目的: 分析慢性乙型肝炎病毒(HBV)感染后不同临床阶段外周血血栓弹力图(TEG)、常规凝血指标检测的水平,探讨TEG与传统凝血指标检测在评估慢性肝病患者凝血功能方面的意义。方法: 选取2019年9月~2020年2月慢性乙型肝炎患者115例,依照慢性肝病患者不同临床阶段的诊断标准分为四组:慢性乙型肝炎组42例(慢乙肝组),乙肝肝硬化组38例(肝硬化组),HBV相关肝衰竭组35例(肝衰竭组),筛选同期30例献血员为健康对照组;分别抽取研究对象外周血检测血常规、常规凝血全套、TEG参数,并进行比较。结果: (1)四组中慢乙肝组较对照组常规凝血指标及TEG参数的结果差异均无统计学意义(P>0.05)。(2)肝硬化、肝衰竭组患者外周血浆PT、APTT较对照组及慢乙肝组明显延长,INR值明显升高( P<0.001),而PTA、FIB、全血中PLT较对照组及慢乙肝组明显降低( P<0.001)。肝衰竭组与肝硬化患者组相比较外周血浆中的PT、APTT较肝硬化患者延长(P<0.001)、INR高于肝硬化患者(P<0.001);肝衰竭患者外周血浆中PTA、FIB、全血中PLT明显低于肝硬化患者(P<0.001)。(3)肝硬化、肝衰竭组患者外周全血R值、K值较对照组及慢乙肝组明显延长,CI值明显升高(P均<0.001),而α-角、MA较对照组及慢乙肝组明显减低( P<0.001)。肝衰竭组与肝硬化组相比较外周全血中的R值、K值较肝硬化患者延长(P<0.001)、CI值高于肝硬化患者(P<0.001);肝衰竭患者外周全血中α-角、MA明显低于肝硬化患者(P<0.001)。(4)R、K、PT、APTT、INR值评估肝衰竭组凝血功能的ROC曲线下面积分别为0.928、0.864、0.805、0.851、0.767(P<0.05)。R值及K值曲线下面积最大,且敏感度及特异度均高于常规凝血指标(P<0.05)。结论: TEG是评估慢性肝病患者凝血状态的一种综合、可靠指标,TEG联合检测常规凝血指标,对全面评估慢性肝病患者凝血功能的变化发挥重要作用。  相似文献

目的:探讨大动脉粥样硬化型脑梗死患者阿司匹林抵抗(AR)的临床特征,并分析AR发生的危险因素。方法:依照TOAST分型标准,选择发病7 d内急性大动脉粥样硬化型脑梗死患者160例,统一给予拜阿司匹林(200 mg,1次/日)口服抗血小板聚集治疗;7 d后用血栓弹力图(TEG)检测血小板抑制率,并根据结果将患者分为阿司匹林敏感(AS)组和AR组;比较2组临床资料及生化指标水平,分析AR的危险因素。结果:TEG检测结果显示,AR者42例(26.3%),AS者118例;2组花生四烯酸抑制率分别为(42.6±18.9)%和(76.3±15.8)%(P0.05)。单因素分析结果显示,AR组吸烟、饮酒、糖尿病史患者比例及血清Hcy、hs-CRP水平显著高于AS组(P0.05)。Logistic回归分析显示,糖尿病史、吸烟及高Hcy是AR发生的独立危险因素。结论:大动脉粥样硬化型脑梗死患者中存在AR,糖尿病史、吸烟史及高Hcy水平是AR发生的独立危险因素。  相似文献

Background: Thromboelastography (TEG) is utilized as a point-of-care test of coagulation function to improve evidence-based blood product replacement in adults. In contrast to standard indices of coagulation, TEG reflects the dynamic interactions among the elements involved in hemostasis, including fibrinolysis. Although normal adult values and various abnormalities have been characterized, normative values for term neonates have not been described. Studies of neonatal TEG remain limited and have small sample sizes with inconsistent methodology. The aim of this study is to provide normative data on healthy term neonates, and to assess the impact of mode of delivery on TEG parameters at term.

Methods: Venous umbilical blood was obtained from the placenta within 10?min of delivery. TEG analysis of citrated kaolin-activated samples was performed for 50 healthy term vaginal and 50 cesarean deliveries. Samples collected for cesarean sections were from scheduled cases or unscheduled ones due to failure of progression of labor.

