ObjectiveThe use of mechanical circulatory support (MCS) in lung transplantation has been steadily increasing over the prior decade, with evolving strategies for incorporating support in the preoperative, intraoperative, and postoperative settings. There is significant practice variability in the use of these techniques, however, and relatively limited data to help establish institutional protocols. The objective of the AATS Clinical Practice Standards Committee (CPSC) expert panel was to review the existing literature and establish recommendations about the use of MCS before, during, and after lung transplantation.MethodsThe AATS CPSC assembled an expert panel of 16 lung transplantation physicians who developed a consensus document of recommendations. The panel was broken into subgroups focused on preoperative, intraoperative, and postoperative support, and each subgroup performed a focused literature review. These subgroups formulated recommendation statements for each subtopic, which were evaluated by the entire group. The statements were then developed via discussion among the panel and refined until consensus was achieved on each statement.ResultsThe expert panel achieved consensus on 36 recommendations for how and when to use MCS in lung transplantation. These recommendations included the use of veno-venous extracorporeal membrane oxygenation (ECMO) as a bridging strategy in the preoperative setting, a preference for central veno-arterial ECMO over traditional cardiopulmonary bypass during the transplantation procedure, and the benefit of supporting selected patients with MCS postoperatively.ConclusionsAchieving optimal results in lung transplantation requires the use of a wide range of strategies. MCS provides an important mechanism for helping these critically ill patients through the peritransplantation period. Despite the complex nature of the decision making process in the treatment of these patients, the expert panel was able to achieve consensus on 36 recommendations. These recommendations should provide guidance for professionals involved in the care of end-stage lung disease patients considered for transplantation.  相似文献

Introduction

Left ventricular (LV) dysfunction is estimated to occur in 10%–25% of the general intensive care unit (ICU) population and is frequently seen as regional wall motion abnormalities (RWMAs). Although RWMA is mostly attributed to myocardial ischemia or infarction, some studies have suggested that nonischemic RWMA might also be prevalent. We sought to establish that RWMA can be seen in critically ill patients with normal coronary arteries and to explore reasons for RWMA in this population.

Methods

In this retrospective study, data from the hospital angiography register and the ICU register were collated between 2012 and 2019. Patients were identified who underwent angiography in conjunction with their ICU stay and had RWMA on echocardiography. Patients were divided into either those with non-obstructed or those with obstructed coronary arteries. Cardiac magnetic resonance imaging (cMRI) examinations were reviewed if they had been performed on patients with non-obstructed coronaries.

Results

We identified 53 patients with RWMA and non-obstructed coronary arteries and 204 patients with RWMA and obstructed coronary arteries. Patients with non-obstructed coronary arteries were more often female, younger, and had fewer cardiovascular risk factors. They less commonly had ST elevation, but more frequently had T-wave inversion or serious arrhythmias. Troponin levels were higher in patients with obstructed coronary arteries, but NT-proBNP was similar between the groups. There were no differences in risk-adjusted 90-day mortality between patients with non-obstructed versus obstructed coronary arteries (OR 1.21, [95% CI 0.56–2.64], p = .628). In those with non-obstructed coronary arteries, follow-up echocardiography was available for 38 patients, of whom 30 showed normalization of cardiac function. Of the 14 patients with non-obstructed coronary arteries on whom cMRI was performed, 7 had a tentative diagnosis of Takotsubo syndrome or myocardial stunning; 4 had a myocardial infarction (preexisting in 3 cases); 1 patient had acute myocarditis; 1 patient had post-myocarditis; and 1 patient was diagnosed with dilated cardiomyopathy.

Conclusion

RWMA can be seen to occur in critically ill patients in the absence of coronary artery obstruction. Several conditions can cause regional hypokinesia, and cMRI is useful to evaluate the underlying etiology.  相似文献

Background and aimsSkin autofluorescence (SAF) can non-invasively assess the accumulation of tissue AGEs. We investigated the association between SAF and kidney dysfunction in participants with T2D.MethodsOf 4030 participants consecutively measured SAF at baseline, 3725 participants free of end-stage kidney disease (ESKD) were included in the analyses. The association of SAF with incident ESKD or ≥30% reduction in estimated glomerular filtration rate (eGFR) was examined with Cox regression, linear mixed-effects model for the association with annual eGFR decline, and mediation analyses for the mediating roles of renal markers.ResultsDuring a median (IQR) 1.8 (1.1–3.1) years of follow-up, 411 participants developed the outcome. SAF was associated with progression of kidney disease (hazard ratio 1.15 per SD, 95% confidence interval [CI] [1.04, 1.28]) and annual decline in eGFR (β ?0.39 per SD, 95% CI [?0.71, ?0.07]) after adjustment for risk factors, including baseline eGFR and urinary albumin-creatinine ratio (UACR). Decreased eGFR (12.9%) and increased UACR (25.8%) accounted for 38.7% of the effect of SAF on renal outcome.ConclusionsSAF is independently associated with progression of kidney disease. More than half of its effect is independent of renal markers. SAF is of potential to be a prognostic marker for kidney dysfunction.  相似文献

