33例肺上沟癌的临床特点及放疗疗效

目的　探讨肺上沟癌的临床特点及放疗疗效和不良反应。方法　回顾性分析 3 3例肺上沟癌住院病人的临床特征和常规放疗的疗效。结果　肺上沟癌占原发性支气管肺癌的 2 9% ,常见症状为 :患侧肩、背和上肢疼痛 ( 78 8% ) ,后 1,2 ,3肋骨或椎骨破坏( 5 7 6% ) ,Horner’s综合征 ( 3 6 7% ) ;少见的症状为 :咳嗽 ( 2 7% ) ,咯血 ( 9% ) ;中位生存期为 8 4月 ;1,3 ,5年生存率分别为 3 5 6% ,12 3 % ,4 6% ;放疗反应可耐受。结论　肺上沟癌相当少见 ,其临床特征基本符合pancoast综合症 ,本病预后差 ,但放疗可缓解疼痛 ,提高生存质量。     

老年人急性阑尾炎的腹腔镜手术治疗

目的 探讨老年人急性阑尾炎的临床特点，更好地把握腹腔镜阑尾切除（LA）术的方法和特点。方法 回顾分析50例老年人急性阑尾炎LA的临床资料。结果 LA均获成功，患者全部治愈，无严重的并发症发生，肠功能恢复时间及住院时间均缩短。结论 老年人反应迟钝，阑尾炎进展快，易发生坏疽及穿孔，合并症多，手术风险较高，LA是安全、可行的，除个别腹膜后阑尾炎外，其余均可施行LA。     

30例急性单发性球形肺炎的临床与CT特点

目的　通过分析 3 0例急性单发性球形肺炎的临床与CT特点 ,提高与周围性肺癌的鉴别能力。方法　对 3 0例住院患者 ,入院后均行胸片、胸部CT、纤维支气管镜检查及痰涂片、痰细菌培养等检查。结果　痰细菌培养均为细菌感染 ,胸片、胸部CT显示病灶为贴近胸膜或沿支气管血管束分布占 70 .0 0 % ;分布在两肺下叶背段或后基底段占 60 .0 0 %。病灶直径 2 .0～ 4.5cm 2 4例 ,占 80 .0 0 % ,2 5例患者经抗炎治疗病灶完全吸收。结论　抗炎治疗是动态观察单发性球形肺炎病灶变化的一种有效方法。     

Differences in aerodynamic characteristics of new and dysfunctional Provox®2 voice prostheses in vivo

Tracheoesophageal voice prostheses need to be replaced due to increased airflow resistance or retrograde leakage of fluid into the trachea as a consequence of biofilm formation. Previous in vitro studies show a change of aerodynamic features of biofilm covered voice prostheses after removal of the prostheses out of the patient. To assess these changes in an in situ situation, aerodynamic characteristics were measured within 45 patients at the beginning and at the end of the wearing process of the Provox 2 voice prosthesis. As a consequence, the influence of biofilm formation on aerodynamic characteristics can be evaluated. In the majority of cases, leakage through the prosthesis was the reason for replacement. No differences were found in the total flow, volume range and intratracheal pressure (ITP) of the voice prostheses measured. The airflow resistance of biofilm covered prostheses was significantly reduced compared to new clean prostheses. However, no correlation was found between the extent of biofilm and the different aerodynamic features measured. Biofilm formation on the Provox 2 is responsible for both reduction in airflow resistance and leakage through the prosthesis by deterioration of the silicone rubber material.     

Bilateral malignant phyllodes tumours.

We report a rare example of bilateral primary malignant phyllodes tumours. The diagnosis was supported by the identification of a benign epithelial element in each lesion. The case illustrates the typical dimorphic features of malignant phyllodes tumours. A contralateral tumour should not be regarded as metastatic without histological confirmation.     

