目的 探讨瞬时脉冲足底静脉泵预防全髋/膝关节置换术后深静脉血栓形成(DVT)的效果,为临床医护人员选择最优的机械预防方式提供参考依据。方法 选取2019年11月至2020年8月在我院关节外科拟行全髋/膝关节置换术的病人210例,随机分为观察组(105例)和对照组(105例);术后,观察组和对照组分别使用瞬时脉冲足底静脉泵和间歇性充气加压装置预防DVT。观察比较两组病人术后DVT的发生率、术后D-二聚体、C-反应蛋白、白细胞介素-6和患肢肿胀情况,住院时间和使用机械设备预防DVT的舒适度。结果 术后2周,观察组DVT总发生率为6.67%,对照组为9.52%,两组比较,差异无统计学意义(P>0.05);观察组病人机械设备使用的舒适度高于对照组,差异有统计学意义(P<0.05)。结论 瞬时脉冲足底静脉泵可有效预防全髋/膝关节置换术后DVT,且舒适度高于间歇充气加压装置,预防DVT的时间成本低,值得临床推广。  相似文献

目的 分析体感交互技术训练对帕金森病患者平衡功能、足底压力及跌倒发生率的影响。方法 选取2020年2月-2022年4月三二〇一医院收治的115例帕金森患者,按不同训练方案分为常规组(n=56)与体感交互组(n=59)。常规药物治疗期间,常规组实施传统平衡训练,体感交互组接受体感交互技术训练。比较两组训练4周后Berg平衡量表(Berg Balance Scale,BBS)评分,动、静态足底压力分布,跌倒发生率。结果 训练后体感交互组BBS评分高于常规组(P<0.05);训练前后比较,常规组BBS评分无统计学意义(P>0.05),体感交互组BBS评分有所提升,具有统计学意义(P<0.05)。训练前两组静态、动态足底压力分布无统计学意义(P>0.05),两组静态站立时双脚压力由低到高排序为:2区<1区<3区<6区<5区<4区<7区<8区;行走时左足:2区<6区<1区<3区<5区<4区<7区<8区,右足:2区<6区<1区<3区<5区<7区<8区<4区。训练后两组站立时双足的3区压力上升,双足压力:2区<1区<6区<5区<3区<4区<7区<8区;行走时左足:2区<6区<1区<5区<3区<4区<7区<8区,右足压力分区未变。训练后两组静态跌倒风险指数评分上升,动态跌倒风险指数评分下降,且体感交互组评分优于常规组(P<0.05)。1个月随访期间,常规组发生11例跌倒,体感交互组发生2例,体感交互组跌倒发生率3.39%低于常规组19.64%(P<0.05)。结论 体感交互技术训练能有效改善帕金森患者平衡功能康复情况,有助于更好调整动、静态足底压力分布,降低跌倒风险,减少跌倒发生情况。  相似文献

ObjectiveThe aim of study was to analyze the association between Quadriceps Angle (QA) and plantar pressure, navicular height (NH), and calcaneo-tibial angle (CTA).MethodsA total of 64 volunteers (mean age: 22.25 ± 2.54 (range:19–33)) participated in this cross sectional study. EMED-m (Novel GmbH, Germany) electronic pedobarograph was employed for dynamic plantar pressure measurement using two step protocol. The angle between the vertical axis of calcaneus and the long axis of Achilles tendon for CTA. The height of navicular tubercle from the ground was measured while the subject was standing on both feet for NH. QA was measured while the subject was standing in a relaxed posture where both feet bearing equal weight.ResultsThere were significant negative correlations between QA and maximum force (MxF) under the 4th. metatarsal head (MH4). The QA was also significantly correlated with MxF and force-time integral (FTI) under the bigtoe (BT). FTI under the 3rd. metatarsal head (MH3), MH4 and 5th. metatarsal head (MH5) were significantly negatively correlated with QA. Pressure-time integral (PTI) under the MH4 and MH5 were found to be significantly negatively correlated with QA. A significant correlation was also found between QA and NH (p < 0.0001), whilst there was no correlation between QA and CTA. Regression analysis showed that NH was appeared as the major contributor for the QA (β = −0.49, p < 0.001) in the dynamic condition, followed by BT-FTI (β = 0.37, p < 0.001) and MH5-MxF (β = −0.21, p < 0.037).ConclusionThese findings may imply that the NH which can at least be controlled by appropriate shoe inserts may affect QA. This way, loading pattern of both plantar region and whole lower extremity may be altered.Level of evidenceLevel III, Diagnostic Study.  相似文献

