目的探讨DNA损伤检查点蛋白调节子1(MDC1)与错配修复(MMR)蛋白在子宫内膜癌中的表达及其与相关临床特征的关系。方法回顾性分析126例子宫内膜癌患者的临床及病理资料,收集患者手术或刮宫标本进行HE染色和免疫组化染色,检测MDC1、MMR蛋白(MSH6、MSH2、PMS2、MLH1)的表达,并根据MDC1及MMR蛋白免疫组化结果,分析MDC1、MMR蛋白表达及结合两种蛋白不同表达与患者临床特征的关系。采用Kaplan-Meier法绘制生存曲线图分析MDC1和MMR蛋白联合检测与总生存率的关系。采用Spearman秩相关性分析MDC1与MMR蛋白表达的相关性。结果与MDC1阳性表达患者相比,MDC1阴性表达患者年龄较小,主要为低级别(1~2级)子宫内膜样腺癌(P<0.05);与MMR表达完整(MMR-p)患者相比,MMR表达缺失(MMR-d)患者年龄较小,主要为低级别(1~2级)子宫内膜样腺癌(P<0.05)。MDC1阴性表达/MMR-p、MDC1阳性表达/MMR-p、MDC1阴性表达/MMR-d、MDC1阳性表达/MMR-d各组患者年龄、组织学类型、组织学分级比较,差异具有统计学意义(P<0.05);而临床分期、肌层浸润深度比较,差异无统计学意义(P>0.05)。MDC1、MMR蛋白联合检测与患者的总生存率无关(P>0.05),MDC1阴性表达占所有子宫内膜癌患者的9.5%(12/126),与MMR-d呈正相关(P<0.001)。结论MDC1、MMR蛋白多在低级别子宫内膜癌中失表达,并且MDC1阴性表达与MMR-d具有正相关性,提示MDC1失表达与子宫内膜癌的微卫星不稳定性密切相关。  相似文献

Background

Advanced low-grade ovarian carcinoma (LGOC) is difficult to treat. In several studies, high estrogen receptor (ER) protein expression was observed in patients with LGOC, which suggests that antihormonal therapy (AHT) is a treatment option. However, only a subgroup of patients respond to AHT, and this response cannot be adequately predicted by currently used immunohistochemistry (IHC). A possible explanation is that IHC only takes the ligand, but not the activity, of the whole signal transduction pathway (STP) into account. Therefore, in this study, the authors assessed whether functional STP activity can be an alternative tool to predict response to AHT in LGOC.

Methods

Tumor tissue samples were obtained from patients with primary or recurrent LGOC who subsequently received AHT. Histoscores of ER and progesterone receptor (PR) were determined. In addition, STP activity of the ER STP and of six other STPs known to play a role in ovarian cancer was assessed and compared with the STP activity of healthy postmenopausal fallopian tube epithelium.

Results

Patients who had normal ER STP activity had a progression-free survival (PFS) of 16.1 months. This was significantly shorter in patients who had low and very high ER STP activity, with a median PFS of 6.0 and 2.1 months, respectively (p < .001). Unlike ER histoscores, PR histoscores were strongly correlated to the ER STP activity and thus to PFS.

Conclusions

Aberrant low and very high functional ER STP activity and low PR histoscores in patients with LGOC indicate decreased response to AHT. ER IHC is not representative of functional ER STP activity and is not related to PFS.  相似文献

Objectives: Cytokeratin (CK) proteins play a vital role in cancer diagnosis, of which,CK-7 is a prominent marker of squamocolumnar junction cells corresponding to the the initiating site of cervical cancer.The current study is aimed to evaluate the expression pattern of CK-7 and to corelate with the clinicopathological features in patients with cervical dysplasia and invasive squamous cell carcinoma. Methodology: The hysterectomy and biopsy specimens from women with cervical dysplasia (n=60) and carcinoma (n=60) were evaluated histopathologically and processed for immunohistochemistry (IHC) staining to assess for CK-7 expression. The relationship between CK-7 expression and tumor characteristics like histological type of cervical intraepithelial neoplasia (CIN), tumor type and grade was evaluated. Data was analyzed using the Chi-square test ,wherein the  p value ≤ 0.05 were taken for statistical significance. Results: Positive CK-7 expression was observed in 25 (41.67%) dysplasia and in 34 (56.67%) carcinoma cases. Majority of the cases were CIN III (n=31, 51.67%), large cell non-keratinizing tumor type (n=54, 90%) and moderately differentiated grade of tumor (n=52, 86.67%), out of which 18 (58.1%), 34 (62.96%) and 30 (57.69%) cases were CK-7 positive, respectively. The difference in clinical diagnosis and tumor characteristics over CK-7 expression was significant (p<0.05). The pattern of CK-7 expression in dysplasia and carcinoma cases were diffuse in 23 (38.33%) and 31 (51.67%) respectively and patchy in 2 (3.33%) and 3 (5%) of them, respectively. Conclusion: Significant positive CK-7 expression in cervical dysplasia and carcinoma indicates a good clinical course and its role as a useful predictable marker for cancer progression.  相似文献