Results: Healthy neonates with uncomplicated vaginal term deliveries resulted in the following TEG parameters: R: 5.41?±?1.34 (mean?±?SD) min; K: 1.62?±?0.75?min; α-angle: 65.39?±?8.77°; MA: 65.86?±?5.81?mm; and LY30: 1.40?±?1.18%. Results for the cesarean delivery neonatal TEG assays showed: R: 5.51?±?1.74 (mean?±?SD) min; K: 1.52?±?0.47?min; α-angle: 64.15?±?4.61°; MA: 64.15?±?4.61?mm; and LY30: 2.44?±?3.51%. Of note, no statistical differences were observed (p?Conclusion: TEG measurements from term neonates were no different when the neonates were delivered vaginally or by cesarean section. Labor had no effect on neonatal TEG levels. Neonatal TEG values may therefore serve as insight for fetal values at the appropriate postconceptional age.  相似文献

The clotting cascade is a complex process and is an important survival mechanism. Major haemorrhage and thromboembolic events remain major causes of increased morbidity and mortality. Drugs affecting coagulation have primarily been utilized to treat or reduce the risk of thromboembolic events. However, the recent progress in the management of major trauma and treating coagulopathy has resulted in further research and development of drugs that improve clotting function. Knowledge of drugs used for both clinical circumstances is now required when working in anaesthesia or intensive care.  相似文献

目的探讨血栓弹力图(TEG)在预防肿瘤患者经外周置入中心静脉导管(PICC)相关性静脉血栓中的作用。方法将符合纳入、排除标准的217名患者随机分为观察组112名和对照组105名。观察组患者分别于PICC置管前、置管后监测TEG,并根据TEG参数提示的凝血状态指导口服阿司匹林预防血栓治疗。对照组患者按常规PICC置管。两组患者分别于PICC置管后6周内每周1次,行血管超声检查,比较观察组血液高凝状态者口服阿司匹林干预前及干预1周后TEG变化情况和2组PICC相关性静脉血栓发生率。结果观察组26例血液高凝状态者口服阿司匹林干预后,TEG各项指标与干预前比较,均P<0.01,差异有统计学意义;观察组与对照组PICC相关性静脉血栓发生率分别为0.89%(1/112)、20.95%(22/105),2组静脉血栓比较,差异有统计学意义(χ^2=23.014,P<0.001)。结论 TEG可有效预测PICC相关性静脉血栓的形成,为患者建立个体化的防治血栓的措施。  相似文献

目的:探讨应用血栓弹力图(thromboelastogram ,TEG )监测胃癌患者围手术期凝血功能的动态变化及其临床意义。方法:选取2014年3 月至2015年5 月期间在新乡市中心医院确诊为胃癌并行手术的178 例患者为试验组,并按肿瘤不同分期、不同浸润深度及有无淋巴结转移进行分组,对所有手术患者进行术前、术后TEG 动态监测,另选取60例健康体检者为正常对照组,行TEG 检测,通过对比分析其围手术期凝血功能变化。结果:实验组术前、术后与对照组比较,其TEG 参数R 、K 值明显减小,Angle、MA、CI值则显著增大,差异具有统计学意义(P < 0.05)。 实验组患者术前、术后对比K 值显著降低;Angle值、MA、CI 显著升高(P < 0.05)。R 、LY30等观测值均无显著变化。肿瘤不同分期、不同浸润深度及有无淋巴结转移的患者相比较,其TEG 监测值均有显著性差异。结论:胃癌患者围手术期表现为高凝状态,且与患者肿瘤分期、浸润深度及淋巴结转移呈正相关。动态监测胃癌围手术期TEG 可为临床提供有价值的凝血信息,并对提高胃癌手术的安全性和降低术后相关并发症有积极的临床意义。   相似文献

Congenital factor VII deficiency is an autosomal recessive serious disorder of blood coagulation with wide genotypic and phenotypic variations. The clinical presentation can vary from asymptomatic patients to patients with major bleedings in severe deficiency (factor VII <1%). Investigations show prolonged PT and low factor VII. Treatment modalities include FFP and repeated recombinant factor VII infusions. We hereby report the first successful LRLT for factor VII deficiency in an infant, the first‐ever youngest baby reported worldwide. A six‐month‐old male child presented with easy bruisability, ecchymotic patches, hematuria, and convulsions. CT of the head showed subdural hemorrhage, which was treated conservatively. He had markedly increased PT (120 s) with normal platelets, and aPTT with factor VII level <1%. Despite the treatment by rFVIIa administration weekly, which was very expensive, he still had repeated life‐threatening bleeding episodes. LRLT was performed with mother as the donor, whose factor VII level was 57%. A factor VII infusion plan for pre‐, intra‐ and postoperative periods was formulated and TEG followed. Postoperatively, his factor VII started increasing from third day and was 38% on 24th day with PT <14 s. He had uneventful intraoperative and postoperative courses. LT is a safe and definite cure for factor VII deficiency.  相似文献

The thromboresistant properties of hydrophilic gels based on copolymers of nitrogen-containing heterocyclic vinyl compounds with vinyl monomers were investigated. The hydrophilic gels were shown to prevent adsorption of fibrinogen, activation of procoagulants, and adhesion of platelets. Hydrogen surfaces possess selective affinity for plasma albumin. The authors consider that the thromboresistant effect of hydrophilic gels is due to the competitive action of plasma albumin.
