IntroductionThere is not much information about the care of infants with hypoxic-ischaemic encephalopathy (HIE) treated with therapeutic hypothermia (TH) in Spain. This includes whether protocols are routinely used, the type of neuro-monitoring performed, and how information on the neurological prognosis is presented to families. The answers to these would allow to detect and implement areas of improvement.MethodA cross-sectional analysis was performed on the responses to structured questionnaires sent to all the Spanish neonatal units that were performing TH in June 2015. Questions were divided into 5 sections: 1) the availability of protocols and technological resources, 2) the use of neuro-monitoring tools, 3) the knowledge and training of the professionals; 4) the prognostic information given to the parents; and 5) the discharge report and the follow-up plan.ResultsMost centres (95%) use servo controlled whole-body cooling methods and have specific management protocols. Sedation is used in 70% of centres, and in 68% of them the onset of enteral feeding is delayed until the end of the cooling period. Amplitude-integrated electroencephalography monitoring is used in more than 80% of the centres, although only in 50% are nurses able to interpret it. Cerebral oxygen saturation is not often monitored (16%). As regards diagnostic-prognostic studies, neuroimaging is universal, but brain damage biomarkers are hardly used (29%). Prognostic information is offered within the first 72 posnatal hours in 21% of the centres, and is given without the presence of the nurse in 70% of the centres. Follow-up is performed by a neuro-paediatrician (84%), with an uneven duration between centres.ConclusionsThe care of infants with HIE treated with TH in Spain is generally adequate, although there are areas for improvement in neuromonitoring, sedation, prognostic information, teamwork, and duration of follow-up.  相似文献

2019年为己亥之年,土运不足,厥阴风木司天,少阳相火在泉。下半年相火淫胜,火气流行,克金、刑水。风木盛则克制土运,土运本不足,致虚者更虚,强者更强,此谓之不相得,不相得则病;且气克运,古称之为天刑,天刑之年多灾害。2019年,冬季主气太阳寒水之上少阳相火客临,致冬不藏反长,天不寒反暖。地气封藏不固,水中阳气不藏,本应该被抑制或杀灭的微生物却因火热而未灭,并得少阳火毒助长而毒性更猛,化生为火疫。2020年为庚子年,主行金运,金克木,可平复2019年偏盛之木运,故瘟毒会被2020年之金运削弱或肃杀。针对此次瘟疫,未病之人预防要点为:虚邪贼风,避之有时;精神内守,病安从来;正气存内,邪不可干;饮食宜忌,尤为重要;避秽化浊,芳香化湿。已病之人的治疗,依据此次瘟疫"少阳火毒夹湿"病机,以咸补之,以甘泻之,以咸耎之,岁谷宜丹,间谷宜豆。此次瘟毒的性质虽为湿热火毒,但患者体质却有寒热虚实之别,感染后症状、病情和预后亦不同,故中医临证时应遵循辨证论治,三因治宜的原则;若体质素有湿热,邪从本气而化,可遵循运气病机施治;若体质本热,湿去而化为火毒,则应以制火为主;若体质本寒,热去而化寒湿,则应温化寒湿,芳香化浊;若体质素虚,出现危重证候,则应在祛邪时,重视固护元气。  相似文献

ObjectivesWe analysed the relationship between serum levels of lactate within 1 h of return of spontaneous circulation (ROSC) and survival and neurological outcomes in patients who underwent therapeutic hypothermia (TH).MethodsThis was a multi-centre retrospective and observational study that examined data from the first Korean Hypothermia Network (KORHN) registry from 2007 to 2012. The inclusion criteria were out-of-hospital cardiac arrest (OHCA) and examination of serum levels of lactate within 1 h after ROSC, taken from KORHN registry data. The primary endpoint was survival outcome at hospital discharge, and the secondary endpoint was poor neurological outcome (Cerebral Performance Category, CPC, 3–5) at hospital discharge. Initial lactate levels and other variables collected within 1 h of ROSC were analysed via multivariable logistic regression.ResultsData from 930 cardiac arrest patients who underwent TH were collected from the KORHN registry. In a total of 443 patients, serum levels of lactate were examined within 1 h of ROSC. In-hospital mortality was 289/443 (65.24%), and 347/443 (78.33%) of the patients had CPCs of 3–5 upon hospital discharge. The odds ratios of lactate levels for CPC and in-hospital mortality were 1.072 (95% confidence interval (CI) 1.026–1.121) and 1.087 (95% CI = 1.031–1.147), respectively, based on multivariate ordinal logistic regression analyses.ConclusionHigh levels of lactate in serum measured within 1 h of ROSC are associated with hospital mortality and high CPC scores in cardiac arrest patients treated with TH.  相似文献