广东省儿童乙型肝炎疫苗纳入计划免疫管理10年后血清学效果分析

为研究广东省乙型肝炎 (乙肝 )疫苗纳入计划免疫管理 10年后儿童血清乙肝病毒感染标志物 (HBVM )的变化 ,采用分层多阶段随机抽样法 ,将全省分 4层 ,共抽取 12个县 2 4个村的 196 3名儿童为研究样本。采血用固相放射免疫法 (SPRIA)检测乙肝病毒表面抗原 (HBsAg)、乙肝病毒核心抗体 (抗 HBc)、乙肝病毒表面抗体 (抗 HBs)。2 0 0 2年调查 1～ 14岁儿童乙肝疫苗接种率为 78 81% ,全程接种率为 74 17%。其中 1～ 9岁儿童乙肝疫苗全程接种率为 81 73% ,10～ 14岁为 6 0 32 %。 1～ 14岁儿童HBsAg携带率、抗 HBc阳性率、HBV感染率分别从 1992年的 19 86 %、5 3 5 9%、71 4 1%降至 2 0 0 2年的 8 5 6 %、2 2 5 7%、31 33%。其中 1～ 9岁儿童的HBsAg携带率、抗 HBc阳性率、HBV感染率分别从 1992年的 17 88%、4 7 0 9%、6 7 4 4 %下降至 2 0 0 2年的 5 75 %、14 4 9%、2 1 81%。乙肝疫苗接种后 ,1～ 3岁儿童抗 HBs阳性率为 5 2 0 2 %～ 6 2 18%。由于实施了加强免疫 ,4～ 14岁儿童的抗 HBs维持在 5 9 32 %～ 6 5 80 %。广东省乙肝疫苗纳入计划免疫管理 10年后 ,全省 1～ 14岁儿童HBV感染率已大幅度下降 ,抗 HBs阳性率大幅度升高 ,取得了良好的经济和社会效益。     

Photodynamic therapy of tumours: value of quantum chemical procedures for characterization of new drugs. Prediction of the electronic structure of zinc(II) phthalocyanine with special emphasis on triplet state excitation energies

Zinc(II) phthalocyanine is the active component of the liposomal formulation CGP 55847 which showed a high activity in photodynamic therapy in a variety of animal tumours. The photophysical properties of zinc(II) phthalocyanine have been studied in detail and compared to those of Photofrin, the only sensitizing agent approved so far for phase III/IV clinical trials. Since the efficacy of photodynamic therapy intrinsically depends on the spectroscopic features of the sensitizer, quantum chemical methods have proven to be an efficient means for optimizing chemical structures. As will be shown, a simple modification of the time-honoured INDO model of Pople allows a prediction of the singlet and triplet state properties of molecules of the size of zinc(II) phthalocyanine with an rms error of ≤ 1000 cm⁻¹.     

Thick malignant melanomas (> 3 mm Breslow) with good clinical outcome: a histological study and survival analysis

Thick malignant melanomas in general tend to have a poor prognosis, but exceptions occur where there may be no further recurrence. The reasons for this difference in clinical behaviour are not fully understood. We have looked at thick malignant melanomas (greater than 3.0 mm) in the East of Scotland that have no evidence of metastasis after a minimum of 6 years follow-up and compared the clinical and histological features with a similar group, associated with histological evidence of metastasis and/or death. Both groups received similar treatment regimes. We have identified 41 patients with thick melanomas in the former group. When compared with the control group, factors found to be significantly different between the two groups were: the nature of the lower margin of the tumour; vascular invasion; and anatomical location.     

传染性非典型肺炎患者泪液和血液中特异性抗体检测的对比研究

目的：通过传染性非典型肺炎(SARS)患者泪液病毒特异性抗体的检测，寻找SARS病毒是否通过泪液或眼结膜传播的理论依据，了解泪液和血液中SARS病毒特异性抗体检测结果的一致性程度。方法：用酶联免疫吸附法(ELISA)检测18例不同病程的SARS患者泪波及血清SARS病毒特异性抗体。结果：18例SARS患者泪液中的特异性IgM类和IgG类抗体均为阴性，而同组患者有13例其血液中的特异性IgM类和IgG类抗体呈阳性。结论：SARS病毒可能不通过泪液或结膜传播。
