BackgroundThe PLUS-M 12-item Short-Form is a self-questionnaire that assesses the perceived capacity of lower limb amputees (LLAs) to perform a number of daily-life activities. Its psychometric properties are excellent (intraclass correlation coefficient [ICC] > 0.9, fast administration and scoring, normative data available), and it can be used in clinical practice or for research purposes.ObjectiveWe aimed to develop a French version of this questionnaire and to assess its psychometric properties.MethodsWe followed international recommendations for translation and cross-cultural validation of questionnaires. In total, 52 LLAs (age 53 ± 16, 40 males, 28/12/12 transtibial/Gritti-Stokes/transfemoral, 20/28/4 ischemic/traumatic/other) participated. Criterion and construct validities were assessed with the Pearson correlation coefficient (PCC) between the PLUS-M 12-item Short-Form and other constructs (Prosthetic-Profile-of-the-Amputee-Locomotor Capabilities Index, Activities-specific Balance Confidence scale, 2-min walking test and Timed Up and Go test), internal consistency with the Cronbach α and reliability with the ICC in 46 individuals who completed the questionnaire twice in a 7-day interval.ResultsThe mean (SD) PLUS-M 12-item Short-Form T-score was 56.1 (7.8; range 40.3 to 71.4). Construct and criterion validity, internal consistency and reliability ranged from low to excellent (r = 0.43 to 0.84, P < 10−2 to 0.002; Cronbach α = 0.90, ICC = 0.89 [0.81–0.94]). We found no floor or ceiling effect.ConclusionsThe French version of the PLUS-M 12-item Short-Form has good to excellent psychometric properties, comparable to those of the original version. Its use could definitely be proposed for both clinical and research purposes, once its validation is completed by assessing other psychometric qualities, especially sensitivity to change.  相似文献

ObjectiveThe purpose of this exploratory study was to determine whether a single session of vibration therapy (VT) would improve muscular and functional performance in individuals with symptomatic peripheral artery disease (PAD).MethodsIn a randomized, balanced cross-over design fourteen PAD participants with intermittent claudication (mean ± standard deviation; age, 73.9 ± 4.6 years; height, 172.6 ± 68.4 cm; body mass, 85.2 ± 15.7 kg) performed VT and control that involved repeated chair rises, timed up-and-go test, and 6-minute walk test. Each intervention was separated by at least 2 days. Wearable VT devices were positioned on the right and left lower limbs that were turned on during functional testing but were turned off for the control intervention.ResultsVT significantly improved (P < .05) repeated chair rises and timed up-and-go test compared with control with a small effect size of 0.46 and 0.45, respectively. Similarly, a significant (P < .01) and meaningful change in 6-minute walk test was noted in VT compared with control.ConclusionsThis exploratory study suggest that VT may enhance functional strength, mobility, and walking performance by extending the onset of claudication and increasing walking distance in PAD with intermittent claudication. However, further study is required to confirm and extend these preliminary findings and determine the potential mechanisms of action in VT.  相似文献

ObjectiveTo compare the gait event identification of five algorithms recommended in the literature with those provided by force plate (gold standard) in children with unilateral or bilateral spastic cerebral palsy (SCP).MethodsThis was a cross-sectional study of the gait of three girls and four boys with a mean age of 8.6 ± 4.7 years. Four children had unilateral SCP with an equinus gait pattern, and the remaining three children exhibited bilateral SCP with a slide/drag gait pattern. Kinematic and kinetic gait data were collected during barefoot walking at a comfortable speed. From a total of 202 steps, the detection of 202 foot-strike (FS) and 194 toe-off (TO) events by each algorithm was compared with the detection of these same events by the force plate. The error between the events detected by the algorithms and those detected by the force plate was determined in milliseconds. Repeated measures ANOVA was used to compare the errors among the algorithms.ResultsThe algorithm reported by Ghoussayni et al. showed the best performance in all situations, except for the identification of FS events on the unaffected side in children with unilateral SCP. For these events, the algorithms reported by Desailly et al. and Zeni et al. showed the best performance.ConclusionGhoussayni et al.’s algorithm can be used to detect gait events in children with SCP when a force plate is not available.  相似文献

Mobility in sub-Saharan Africa links geographically-separate HIV epidemics, intensifies transmission by enabling higher-risk sexual behavior, and disrupts care. This population-based observational cohort study measured complex dimensions of mobility in rural Uganda and Kenya. Survey data were collected every 6 months beginning in 2016 from a random sample of 2308 adults in 12 communities across three regions, stratified by intervention arm, baseline residential stability and HIV status. Analyses were survey-weighted and stratified by sex, region, and HIV status. In this study, there were large differences in the forms and magnitude of mobility across regions, between men and women, and by HIV status.We found that adult migration varied widely by region, higher proportions of men than women migrated within the past one and five years, and men predominated across all but the most localized scales of migration: a higher proportion of women than men migrated within county of origin. Labor-related mobility was more common among men than women, while women were more likely to travel for non-labor reasons. Labor-related mobility was associated with HIV positive status for both men and women, adjusting for age and region, but the association was especially pronounced in women. The forms, drivers, and correlates of mobility in eastern Africa are complex and highly gendered. An in-depth understanding of mobility may help improve implementation and address gaps in the HIV prevention and care continua.  相似文献

