目的 比较3种神经病理性疼痛筛查量表在中轴型脊柱关节炎患者中的信度、效度和适用性。方法 采用painDETECT问卷、神经病理性疼痛4问卷和利兹神经病理性症状和体征评估疼痛量表对186例中轴型脊柱关节炎患者进行调查,评价量表的信度和效度。结果 painDETECT问卷、神经病理性疼痛4问卷和利兹神经病理性症状和体征评估疼痛量表的Cronbach α系数分别为0.807,0.697和0.623, Guttman分半系数分别为0.846、0.691和0.701。painDETECT问卷共提取2个公因子,累计方差贡献率55.171%。神经病理性疼痛4问卷共提取4个公因子,累计方差贡献率66.627%。利兹神经病理性症状和体征评估疼痛量表共提取2个公因子,累计方差贡献率50.836%。3种量表评估结果一致性Kappa值和相关性r值均大于0.500。患者对3种量表首选率分别为71.5%、3.2%和14.0%。结论 painDETECT问卷信度和效度良好,首选率最高。其余2种量表信度尚可,效度良好。3种量表一致性与相关性较强。 相似文献

目的构建“党员—党支部书记—党支部工作”为一体的基层党建考核体系,推动公立医院基层党建质量持续提升。方法党员实施“100分+20分”积分量化考核,党支部书记实施“1+1”述职评议考核,党支部工作实施“532”量化考核,注重三者考核结果运用的深度融合。结果2020年党员积分量化考核中,总分100分以上者占比由2019年54.65%提高至58.31%,党员参与医院发展和干事创业的积极性、主动性不断增强;2020年党支部书记考核成绩由2019年99.07分提高至99.30分,对党支部书记更加满意和信任;2020年党支部工作考核成绩由2019年89.53分提高至94.22分(P<0.001),基层党建工作质量明显提升。结论常态化考核促进了基层党建工作更加科学规范,党建与业务工作更加相融相促,推动公立医院高质量发展。 相似文献

ObjectiveData on prognostic tools for indicating mechanical ventilation in older people with COVID-19 are still limited. The aim of this research was to evaluate if the Multidimensional Prognostic Index (MPI), based on the Comprehensive Geriatric Assessment (CGA), may help physicians in identifying older hospitalized patients affected by COVID-19 who might benefit from mechanical ventilation.DesignLongitudinal, multicenter study.Settings and Participants502 older people hospitalized for COVID-19 in 10 European hospitals.MethodsMPI was calculated using 8 different domains typical of the CGA. A propensity score, Cox's regression analysis was used for assessing the impact of mechanical ventilation on rehospitalization/mortality for 90 days' follow-up, stratified by MPI = 0.50. The accuracy of MPI in predicting negative outcomes (ie, rehospitalization/mortality) was assessed using the area under the curve (AUC), and the discrimination with several indexes like the Net Reclassification Improvement (NRI) and the Integrated Discrimination Improvement (IDI).ResultsAmong 502 older people hospitalized for COVID-19 (mean age: 80 years), 152 were treated with mechanical ventilation. In the propensity score analysis, during the 90-day follow-up period, there were 44 rehospitalizations and 95 deaths. Mechanical ventilation in patients with MPI values ≥ 0.50, indicating frailer participants, was associated with a higher risk of rehospitalization/mortality (hazard ratio 1.56, 95% CI 1.09-2.23), whereas in participants with MPI values < 0.50 this association was not significant. The accuracy of the model including age, sex, respiratory parameters, and MPI was good (AUC = 0.783) as confirmed by an NRI of 0.2756 (P < .001) and an IDI of 0.1858 (P < .001), suggesting a good discrimination of the model in predicting negative outcomes.Conclusions and ImplicationsMPI could be useful for better individualizing older people hospitalized by COVID-19 who could benefit from mechanical ventilation. 相似文献

Oocyte and embryo grading is one of the most important steps in assisted reproductive technology to identify the best among cultured embryos for transfer or vitrification. The most commonly used non-invasive method is morphological assessment of gametes and embryos using a microscope. This method despite being simple and cost-effective has interobserver and intraobserver variability and subjectivity and has little to offer about the physiological state of embryos. This review sourced research articles and reviews pertaining to other non-invasive assessment methodologies from Medline and PubMed to collate latest technologies in vogue and identify novel methodologies of the future. The review assesses the current understanding in oocyte and embryo grading and touches upon novel non-invasive techniques and potential biomarkers to identify the best embryo. The latest available literature on time-lapse imaging, hierarchical algorithms, omics (consisting of proteomics and secretomes), miRNAs, mitochondrial RNAs and artificial intelligence has been accessed to summarize the enormous information available, to identify gaps in current interpretations, to identify emerging technologies and to provide direction for future research. Objective: Leukemia represents a serious public health concern as the incidence is increasing worldwide. In this study we aimed to describe the epidemiological profile of acute lymphoblastic (ALL) and myeloid (AML) leukemia, identify disease clusters and find association with possible risk factors. Methods: Data on leukemia cases were provided by the National Institute of Health of the Republic of Armenia for the period of 2012-2018. Age-standardized incidence rate was calculated using Segi World Population. SaTScan purely spatial analysis was applied to find leukemia clusters. To find association between leukemia and agricultural and mining activities and demographic data Poisson regression model was used. Results: During the studied period 259 new cases of ALL and 478 AML were recorded. The age-standardized incidence rate was 1.5 and 1.9 per 100,000 inhabitants with male to female ratio of 0.97 and 1.1 for ALL and AML, respectively. No significant changes in ALL or AML incidence trends were found. For ALL significant cluster encompassing Shirak, Lori, Tavush and Armavir provinces of Armenia was identified, while Kotayk and Ararat was provinces with the highest incidence of AML. We found significant positive association of ALL with crop density, while no elevated risk estimates were found between AML and exposure variables. Conclusion: Altogether, our results suggested that acute leukemias incidence in Armenia follows the pattern described for developing countries. 相似文献