ObjectiveHyperglycemia leads to increase advanced glycation end products (AGEs) in patients with type 1 and type 2 diabetes. Subsequently, formation of AGEs can cause increased plantar fascial thickness (PFT), an imaging feature of plantar fasciitis (PF). This study evaluates the prevalence of PF in a contemporary cohort of type 1 diabetes and type 2 diabetes patients managed according to current standards, compared to patients without diabetes.Research design and methodsThis is a five-year prevalence study in a large tertiary health system (approximately 535,000 patients/visits/year) with a single electronic medical record (EMR), applying a cohort discovery tool and database screen (Data Direct) with use of ICD-9 and ICD-10 codes. All patients with a PF diagnosis between 01/01/2011 and 01/01/2016 were included and divided into 3 groups: type 1 diabetes (7148 patients), type 2 diabetes (61,632 patients), and no diabetes (653,659 patients). Prevalence rates were calculated, accounting for other risk factors including BMI and gender using Fisher's exact test.ResultsThe overall prevalence of PF in the entire study population was 0.85%. Prevalence rates were higher in patients with diabetes, particularly with type 2 diabetes (42% and 64% higher compared with patients with type 1 diabetes and no diabetes respectively). Individually, PF rates were 0.92% in type 1 diabetes and 1.31% in type 2 diabetes compared with 0.80% in patients with no diabetes (Type 1 vs. no diabetes p = 0.26; Type 2 vs. no diabetes p ? 0.0001; Type 1 vs. Type 2 diabetes p = 0.0054). Females in all groups had higher prevalence of PF than males (p ? 0.0001 for all), with those patients with diabetes having higher prevalence rates than those without diabetes. Patients with higher BMI levels (BMI ≥ 30 kg/m2) were also more likely to have PF in all categories except males with type 1 diabetes (p = 0.40).ConclusionsIn this large contemporary population managed in a tertiary health system, prevalence rates of PF were substantially higher in patients with diabetes compared with no diabetes, particularly in type 2 diabetes. Female gender and higher BMI were also associated with higher prevalence of PF in this cohort.  相似文献

Background:Stretching has been proven to be effective on pain and range of motion (ROM) in patients with plantar fasciitis. Despite recent gain in popularity and the proposed theories of effectiveness of foam roller, there is a lack of literature on the effect of foam rolling on plantar fasciitis.Objective:The objective of this study was to compare the effects of foam rolling and stretching on pain and ankle ROM in patients with plantar fasciitis.Methods:A total of 50 participants were included and randomly allocated to the stretching and foam roller groups. Visual analog scale (VAS), pressure pain thresholds (PPTs) for gastrocnemius, soleus and plantar fascia and weight-bearing lunge test (WBLT) measurements were recorded at baseline and immediately after treatment.Results:Within-group analysis has shown there is a statistically significant difference (p<0.001) in all the outcome measures in both foam roller and self-stretching groups. The between-groups analysis showed no statistical significance difference in VAS, plantar fascia PPT and WBLT parameters (with p-values of 0.171, 0.372 and 0.861, respectively); however, significant differences were found in gastrocnemius PPT (p=0.029) and soleus PPT (p=0.013).Conclusion:It was seen that both stretching and foam rolling techniques helped in reducing pain and increasing the ROM. Ganglions are cystic lesion more commonly seen around the wrist joint. Gangliomas of plantar aspect of the foot are rare. We have presented a case of an unusual serpiginous ganglioma of the plantar aspect of the foot. Less literature is available regarding plantar foot gangliomas. The treatment is challenging because of occurrence of the lesion at weight bearing zone. Available options for treatment includes conservative with splint, intralesional injection,arthroscopic excision and open excision.  相似文献

IntroductionIn Plantar Fasciitis, the main concern of the patients is the pain that disturbs their day to day activities. Different modalities of treatments are being used for its pain management. This study seeks to investigate and compare decrease in level of pain following treatment with Methylprednisolone injections (DMP) Vs Extra-Corporeal Shock Wave Therapy (ESWT) in plantar fasciitis.MethodsThis prospective comparative non randomized study was conducted in 60 patients of any age presenting with Plantar Fasciitis at B&B Hospital, Kathmandu. Patients were divided into 2 groups (30 each) based on patients preference. Methylprednisolone injection was given to one group and another group received ESWT. Follow up of both groups were carried out at 6 weeks, 3 months and 6 months and the outcome was measured with Visual Analogue Pain Scale (VAS). Statistical analysis wasdone using SPSS software, version 13. Chi-square and Independent t-test were applied to look for significant variations in outcome.ResultsFollow-up at 6 weeks revealed 26 (86.7%) patients attaining VAS < 5 in ESWT group in comparison to 16 (53.3%) patients of DMP group (p = 0.005). At the end of 6 months, 5 patients in DPM group still had significant pain (VAS ≥ 5) compared to 2 patients in ESWT group (p = 0.02). However 11 patients of DMP group and 23 of ESWT group received single episode of treatment only and had persistent symptomatic pain relief (VAS < 5) during all follow ups at 6 weeks, 3 months and 6 months (p = 0.004).ConclusionsPlantar fasciitis was more prevalent in overweight population and females. Significant improvement in pain was observed with both ESWT and DMP Injections. However, ESWT was found to be more effective than DMP Injections for treatment of Plantar Fasciitis.  相似文献