AimIt is unknown whether older patients with out of hospital cardiac arrest (OHCA) have worse outcomes because of aging itself, or because age can be a marker for overall health status. We aimed to study the prognostic utility of age and pre-arrest comorbidities.MethodsWe conducted a retrospective cohort study, reviewing electronic health records of all adults treated for non-traumatic OHCA in the University of Michigan Emergency Department (N = 588). Primary covariates included age, Charlson Comorbidity Index (CCI), and a combined Charlson-age index. The primary dichotomized outcome was favorable neurological outcome (cerebral performance category, 1–2), evaluated by logistic regressions.ResultsDementia (p = 0.01), witnessed arrest (p = 0.03), bystander CPR (p < 0.001), presenting rhythm (p < 0.001), and mild therapeutic hypothermia (p < 0.001) were associated with the primary outcome. Increasing age (unadjusted OR for each decade of life, 95% CI: 0.78, 0.70–0.88; adjusted 0.79, 0.67–0.94) was negatively associated with likelihood of a favorable neurological outcome. CCI and combined Charlson-age index significantly predicted outcome in the unadjusted, but not adjusted analysis. Composite variables were stronger predictors in patients with shockable than non-shockable presenting rhythms (interaction terms: age and rhythm [p = 0.004], CCI and rhythm [p = 0.01]).ConclusionAge, but not CCI, was significantly associated with less favorable neurological outcomes in patients with OHCA after adjusting important covariates. Age appears to be an independent predictor of prognosis rather than a marker for comorbidity.  相似文献

Therapeutic hypothermia is often used for traumatic brain injury because of its neuroprotective effect and decreased secondary brain injury. However, this procedure lacks clinical evidence supporting its efficacy, and adverse outcomes have been reported during general anesthesia. A 61-year-old man with a history of percutaneous coronary intervention (PCI) was admitted with traumatic brain injury. Immediately after admission, he underwent mild therapeutic hypothermia with a target temperature of 33.0°C for neuroprotection. During general anesthesia for emergency surgery because he developed a mass effect, hypothermic cardiac arrest occurred following an additional decrease in the core body temperature. Moreover, myocardial infarction caused by restenosis of the previous PCI lesion also contributed to the cardiac arrest. Although the patient recovered spontaneous circulation after an hour-long cardiopulmonary resuscitation with rewarming, he eventually died of subsequent repetitive cardiac arrests. When anesthetizing patients undergoing therapeutic hypothermia, caution is required to prevent adverse outcomes that can be caused by unintentional severe hypothermia and exacerbation of underlying heart disease.  相似文献

目的:探究浅低温对心肌缺血-再灌注损伤合并脓毒症大鼠模型的心肌保护作用。方法:共15只SD大鼠随机分为心肌缺血-再灌注损伤合并脓毒症浅低温(MHT)组、心肌缺血-再灌注损伤合并脓毒症常温(NT)组和假手术常温组作为对照组。大鼠心肌缺血后再灌注同时给予内毒素,然后MHT组将大鼠置于冰床降温,15 min内达(33.0±0.5)℃,随后维持2 h。NT组及对照组实验全程维持在(37.0±0.5)℃。实验全程观察和记录血流动力学指标。浅低温或常温维持2 h后用酶联免疫吸附测定法检测血乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB),观察心肌梗死面积、病理学变化,免疫印迹法检测心肌烟酰胺腺嘌呤二核苷酸磷酸氧化酶2(NOX2),免疫组化检测心肌胱天蛋白酶-3(caspase-3)和肿瘤坏死因子-α(TNF-α)。结果:MHT组再灌注2 h心率较NT组降低[(396.63±39.93)次/min vs(.457.74±32.77)次/min,P<0.05];左心室最大收缩压升高[(129.14±34.05)mmHg vs.(85.63±14.74)mmHg,1 mmHg=0.133 kPa,P<0.05]。MHT组收缩期压力最大变化速率(+max dp/dt)、左心室舒张末期压力(LVEDp)和舒张期压力最小变化速率(-min dp/dt)再灌注2 h较NT组绝对值也有所增加,且均高于对照组,但差异无统计学意义。MHT组的LDH和CK-MB均较NT组显著降低[(1153.61±127.85)U/L vs(.2213.34±599.60)U/L,P<0.05;(1103.22±114.75)U/L vs(.1731.33±688.05)U/L,P<0.05]。心肌病理变化提示MHT组心肌坏死呈点状或灶状,心肌间质淤血程度轻且炎细胞浸润较少。NT组心肌出现水肿、灶状或片状坏死,心肌间质区淤血程度重及存在大量炎细胞浸润。此外,MHT组的心肌梗死面积比率明显小于NT组[(11.23±2.82)%vs.(19.25±4.45)%,P<0.05]。免疫印迹法显示,NOX2在MHT组中相对表达量较NT组减少,但差异无统计学意义(P>0.05)。MHT组的caspase-3相对表达量及TNF-α相对表达量均较NT组显著降低[(1.06±0.34)%vs.(3.27±0.67)%;(12.23±3.31)%vs(.31.14±1.69)%],差异均有统计学意义(P均<0.05)。结论:浅低温对心肌缺血-再灌注损伤合并脓毒症有心脏保护作用,可改善心功能,减少心肌梗死面积,减轻心肌损伤、炎症、氧化应激及凋亡。  相似文献

Neonatal encephalopathy (NE) is a significant complication of the peripartum period. It can lead to lifelong neurologic disabilities, including cerebral palsy, cognitive impairments, developmental delays, and epilepsy. Induced hypothermia is the first therapy, which has shown promise in improving the outcomes for neonates with moderate to severe NE following a presumed intrapartum insult.NE is also a frequent source of medical malpractice litigation. In this paper, we will review salient features of the American Tort System as it pertains to medical malpractice. We will discuss the obstetric medico-legal implications of therapeutic hypothermia and suggest a five-step approach to analyzing neonatal cases for causation, etiology, timing of occurrence, responsibility, and liability.
