从文理、医理以及文化内涵多角度剖析《灵枢·决气》所载"六气为一气"的思想内涵。精、气、津、液、血、脉"六气"为人体之基础,然同源于"一气",其功能体现了阴阳互根互用。"六气为一气"观的文化内涵包括三个方面:一是在中国古代地理、社会、政治、哲学文化大环境下,其思维方式以"尚同""求一"为根本;二是万物为"一气"的哲学思维,即"气一元论";三是"气一元论"的延伸即"整体思维",正是中国古代文化"同源"思想的体现。  相似文献

模型化推理是指以模型特别是思维模型为中介或工具,由一个或几个前提推出结论的思维方法。《黄帝内经》由于其历史条件的限定,较多地采用了天然实物模型进行推理,而水与人类生活密切相关,自然也就成了人类认识事物乃至人体生命活动的最为普遍的天然模型。通过阐述水模型与《黄帝内经》血脉气血循环、经脉气血运行、十二经脉体系建构、六淫病因认识、气血津液病机以及相关临床诊治思维的关系,分析指出《黄帝内经》天然模型化推理具有直观性、经验性、功能性、非结构性、整体全息性、时序性等特征。  相似文献

The current WHO Recommendations to assure the quality, safety and efficacy of live attenuated yellow fever vaccines were adopted in 2010. This document recommends that vaccine virus master and working seed lots should be tested for viscerotropism, immunogenicity and neurovirulence in monkeys. A vaccine manufacturer has reported, recently, discrepancies on the clinical scoring of monkeys during assessment of working seed lots and suggested aligning neurotropism assessment of yellow fever vaccines virus seed lots with that of neurovirulence testing of polio vaccines virus seed lots. In this approach, clinical signs are recorded but do not form part of the assessment or pass/fail criteria. At its 71st meeting in August 2020, the ECBS agreed to establish a drafting group and to consult with manufacturers and other stakeholders on the proposed amendment. Then a survey had been conducted to seek opinions of stakeholders on the neurotropism testing and revision of current WHO Recommendations for yellow fever vaccines. It was recognized from the answers of the survey that the test for neurovirulence in monkeys presents several technical challenges which could be addressed in the amended version of the Recommendations.On 18–19 March 2021, a virtual WHO working group meeting was held to discuss a proposed draft of the amended text with participants of yellow fever vaccine manufacturers and relevant regulators. Overall, there was a consensus among manufacturers and regulators that clinical evaluation provides important information and should be retained as part of the neurotropism test. However, there was also agreement that the test is somewhat subjective, and that analysis can be difficult. It was recognized that there was potential for improvement in both test execution and analysis to increase harmonization between manufacturers. Alternative tests to the non-human primates neurovirulence test would be useful but it was agreed that none seem to be sufficiently developed at present. Based on these working group discussions, it was proposed that the appendix on neurotropism test to be further revised by the WHO drafting group and submitted to ECBS for review and adoption.Issues other than neurotropism test were discussed in the meeting as well. There were a number of points identified during the meeting, such as new platform of production, animal models, deep sequencing, international standards, that are outside the current recommendations that are worthy of further discussion. Therefore, it is recommended that there would be a future meeting with various stakeholders to discuss the potential revision of the whole Recommendations for yellow fever vaccines in order to meet the current needs.  相似文献

The liver is the major drug-metabolizing and drug-detoxifying organ. Many drugs can cause liver damage through various mechanisms; however, the liver response to injury includes a relatively narrow spectrum of alterations that, regardless of the cause, are represented by phlogosis, oxidative stress and necrosis. The combination of these alterations mainly results in three radiological findings: vascular alterations, structural changes and metabolic function reduction. Chemotherapy has changed in recent decades in terms of the drugs, protocols and duration, allowing patients a longer life expectancy. As a consequence, we are currently observing an increase in chemotherapy-associated liver injury patterns once considered unusual. Recognizing this form of damage in an early stage is crucial for reconsidering the therapy regimen and thus avoiding severe complications. In this frontier article, we analyze the role of imaging in detecting some of these pathological patterns, such as pseudocirrhosis, “yellow liver” due to chemotherapy-associated steatosis-steatohepatitis, and “blue liver”, including sinusoidal obstruction syndrome, veno-occlusive disease and peliosis.  相似文献