【目的】观察荷芪散治疗痰瘀互结证2型糖尿病(T2DM)合并冠心病(CHD)的临床疗效及其对血清氧化三甲胺(TMAO)水平的影响。【方法】将40例痰瘀互结证T2DM合并CHD患者随机分为观察组和对照组,每组各20例。2组患者均给予降压、调脂及抗血小板聚集等西医基础治疗,在此基础上,对照组给予利拉鲁肽治疗,观察组在对照组的基础上加用荷芪散治疗,疗程为1个月。观察2组患者治疗前后中医证候积分、糖代谢指标、脂代谢指标、左心室功能相关指标、炎症因子、内皮素1(ET-1)以及肠道菌群代谢产物TMAO水平的变化情况,并评价2组患者的临床疗效和安全性。【结果】(1)疗效方面:治疗1个月后,观察组的总有效率为95.0%(19/20),对照组为70.0%(14/20),组间比较,观察组的临床疗效明显优于对照组(P<0.05)。(2)中医证候积分方面:治疗后,2组患者的中医证候积分均较治疗前明显下降(P<0.01),且观察组对中医证候积分的下降作用明显优于对照组(P<0.01)。(3)糖代谢相关指标方面:治疗后,2组患者的空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)均较治疗前明显下降(P<0.01),且观察组对FPG、2hPG、HbAlc、FINS、HOMA-IR的下降作用均明显优于对照组(P<0.05或P<0.01)。(4)脂代谢相关指标方面:治疗后,2组患者总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)均较治疗前明显下降(P<0.01),且观察组对TC、TG和LDL-C的下降作用均明显优于对照组(P<0.05)。(5)炎性因子、血管内皮因子以及肠道菌群代谢产物方面:治疗后,2组患者血清白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)、ET-1和TMAO水平均较治疗前明显下降(P<0.01),且观察组对血清IL-6、hs-CRP、ET-1和TMAO水平的下降作用均明显优于对照组(P<0.05)。(6)超声心动图方面:治疗后,2组患者的左室射血分数(LVEF)、二尖瓣口血流频谱E峰与A峰流速比值(E/A)均较治疗前升高(P<0.01),且观察组对LVEF、E/A的升高作用均明显优于对照组(P<0.05)。(7)治疗过程中,2组患者均未出现相关并发症及药物不良反应。【结论】荷芪散联合利拉鲁肽治疗痰瘀互结证T2DM合并CHD患者,可以提高临床疗效,降低中医证候积分,改善糖脂代谢和胰岛素抵抗,提高左心室功能,降低炎症因子以及ET-1的表达,并降低血清TMAO水平,其疗效优于单用利拉鲁肽治疗。  相似文献

目的 探讨腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)中不同气腹压对急性结石性胆囊炎(acute calculous cholecystitis,ACC)患者的影响。 方法 选取我院收治的ACC患者60例为研究对象,采用随机数字表法分为观察组和对照组,各30例。2组均行LC治疗,对照组术中维持常规气腹压(12~15 mmHg),观察组术中维持低气腹压(7~8 mmHg)。比较2组围术期指标、术后疼痛程度、并发症发生率,术前及术后24 h对两组肝功能及内皮细胞损伤相关因子进行检测对比。 结果 观察组首次排气时间、首次进食时间短于对照组(P<0.05);观察组术后1~24 h疼痛程度均低于对照组(P<0.05);术后24 h 2组丙氨酸转氨酶、天冬氨酸转氨酶、总胆红素、γ-谷氨酰转移酶、血管性假血友病因子、血栓调节蛋白、内皮素1均升高,但观察组低于对照组(P<0.05);观察组并发症发生率与对照组比较,差异无统计学意义(P>0.05)。 结论 LC术中维持低气腹压在术野清晰度、疗效、安全性方面与正常气腹压效果相当,且有助于促进患者术后胃肠功能恢复,减轻术后疼痛程度,对保护肝功能及抑制内皮细胞损伤具有积极作用。  相似文献

目的: 探究普拉克索能否通过抑制心肌细胞MPTP开放减轻脓毒症大鼠心功能障碍。方法: 将SD大鼠分为假手术组、盲肠结扎穿孔术组(CLP组)、盲肠结扎穿孔术+环孢菌素A组(CsA组)、盲肠结扎穿孔术+普拉克索组(PPX组)。CLP组、CsA组和PPX组统一使用盲肠结扎穿孔术建立脓毒症大鼠模型,其中CsA组静脉注射CsA(2 mg·kg-1),PPX组予以普拉克索(1 mg·kg-1)灌胃。观察SD大鼠一般情况、存活率及腹腔解剖情况,超声心动图检测心功能,ELISA法测定血清PCT、NT-proBNP、cTnI水平,通过线粒体肿胀测定MPTP的开放程度,HE染色及电镜观察各组心脏组织病理改变。结果: 术后36 h,假手术组大鼠一般情况、腹腔解剖及心脏病理改变正常,CLP组明显变差,CsA组及PPX组病变程度较CLP组减轻;各组大鼠的存活率:假手术组(100%)>CsA组(75.0%)>PPX组(62.5%)>CLP组(43.8%);CsA组、PPX组心脏左室射血分数均低于假手术组(P<0.05),均较CLP组升高(P<0.05);CLP组、CsA组、PPX组血清PCT、NT-proBNP、cTnI水平均高于假手术组(P<0.05),CsA组、PPX组低于CLP组(P<0.05);CLP组、CsA组、PPX组心肌细胞MPTP的开放程度大于假手术组(P<0.05),CsA组、PPX组小于CLP组(P<0.05)。结论: 心肌细胞MPTP的异常开放是引起大鼠脓毒症心肌损伤的机制之一,普拉克索可以通过抑制心肌细胞MPTP的异常开放减轻脓毒症大鼠的心功能损伤。  相似文献

