficient tested the association across samples.Results47 of the 58 AIMS items met the criterion for stable regression to calculate item locations of the Brazilian dataset. Based on the age when 50% of the infants passed a criterion, most of the items from the Brazilian sample (n = 28) differed by two weeks or less compared to the Canadian normative sample.ConclusionThe sequence and age for the emergence of AIMS items were similar between the Brazilian and Canadian samples. Canadian norms are appropriate for clinical decisions and research with Brazilian infants.  相似文献

Community health and social care practitioners play an increasingly important role in the health promotion agenda, but lack confidence in having effective health behaviour change (HBC) conversations with members of the public. This study reports the development and evaluation of a training intervention based on health psychology to improve health and social care practitioner self-rated confidence, competence and intention to use five behaviour change techniques (BCTs) in their HBC conversations. A 2-day behavioural science interprofessional skills training course plus online learning module was designed for health and social care staff across North East Scotland, teaching five evidence-based BCTs (e.g. Action Planning), plus person-centred communication skills. Participants rated confidence, competence and future intention to use the BCTs on likert scales (1–10) pre-course and post-course, and provided acceptability data. 177 participants aged 20–64 took part, qualitative and quantitative data suggested that the course had high acceptability. Paired samples t tests (n = 120 with complete data) showed significant improvements in confidence, competence and intention following the course, which remained significant with a conservative analysis (n = 174) assuming no change for missing data. Perceived competence in Action Planning increased most during the course (mean change 3.09). In conclusion, health psychology-based skills training can improve practitioner confidence, competence and intention to use evidence-based BCTs; further evaluation is needed to assess practice change.  相似文献

BackgroundDifficulties in fine and gross motor skills are often overlooked as developmental problems, although approximately 6–13% of all school-age children have poor motor coordination. Understanding motor coordination is important from the perspective of school adaptation. This longitudinal cohort study aimed to determine whether fine and gross motor skills in preschool children predict later academic achievement and psychosocial maladaptation.Methods & ProceduresParticipants were 2,501 children from nursery and elementary schools (5–13 years old). The motor skills of preschool children were assessed by their nursery teacher immediately before entering elementary school. The Strengths and Difficulties Questionnaire and a standardized Japanese test were administered annually throughout elementary school.ResultsEarly motor difficulties in preschool children had significant effects on their academic achievement and psychosocial maladaptation up until the sixth grade. Gross motor difficulties in preschool were associated with the later peer problems and worsened emotional symptoms.ConclusionsMotor skills in preschool children are useful as a predictor of later psychosocial maladaptation and academic achievement.  相似文献