IntroductionThe modified Functional Movement Screen™ (mFMS) has been used to screen for mobility, stability, motor control, and balance in older adults, yet, its relationship to measures of physical fitness is not fully understood. The purpose of this study was to determine the association between mFMS scores and measures of physical fitness in older adults. A secondary aim was to determine physical fitness differences depending on mFMS Lower Body Motor Control Screen scores.MethodsOne hundred and eight older adults completed this cross-sectional study. Measurements of physical fitness included: Handgrip Strength (HG), Back-Leg Strength Dynamometer (BLS), 8 foot Up and Go (8UG), Vertical Jump (VJ), Medicine Ball Throw (MBT), Chair Stand (CST), Arm Curl (AC), and 6-Minute Walk test (6 MW). The mFMS consisted of four screens: Shoulder Mobility Screen (SMS), Deep Squat (DS), Active Straight Leg Raise (ASLR), and a Lower Body Motor Control Screen (LB-MCS). Spearman's R correlations determined associations between physical fitness tests and mFMS scores (DS, SMS, ASLR). Independent t-tests or Mann Whitney U tests determined whether individuals who passed the LB-MCS displayed higher physical fitness scores.ResultsThe DS was significantly correlated with all fitness measures (p < 0.05). Higher DS scores were associated with better HG (r = 0.31), BLS (r = 0.49), VJ height (r = 0.54), MBT (r = 0.41), CST (r = 0.57), AC (r = 0.30), 6 MW (r = 0.50), and 8UG (r = −0.61) performance. Individuals who passed the LB-MCS displayed superior BLS, 8UG test, and 6 MW test performance (p < 0.05).Discussion & conclusionHigher DS scores are associated with higher physical fitness scores. Individuals who passed the LB-MCS displayed better physical fitness scores. Practitioners may desire to use the mFMS to measure physical fitness in older adults.  相似文献

Background

Limited research has evaluated the relationship between dietary behavior and mortality among those with mobility limitations.

Objective

To examine the association between dietary behavior and mortality in a national sample of American adults with mobility limitations.

Methods

Data from the 2003–2006 National Health and Nutrition Examination Survey were utilized. Participants were followed through 2011. Based on self-report, analyzed participants included those with mobility limitations (N = 1369). Dietary behavior was assessed from the alternate healthy eating index (AHEI).

Results

For the sample, 108,010 person-months occurred with an all-cause mortality rate of 2.07 per 1000 person-months. Dietary behavior was associated with reduced all-cause mortality risk when expressed both as a continuous variable and binary variable (i.e., meeting dietary guidelines). With regard to the latter, and after adjustments, those meeting dietary guidelines (vs. not) had a 40% reduced hazard of all-cause death (HR = 0.60; 95% CI: 0.38–0.97; P = 0.03).

Conclusion

Among adults with mobility limitations, and thus, who unable to engage in sufficient physical activity, dietary behavior may have survival benefits.  相似文献

Objectives

The objectives of this study were to determine the association between patients’ functional status at discharge from skilled nursing facility (SNF) care and 30-day potentially preventable hospital readmissions, and to examine common reasons for potentially preventable readmissions.

Design

Retrospective cohort study.

Setting

SNFs and acute care hospitals submitting claims to Medicare.

Participants

National cohort of Medicare fee-for-service beneficiaries discharged from SNF care between July 15, 2013, and July 15, 2014 (n = 693,808). Average age was 81.4 (SD 8.1) years, 67.1% were women, and 86.3% were non-Hispanic white.

Measurements

Functional items from the Minimum Data Set 3.0 were categorized into self-care, mobility, and cognition domains. We used specifications for the SNF potentially preventable 30-day postdischarge readmission quality metric to identify potentially preventable readmissions.

Results

The overall observed rate of 30-day potentially preventable readmissions following SNF discharge was 5.7% (n = 39,318). All 3 functional domains were independently associated with potentially preventable readmissions in the multivariable models. Odds ratios for the most dependent category versus the least dependent category from multilevel models adjusted for patients’ sociodemographic and clinical characteristics were as follows: mobility, 1.54 (95% confidence interval [CI] 1.49–1.59); self-care, 1.50 (95% CI 1.44–1.55); and cognition, 1.12 (95% CI 1.04–1.20). The 5 most common conditions were congestive heart failure (n = 7654, 19.5%), septicemia (n = 7412, 18.9%), urinary tract infection/kidney infection (n = 4297, 10.9%), bacterial pneumonia (n = 3663, 9.3%), and renal failure (n = 3587, 9.1%). Across all 3 functional domains, septicemia was the most common condition among the most dependent patients and congestive heart failure among the least dependent.

Conclusions

Patients with functional limitations at SNF discharge are at increased risk of hospital readmissions considered potentially preventable. Future research is needed to determine whether improving functional status reduces risk of potentially preventable readmissions among this vulnerable population.  相似文献