目的 探究急性心肌梗死(AMI)患者血清lncRNA p21表达水平及临床意义。方法 选取河南科技大学第一附属医院心外科2019年1月至2021年1月收治的92例AMI患者(AMI组),根据冠脉病变程度分为轻度病变组(n=29)、中度病变组(n=43)和重度病变组(n=20);同期选择本院体检健康者92例为对照组。AMI患者在经皮冠状动脉植入术(PCI)术后随访24个月,将患者分为心血管不良事件(MACE)组(n=22)和非MACE组(n=70)。采用实时荧光定量聚合酶链式反应检测研究对象血清中lncRNA p21表达水平。采用Pearson法分析AMI患者血清lncRNA p21水平与肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)水平的相关性。采用受试者工作特征(ROC)曲线评价血清lncRNA p21水平预测AMI的价值。结果 与对照组相比,AMI组血清lncRNA p21相对表达水平较低(P<0.05),CK-MB及cTnI水平较高(P<0.05)。与轻度病变组相比,中度病变组、重度病变组血清lncRNA p21相对表达水平较低(P<0.05),CK-MB、cTnI水平较高(P<0.05);与中度病变组相比,重度病变组血清lncRNA p21相对表达水平较低(P<0.05),CK-MB、cTnI水平较高(P<0.05)。AMI患者血清lncRNA p21水平与cTnI、CK-MB水平均呈负相关(P<0.05)。ROC曲线显示,lncRNA p21水平预测AMI的曲线下面积(AUC)为0.861,其灵敏度、特异度分别为70.70%、89.10%;lncRNA p21联合cTnI、CK-MB预测AMI的AUC为0.956,其灵敏度、特异度分别为89.10%、94.60%。与非MACE组相比,MACE组治疗前血清lncRNA p21相对表达水平较低(P<0.05)。结论 AMI患者血清lncRNA p21表达水平下降,lncRNA p21对AMI具有一定的诊断价值。  相似文献

目的探讨脂蛋白a[Lp(a)]联合单核细胞/高密度脂蛋白胆固醇(HDL-C)比值(MHR)在非ST段抬高型急性冠脉综合征(NSTE-ACS)患者冠状动脉(冠脉)病变严重程度及预后中的评估及其价值。方法选择2020年3月至2021年3月安徽医科大学第一附属医院符合纳入标准的NSTE-ACS患者(n=310),依据冠脉造影结果进行Gensini评分,以Gensini积分的第3四分位数(QU=38)为切点,分为高危组(QU≥38,n=74)和非高危组(QU<38,n=236),比较其一般资料,并使用Logistic回归分析Lp(a)、MHR与冠脉病变严重程度的相关性;使用ROC曲线评估Lp(a)联合MHR评估冠脉病变严重程度及预后的价值。结果高危组和非高危组患者性别、吸烟史、高血压病、糖尿病、高胆固醇血症、肾功能不全等病史、他汀药物应用发生率差异无统计学意义(P>0.05),但高危组患者年龄、血Lp(a)水平和MHR均显著高于非高危组(P<0.01)。Spearman相关分析发现,Lp(a)和MHR分别与Gensini评分呈正相关(r=0.316,P<0.05;r=0.715,P<0.05)。Logistic多因素二元回归分析示,Lp(a)、年龄和MHR是冠脉高危病变的独立危险因素(P<0.01,P<0.05)。ROC曲线分析示,Lp(a)和MHR联合检测对冠脉高危病变评估的AUC(0.854),分别大于Lp(a)和MHR单独检测的AUC(0.852、0.754);联合评估的敏感度73.0%,高于单独检测Lp(a)的55.6%,但低于MHR的82.4%;联合评估的特异度85.6%,高于Lp(a)及MHR单独检测(84.1%、61.9%)。ROC曲线分析示,Lp(a)和MHR联合对NSTE-ACS患者不良预后评估的AUC(0.800),分别大于Lp(a)和MHR单独检测的AUC(0.781、0.739);联合评估患者预后的敏感度为72.2%,高于Lp(a)及MHR单独检测(66.7%、66.7%);联合评估的特异度为81.9%,高于MHR单独检测的80.9%,但低于Lp(a)单独检测的84.8%。结论异常升高的血Lp(a)水平联合MHR对NSTE-ACS患者冠脉病变的严重程度和不良预后具有较高的判断价值。  相似文献
