目的 采用两样本孟德尔随机化研究方法探讨血清生长分化因子15(GDF15)水平与慢性淋巴细胞白血病(CLL)发生之间的关联。方法 基于欧洲人群血清GDF15和CLL的全基因组关联研究公开数据库,筛选与血清GDF15水平相关的遗传变异位点作为工具变量,采用逆方差加权法评估遗传学预测的血清GDF15浓度与CLL发生的关联,采用最大似然比法进行敏感性分析,采用MR-Egger回归探讨工具变量潜在多效性。结果 研究共纳入3个单核苷酸多态位点作为工具变量,逆方差加权法结果显示,血清GDF15水平与CLL发生风险之间存在负相关,GDF15浓度每升高一个标准差(SD),CLL发生风险降低33%(95%置信区间:2%~54%)(P=0.039)。敏感性分析得到了一致的结果。此外,MR-Egger回归未发现存在多效性。结论 本研究结果提示,在欧洲人群中,血清GDF15水平与CLL发生之间可能存在负相关,仍需大样本人群研究及体内外实验进一步阐明GDF15在CLL发生发展中的作用及其潜在生物学机制。  相似文献

目的:对2型糖尿病患者进行回顾性研究,探讨中医证型分布和相关因素之间的联系。方法:将湖南省卫生健康委信息统计中心的三甲中医院病历数据进行筛选、清理,最终选取898例患者作为研究对象,并进行回顾性分析,研究患者中医证型并分析证型与年龄、性别、实验室指标之间的关系。结果:898例2型糖尿病患者中证型最多的为肝肾阴虚,合并疾病较多的是心脑血管疾病;在年龄与性别方面,不同中医证型患者分布,差异有统计学意义(P<0.05);实验室指标方面,糖化血红蛋白(HbA1c)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TAG)、血肌酐(Cr)、血尿素氮(BUN)、谷草转氨酶(GOT)、平均血小板体积(MPV)、纤维蛋白原(FIB)不同证型间分布差异均有统计学意义(均P<0.05),其余指标不同证型间比较差异无统计学意义(P>0.05)。结论:性别、年龄、实验室指标可以在一定程度上反映出2型糖尿病不同中医证型的区别,对于2型糖尿病的治疗应及时检测以上指标的水平值,并明确相应的证型,针对性地给予辨证论治或中西医结合治疗,在疾病的中后期要预防心脑血管疾病以及各种并发症的发生,改善患者的生命质量。  相似文献

BackgroundThe prevalence of osteoarthritis (OA) is constantly increasing with age. Adipose-derived (AD-) and umbilical cord-derived (UC-) mesenchymal stem cells (MSCs) are attractive alternatives in OA therapy and regenerative medicine. However, whether there are differences in the efficacy of MSCs derived from different tissues in the cartilage regeneration, and the frequency of administration of MSCs needs to be further studied.ExperimentUC-MSCs and AD-MSC were isolated from the umbilical cord and subcutaneous fatty tissue of humans respectively and identified by flow cytometry. In vitro, the proliferation ability and chondrogenic potential of AD-MSCs and UC-MSCs were analyzed. In vivo, forty-three Sprague-Dawley rats were used for the OA model induced by ACLT surgery. OA rats were divided into a sham group, an ACLT model group, and two therapy groups (treated with AD-MSCs or UC-MSCs). Therapy groups were treated using a single or repeated twice injection of AD-MSCs and UC-MSCs at a concentration of 1.0 × 106 cells and were followed up for 12 weeks. Serial sections of knees were examined for histological, immunohistochemical and TUNEL analysis.ResultsWe demonstrated that the proliferation of UC-MSCs was higher than that of AD-MSCs, consistent with the bigger pellets from UC-MSCs in a chondrogenic induction medium. Degeneration of articular cartilage was observed in histological appearance of Safranine O and Toluidine blue staining, and quantitative results of modified Mankin’s Score. Importantly, both AD-MSCs and UC-MSCs transplantation significantly attenuated ACLT surgical-induced OA development. In addition, ACLT-induced reduction in cartilage extracellular matrix synthesis (aggrecan) was significantly suppressed by AD-MSCs or UC-MSCs transplantation. TUNEL assay showed that AD-MSCs and UC-MSCs treatments significantly protected chondrocytes against apoptosis compared with the ACLT group. No significant differences were observed between single injections and repeated twice injections.ConclusionsThe current study suggested that, in vitro, AD-MSCs and UC-MSCs showed a comparable chondrogenic potential, although UC-MSCs displayed a superior proliferation capacity. Furthermore, our results confirmed that the injection of AD-MSCs and UC-MSCs, either single or repeated twice, could significantly inhibit the progression of ACLT-induced osteoarthritis with a similar effect, and MSCs transplantation can decrease the apoptosis of articular chondrocytes caused by ACLT.  相似文献