Objectives: Oral Submucous Fibrosis (OSMF) is potentially malignant disorder known to transform into oral cancer. The aim of this study is to determine the degree and pattern of expression of aberrant Ki67 in OSMF, oral squamous cell carcinoma (OSCC) and in normal oral mucosa  patients (NOM). The objective is to correlate between Ki67 expression and degree of dysplasia in OSMF patents and also Ki67 expression with clinical and histological grading of OSMF and OSCC patient. Materials and Methods: A prospective cross-sectional study was conducted with purposive sampling technique from Jan 2017 to April 2020.The study groups consist of 35 OSMF cases, 10 cases of each OSCC and NOM. The samples were immunohistochemically analyzed for expression Ki67 antigen using chi-square test (P < 0.05). Results: The expression of Ki67 was significantly higher in OSMF than that of NOM samples, but less than that of OSCC samples. Present study showed correlation between expression of Ki67 with clinical staging and histological grading both in OSMF and OSCC patients. Conclusion: The study demonstrated a high incidence of Ki67 over expression in, OSMF and OSCC. Hence, Ki-67 can be widely used as a proliferation marker to measure growth fraction of cells in OSMF and also for determining the severity of epithelial dysplasia.  相似文献

目的:探讨细针抽吸细胞块免疫组织化学检查对乳腺癌的诊断价值。方法:选取2017年1月~2019年12月82例疑似乳腺癌患者,均采用细针抽吸细胞块免疫组织化学检查、空芯针抽吸细胞块免疫组织化学检查,观察两种检验方法与病理结果的差异性。结果:病理诊断阳性患者62例,阴性20例;细针抽吸细胞块免疫组织化学检查结果中,阳性56例,阴性26例;空芯针抽吸细胞块免疫组织化学检查结果中,阳性57例,阴性25例;细针抽吸细胞块免疫组织化学检查特异性、敏感性、阳性预测值、阴性预测值与空芯针抽吸细胞块免疫组织化学检验结果无明显对比差异(P>0.05);1例患者实施细针、空芯针穿刺后,出现一定的疼痛、血肿情况。结论:细针抽吸细胞块免疫组织化学检查对乳腺癌具有较高的诊断价值,具有较高的诊断安全性。  相似文献

目的:探讨干扰素-α诱导蛋白27(IFI27)在胰腺癌组织中的表达情况及其临床意义。方法:免疫组织化学法检测IFI27蛋白在胰腺癌、癌旁及慢性胰腺炎组织中的表达情况,分析阳性表达与胰腺癌患者相关临床病理参数和预后的关系。利用生物信息学方法基于TCGA数据库分析IFI27在胰腺癌组织中的表达差异和预后相关性。结果:IFI27在胰腺癌组织中表达的阳性率高于癌旁以及慢性胰腺炎组织(P<0.05),且与血管侵犯、淋巴结转移以及肿瘤分化程度相关。胰腺癌患者的总体生存期与肿瘤分化程度、血管侵犯、淋巴结转移以及IFI27阳性表达相关。多因素分析显示IFI27阳性表达是胰腺癌患者预后的独立危险因素。基于TCGA数据库分析得出IFI27在胰腺癌组织中的表达量高于正常组织(P<0.05),生存曲线提示IFI27低表达患者的预后优于高表达患者(P<0.05)。结论:IFI27高表达的胰腺癌患者预后差,IFI27阳性表达是胰腺癌患者的独立预后指标。  相似文献

