目的探讨ABCLOVE嗓音训练法在儿童声带小结治疗中的应用,以及对患儿发声功能的临床改善效果。方法选取2019年1月至2021年3月收治的100例声带小结患儿为研究对象,利用随机数字表分组方式将患儿分为对照组和观察组各50例。对照组采用药物治疗及行为干预,观察组给予ABCLOVE嗓音训练法及行为干预治疗,每周1次,每次30分钟,共6周。比较两组干预前后电子喉镜下声带小结改善情况、声音嘶哑评估(grade roughness breathiness asthenia strained,GRBAS)和嗓音声学分析等指标。结果治疗后观察组总嘶哑度(grade,G)、粗糙声(roughness,R)评分较对照组下降明显(P<0.05),嗓音声学分析中基频微扰(jitter)、振幅微扰(shimmer)、声门噪声激发比(glottal to noise excitation ratio,GNE Ratio)指数观察组均显著低于对照组(P<0.05),最长发声时间(MPT)观察组较对照组明显延长(P<0.05),电子喉镜检查显示观察组治疗有效率、声带小结改善情况优于对照组(P<0.05),且两组治疗前后各参数均有显著改善(P<0.05)。结论ABCLOVE嗓音训练治疗儿童声带小结疗效显著,可有效改善患儿发音功能,降低嗓音嘶哑程度,改善患儿嗓音质量。  相似文献

ObjectiveTo assess adequacy of present means of clinical communication between physicians and (Covid-19) patients’ family members, to analyse their perspectives and recommend felicitous practices for virtual conversation during ongoing pandemic.MethodsCross-sectional questionnaire-based (20 questions) anonymous online survey was conducted including patient’s relatives (Group-1) and treating physicians (Group-2), through Google Forms.ResultsResponse Rate was 82.5%. Group-1 and Group-2 included 155 and 204 respondents respectively. Group-1 preferred update by resident doctors (39%), twice a day (41.9%), daily case-summaries (80%) and hand-written document/electronic messages (53%,31%) as consent. Whereas Group-2 favored update by senior consultants (63%), daily one appraisal (55.9%) and scanned copies of hand written consent (81%) before high-risk procedures. The groups broadly agreed on the desired duration for a fruitful discussion (5–10 min) and designating one responsible person from the family for daily appraisal.ConclusionUse of modern techniques/technologies of communication (voice/video calls, texts) during the ongoing pandemic is acceptable to majority.Practice implicationsStudy proposes a senior physician should communicate to a designated responsible family member at-least once a day for stable and twice a day for critical covid patients (more if patient’s health condition changes), either by voice or video calls for 5–10 min.  相似文献

目的 比较吞咽障碍与吞咽正常的脑卒中患者嗓音功能及心理状态,为制订针对性康复策略提供参考。 方法 便利选取337例脑卒中患者,通过洼田饮水试验和标准吞咽功能评估分为吞咽障碍组122例与吞咽正常组215例。采用嗓音障碍指数量表、GRBAS法、Praat声学软件进行主、客观嗓音功能评估,采用医院焦虑抑郁量表评估心理状态。 结果 与吞咽正常组比较,吞咽障碍组GRBAS法中的总嘶哑度、粗糙度、气息度、无力度评分及嗓音障碍指数量表评分、客观声学指标差异有统计学意义(均P<0.05);吞咽障碍组与吞咽正常组焦虑、抑郁评分比较,差异有统计学意义(均P<0.05)。 结论 脑卒中吞咽障碍患者嗓音功能受损及焦虑抑郁情绪显著高于吞咽正常患者,需开展吞咽-嗓音-心理联合康复。  相似文献

This paper revisits a 2003 publication in Nursing Philosophy: The need for accurate perception and informed judgement in determining the appropriate use of the nursing resource: hearing the patient's voice. The author suggests that the basic ideas and focus of this 16‐year‐old paper are still topical and relevant in considerations of nursing care. However, it is also suggested that greater attention to the importance of the nurse–patient relationship in considerations of resource allocation, and potential rationing of nursing care, would have strengthened the original paper.  相似文献

ABSTRACT

Each year, over 600 youth under the age of 25 experience literal homelessness in Austin, Texas. Of these youth, 76% have a history of involvement with foster care or the juvenile justice system—far exceeding the rates of system involvement observed among homeless youth in other large communities in the United States (approximately 54%; Voices of Youth Count, 2017). Recently, Austin was selected by the U.S. Department of Housing & Urban Development as one of ten communities nationwide to participate in the Youth Homelessness Demonstration Program (YHDP), a federal initiative designed to effectively end youth homelessness by 2020. Youth with lived experience are central players in the development and implementation of Austin's response to the youth homelessness crisis. A group of twelve of these youth (the “Austin Youth Collective to End Youth Homelessness,” or AYC) serve as key decision-making partners within the Austin YHDP team. Representatives of the AYC are deeply involved at every level of the community planning process, from the development of youth-centric housing options to system redesign efforts to ensure that youth do not exit the foster care or juvenile justice systems to homelessness. The article provides an overview of the development and contributions of the AYC—including their programmatic recommendations and advocacy work—as well as key recommendations for communities looking to leverage youth voice at the systems level.  相似文献

目的 基于《国际疾病分类》第11次修订本(ICD-11)、《国际功能、残疾和健康分类》(ICF)和《国际健康干预分类》(ICHI)探讨言语嗓音功能障碍的诊断标准、功能评估和整体康复干预方案。 方法 基于ICD-11和ICF对言语嗓音障碍及其常见疾病诊断标准进行分析,确认疾病和功能诊断标准。采用ICF的理论,分析言语嗓音功能评估工具的主要内容和评估指标。基于ICF和ICHI对现有言语嗓音功能康复方法进行分析,构建结构化言语嗓音康复干预方案。 结果和结论 言语嗓音功能障碍诊断为“语音障碍”(MA82),属于“涉及言语或语音的症状或体征”中的一种,与之相关的疾病为言语障碍(MA80)、喉恶性肿瘤(2C23)、脑出血(8B00)、喉水肿(CA0H.3)等。言语嗓音功能障碍属身体功能障碍,并可能影响交流等活动和参与(d3、d7、d8、d9),以及环境因素和个人因素。对言语嗓音功能的评估应从嗓音产生功能(b3100)和嗓音音质功能(b3101)两方面进行。结构化言语嗓音康复治疗方案主要涉及身体结构与功能、活动和参与、环境因素和健康行为四个方面,方法包括评估类(评估、测试、观察)、训练治疗类(训练、辅导)、教育咨询类(教育、建议、咨询)和支持类(社会支持和心理支持)等。  相似文献

ABSTRACT

Compared with humans, who have more powerful auditory ability in discriminating and identifying speakers in noisy environments, traditional forensic automatic speaker recognizers do not perform well when dealing with noisy recordings. This paper proposes a GMM-UBM Forensic Automatic Speaker Recognition (FASR) System to reduce the effect of noise on performance. The system uses Gammatone Frequency Cepstral Coefficients (GFCC) based on an auditory periphery model and also incorporates a Principal Component Analysis (PCA) algorithm. The system was tested and validated using Mandarin voice databases compromised with different levels of white noise and office noise. The performance of the system was compared with a baseline system using Mel Frequency Cepstral Coefficients (MFCC) and also PCA under the same conditions. The results show that the performance of the combined GFCC system achieved a substantial improvement when compared with the baseline MFCC system under conditions of a high level of office noise.  相似文献