A port‐wine stain (PWS) is a congenital capillary malformation that is seen in 0.3–0.5% of newborns. Although many types of lasers have been used to treat PWSs, few studies have investigated the efficacy of 577‐nm Pro‐Yellow lasers. The aim of this study was to establish the efficacy of Pro‐Yellow lasers in PWS treatment. In total, 26 patients, comprising 15 females (57.7%) and 11 males (42.3%) with a mean age of 24.7 ± 11.8 (range 1–50) years were included in the study. All were treated with a 577‐nm Pro‐Yellow laser at 4‐week intervals, 3–10 times. The sessions initially used scan mode and a fluence dose of 24 J/cm2 per session, on average. The mean dose was increased by 4 J/cm each session and the maximum dose administered was 44 J/cm2. A mean regression of 68.8 ± 13.9% in the size of the lesions over 5.23 ± 2.7 sessions was observed. There was no statistically significant difference between the female and male patients when they were compared in terms of the number of treatment sessions (p = .789) and treatment success (p = .39). These case series demonstrate that the Pro‐Yellow laser is a safe and well‐tolerated treatment for PWSs. However, deep‐seated lesions required more treatment sessions and it was observed that clinically the lesions did not completely disappear.  相似文献

根据《内经》遗篇《刺法论》《本病论》提出三虚兼“邪鬼”(致病原)致疫说,认为疫疠产生的根本原因是人气不足,天气如虚,人神失守,“邪鬼”(致病原)干人,其对现代疫病防治的意义在于对当地天地之气交特点、个体的体质特点和病情特点的把握。对于突发的新型传染病等流行性疾病,当病因或病原体及其致病特点尚未被现代医学认识清楚之前,以中医学的辨天地之气交、六气六经辨证、温病学疫气理论指导疾病的预防和治疗具有重要的意义,为构建中医药疫病防治体系,重构疫病学科提供借鉴。   相似文献

目的探讨黄酒多酚(YWPC)对同型半胱氨酸(Hcy)诱导的大鼠血管平滑肌细胞(VSMCs)基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)表达和活性的影响及其信号通路。方法SD大鼠主动脉VSMCs原代细胞培养鉴定,取4~7代细胞用于实验。YWPC组VSMCs分别用1、10、100mg/L的YWPC干预,采用Westernblot法检测VSMCs中MMP-2、MMP-9、AKT、pAKT的表达情况;明胶酶谱检测VSMCs中MMP-2和MMP-9的活性。用IGF-1(3ng/ml)干预VSMCs8h激活PI3K/AKT通路后,用YWPC干预VSMCs,检测MMP-2、MMP-9的表达和活性。结果Westernblot结果显示,与Hcy组比较,YWPC组VSMCs中MMP-2/9的表达减少,pAKT表达减少(均P<0.01);明胶酶谱结果显示,与Hcy组比较,YWPC组VSMCs中MMP-2/9的活性降低。在加入IGF-1后,与对照组比较,IGF-1组VSMCs中pAKT、MMP-2/9的表达和活性增加(P<0.01);与YWPC组比较,YWPC+IGF-1组VSMCs中pAKT、MMP-2/9的表达和活性增加(P<0.01)。结论YWPC通过抑制PI3K/AKT信号通路抑制Hcy诱导的VSMCs中MMP-2/9的表达和活性。  相似文献

GoalsStarting from the example of the exostosis of the external auditory canal, a modification due to the impact of the sea on the body, this article considers how this pathology is experienced and interpreted by those who contract it. After defining “surfer's ear” from an anthropo-medical point of view, we will address the meaning that individuals give to it, such as the belief of becoming fish.MethodologyOver the course of several years of field observation and interviews, mainly on the beaches of the Atlantic Pyrenees, we were able to compare different individuals’ discourses about how the surfing body, immersed in nature, is perceived and experienced.ResultsThis helps to understand thoughts that can link people to the ocean, and more widely to nature, in our contemporary Western society. Thus, penetrating and being penetrated by this living water implies a sensual relationship with the environment, in a search both for erotic pleasure and an enveloping maternal security, lost from birth.DiscussionThis relationship to the environment implements a sensory ecology where the human becomes nature, and nature takes human form. This is an occasion for the reinvention of links, not in an opposition between human and non-human, but by reminding humans that they can find balance in abandoning themselves to nature.ConclusionPathologies of the ear define an imaginary clinic of the body. The body is thus rendered unfit by the physical sensations of marine pathology but, above all, by the belief in a psychic envelopment in water.  相似文献

[目的]探究《内经》脉时合参思想和经验,以期更全面地学习中医学脉学理论,提高临床疗效。[方法]撷选《内经》相关原文,对脉诊理论中的时间因素做简要分析,探讨昼夜、月相、季节等时间因素对脉象的影响,总结《内经》脉时合参思辨方法。[结果]昼夜、月相、季节等时间因素与脉象关系密切,自然界的阴阳消长等具有明显的时间性,人体脉象与其相适应。脉时合参不仅可以知晓脉象随昼夜、月相、季节等的生理变化,还可根据脉、脏、时的关联准确判定病位,通过脉逆四时与否细测病情轻重,依据脉象随病情发展的阶段性规律预判病势预后,以及因时制宜指导临床治疗。[结论]《内经》脉时合参思想,内容丰富,在临床中运用广泛,蕴涵着天人一体的整体观和中医学哲学思辨方法,值得深入挖掘和学习。  相似文献