目的 观察术前超声联合免疫组织化学(免疫组化)预测甲状腺乳头状癌(PTC)颈部中央区淋巴结转移(CLNM)的价值。方法 回顾性分析153例接受手术治疗的PTC患者,根据术后病理将其分为CLNM阳性组(n=75)和CLNM阴性组(n=78);比较2组患者术前超声及免疫组化检测结果,以logistic回归分析评价术前超声联合免疫组化预测颈部CLNM的价值。结果 组间患者年龄、病灶最大径、微钙化、纵横比、紧邻被膜及HBME-1表达水平差异均有统计学意义(P均<0.05)。PTC患者年龄≤45岁、病灶最大径>2.0 cm、微钙化、紧邻被膜及HBMI-1(++/+++)为颈部CLNM的危险因素;据此建立的logistic模型用于预测PTC颈部CLNM的受试者工作特征(ROC)曲线下面积为0.823[95%CI(0.785,0.888)]。结论 术前超声联合免疫组化可预测PTC颈部CLNM;PTC患者年龄≤45岁、病灶最大径>2.0 cm、微钙化、紧邻被膜及HBMI-1(++/+++)均为颈部CLNM的危险因素。  相似文献

Background: The presence of cancer stem-like cells within tumor microenvironment distinctly governs response to chemo-radiotherapy. The ALDH1 (Aldehyde dehydrogenase 1) has emerged as a cancer stem cell (CSC) marker in various tumors. The aim of the study was to examine the expression of ALDH1 in HNSCC patients undergoing radiotherapy to evaluate its correlation with clinicopathological parameter, treatment response and survival. Methods: Expression of ALDH1 was evaluated by immunohistochemistry in 90 histopathologically confirmed HNSCC patients and 90 matched controls. The association between ALDH1 expression, clinicopathological parameters and treatment response was determined. Results: The immunohistochemistry results showed that ALDH1 was consistently expressed in all the HNSCC specimens although at different intensities. On the other hand, control specimens did not show similar expression of ALDH1. ALDH1 expression demonstrated statistically significant association with tumor size (p<0.001), lymph node status (p<0.001), stage (p<0.001), grade (p<0.001) and treatment response (p<0.001). Multivariate ordinal logistic regression analysis indicated alcohol and ALDH1 as an independent predictor of responsiveness to radiotherapy in HNSCC patients. Multivariate Cox regression analysis indicated that lymph node status (p=0.020), grade (p=0.006) and recurrence (p=0.002) were potential independent predictors of overall survival. Conclusion: From previous studies, ALDH1 has been contemplated not only as a promising prognostic and diagnostic marker but also as a likely drug target. Our study gives new understanding regarding the association between ALDH1, cancer prognosis and radioresistance. Our findings suggest that ALDH1, lymph node status, grade and alcohol could be the viable targets for HNSCC and it also provides new prospects for radiotherapy sensitivity in HNSCC.  相似文献

Background: Tumor cell autophagy can influence cellular immunity by participation in the recognition and modification of tumor-related antigens. Objectives: The objective of this study was to evaluate the immunohistochemical expression of the autophagy-related marker; light chain 3B (LC3B) in tumor cells and the assessment of T lymphocytes by a cluster of differentiation 3 (CD3) in gliomas, and to correlate them with the available clinic-pathological variables in glioma patients. Materials and methods: Immunohistochemical staining for LC3B and CD3 was performed on 60 paraffin-embedded glioma tissue. LC3B immunoreactivity score of 0-6 was designated negative, and those scoring 7-12 were considered positive. The median level of CD3 positive T lymphocytes was calculated for both high and low-grade gliomas. In low-grade gliomas, the CD3 expressing T lymphocytes equal to or more than 2.6, were considered positive while in high-grade gliomas, those equal to or more than 16 were considered positive. Results: LC3B expression in tumor cells was detected in 24/60 (40%) of gliomas. Expression of LC3B was significantly more frequent in high-grade gliomas (23/33, 69.7%) compared to low grade ones (1/27, 5%), (p value= 0.000). LC3B expression was correlated with the patients age (P value= 0.047) & histological variants (P value= 0.000). CD3 positive T lymphocytes were significantly more prominent in high-grade gliomas (25/33 , 41.7%)  than low-grade ones (2/27, 3.3%), (P value= 0.001). A significant association was noted between CD3 expression and the patients age (P value= 0.003), sex (P value: 0.035) and histological variants (P value= 0.001). LC3B expression in tumor cells was significantly correlated with CD3 positive T lymphocytes (P value: 0.000). Conclusion: Autophagic activity of tumor cells and T lymphocyte infiltrates were reported more in high-grade gliomas compared to low-grade ones, giving high-grade gliomas the chance in autophagy target therapy & immunotherapy.  相似文献