目的 探讨外周血单核细胞髓样分化因子88(My D88)与Toll样受体4(TLR4)在早产儿窒息后多器官功能障碍综合征(MODS)中的表达,为早产儿窒息后MODS的早期诊断及预后评估提供新思路。方法 连续性纳入2019年1月-2020年12月唐山市妇幼保健院新生儿科收治的115例窒息早产儿进行前瞻性研究,按照1 min Apgar评分将窒息早产儿分为轻度窒息组(n=70)与重度窒息组(n=45),以同期的50例健康早产儿作为对照组。比较3组早产儿中MODS发生率、MODS病死率及单器官损害发生率,外周血单核细胞My D88 mRNA及蛋白与TLR4 mRNA及蛋白的表达水平。结果 重度窒息组、轻度窒息组与对照组的MODS发生率分别为73.33%、31.43%、6.00%,MODS病死率分别为24.44%、5.71%、0,3组间比较差异有统计学意义(χ2=47.845、16.978,P<0.001);重度窒息组与轻度窒息组的单器官损害发生率明显高于对照组(P<0.05),重度窒息组明显高于轻度窒息组(P<0.05);重度窒息组与轻度窒息组的My D88 mRNA及蛋白与TLR4 mRNA及蛋白均明显高于对照组,且重度窒息组明显高于轻度窒息组(F=4.251、4.804、4.772、4.690,P<0.001)。结论 早产儿窒息后MODS发生率、外周血单核细胞My D88与TLR4表达水平均明显增高,My D88与TLR4有望成为早产儿窒息后MODS的早期诊断及预后评估指标。  相似文献

Molecular markers can serve as diagnostic tools to support pathological analysis in thyroid neoplasms. However, because the same markers can be observed in some benign thyroid lesions, additional approaches are necessary to differentiate thyroid tumor subtypes, prevent overtreatment and tailor specific clinical management. This applies particularly to the recently described variant of thyroid cancer referred to as noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP). This variant has an estimated prevalence of 4.4% to 9.1% of all papillary thyroid carcinomas worldwide. We studied 60 thyroid lesions: 20 classical papillary thyroid carcinoma (CPTC), 20 follicular variant of PTC (FVPTC) and 20 NIFTP. We examined morphological and molecular features to identify parameters that can differentiate NIFTP from the other PTC subtypes. When blindly investigating the nuclear architecture of thyroid neoplasms, we observed that NIFTP has significantly longer telomeres than CPTC and FVPTC. Super-resolved 3D-structured illumination microscopy demonstrated that NIFTP is heterogeneous and that its nuclei contain more densely packed DNA and smaller interchromatin spaces than CPTC and FVPTC, a pattern that resembles normal thyroid tissue. These data are consistent with the observed indolent biological behavior and favorable prognosis associated with NIFTP, which lacks BRAFV600E mutations. Of note, next-generation thyroid oncopanel sequencing was unable to distinguish the thyroid cancer histotypes in our study cohort. In summary, our data suggest that 3D nuclear architecture can be a powerful analytical tool to diagnose and guide clinical management of NIFTP.  相似文献

Over the last 40 years, the incidence and prevalence of gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs) have continued to increase. Compared to other epithelial neoplasms in the same organ, GEP-NENs exhibit indolent biological behavior, resulting in more chances to undergo surgery. However, the role of surgery in high-grade or advanced GEP-NENs is still controversial. Surgery is associated with survival improvement of well-differentiated high-grade GEP-NENs, whereas poorly differentiated GEP-NENs that may benefit from resection require careful selection based on Ki67 and other tissue biomarkers. Additionally, surgery also plays an important role in locally advanced and metastatic disease. For locally advanced GEP-NENs, isolated major vascular involvement is no longer an absolute contraindication. In the setting of metastatic GEP-NENs, radical intended surgery is recommended for patients with low-grade and resectable metastases. For unresectable metastatic disease, a variety of surgical approaches, including cytoreduction of liver metastasis, liver transplantation, and surgery after neoadjuvant treatment, show survival benefits. Primary tumor resection in GEP-NENs with unresectable metastatic disease is associated with symptom control, prolonged survival, and improved sensitivity toward systemic therapies. Although there is no established neoadjuvant or adjuvant strategy, increasing attention has been given to this emerging research area. Some studies have reported that neoadjuvant therapy effectively reduces tumor burden, improves the effectiveness of subsequent surgery, and decreases surgical complications.  相似文献
