“母乳最好”的宣传话语在目前育龄人群中几乎耳熟能详,但很多信奉科学育儿、希望母乳喂养的都市女性却发现母乳喂养困难重重,其中遭遇“母乳不足”的案例较为多见。回顾历史发现,“母乳不足”的现象全球普遍存在,这个话题在20世纪80年代的英语学术圈曾引起热议。经过十余年讨论后,学者们基本认同大部分遭遇母乳不足的女性并不是真正的“母乳不足”,而是由于种种社会文化因素干扰造成母乳喂养失败的这一结论,其中产后初期的奶瓶介入是重要原因。术语也由insufficient milk syndrome变为perceived insufficient milk,以此来强调“母乳不足”不是一种病理现象,而更多是一种文化感知。  相似文献

Aims: This paper focuses on the reasons for the under-representation of British South Asians in substance use services. Based on a small-scale evaluation of a substance use service that delivers targeted outreach support within two predominantly Pakistani and Bangladeshi communities in the north west of England, this paper contributes to the debate around how substance use services can best engage with young British Pakistani and Bangladeshi substance users.

Methods: Semi-structured interviews (with six staff members, 18 young Pakistani and Bangladeshi service users, and 18 stakeholders and partner agencies), a detailed ethnographic observation of the service, and an analysis of routinely collected quantitative monitoring data.

Findings: The paper highlights the importance of what Fountain terms low threshold/open access services. Alongside this, the paper argues that the building of trust and confidence in a substance use service is a key when it comes to engaging with young Pakistani and Bangladeshi substance users. Yet this necessary process takes time: something that is at odds with the current trend towards short-term funding regimes and ‘quick wins’.

Conclusions: The paper concludes by advocating the need for, not only a diverse range of engagement strategies, but also a longer term approach when it comes to developing and delivering substance use services aimed at successfully engaging with this particular group of substance users.  相似文献

Abstract

In light of a growing body of evidence demonstrating pervasive health disparities, medical schools are increasingly incorporating educational programs on social medicine in undergraduate and graduate medical curricula. In 2015, we significantly restructured the cultural competency instruction for medical students at our institution, focusing on achieving greater health equity through caring for vulnerable populations and acknowledging and addressing bias and stereotyping. In order to facilitate educational sustainability while students were immersed in clinical care, a key element of our approach included extending teaching into the clerkship year. The resulting longitudinal thread, Health Equity and Social Justice, empowers future physicians with the knowledge and skills to work towards greater health equity. This article discusses the lessons learned in the implementation of this novel educational program. Our approach can serve as a model for other institutions considering similar instructional reform.  相似文献

The papers in this issue have specific and focused targets that are essential for making incremental progress on the very difficult problem of identifying the coevolutionary interactions of cognition and culture. The purpose of this paper is to discern the shape of a few of the large problems that loom over these more narrowly focussed papers, and to explain and assess the ways these papers contribute to their solution. The background problems described are (a) the character of the selective interactions between the evolution of culture and of cognition; (b) the special features of cumulative cultural evolution; and (c) the place of language in an account of cognition–culture coevolution. The paper ends with some reflections on the extraordinarily difficult challenge of testing scenarios in this field.  相似文献

The aim of the study was to explore the relationship between college graduates’ dual self-consciousness, job search clarity and perceived stress, and reveal the mediating role of perceived stress between dual self-consciousness and job search clarity. In this study, 467 college graduates were investigated using the Dual Self-Consciousness Scale, Job Search Clarity Scale, and Perceived Stress Scale. After controlling for gender, age, and region, the results revealed that: (1) private self-consciousness has a significant positive predictive effect on job search clarity; (2) perceived stress has a significant negative predictive effect on job search clarity; (3) perceived stress plays partial mediation effects between private self-consciousness and job search clarity; (4) perceived stress plays complete mediation effects between public self-consciousness and job search clarity; (5) perceived stress has suppressing effects between public self-consciousness and job search clarity.  相似文献
