目的:探讨ICU护士执行身体约束缩减行动的障碍因素,为制定身体约束标准化管理方案提供依据。方法:基于理论域框架(TDF)制定访谈提纲,采用目的抽样法对天津市某三级甲等医院的12名ICU护士进行半结构式访谈,按照TDF的相关领域对转录后的资料进行归类,用内容分析法对访谈资料进行分析。结果:ICU护士对身体约束缩减行动理解存在偏差、对约束替代措施内容的不熟悉是其执行身体约束缩减行动的障碍因素之一;身体约束缩减方案评估工具、决策流程的不规范对缩减约束行动的开展造成了一定阻碍;环境因素的限制及对缩减约束结果的担忧,使ICU护士没有足够的信心去执行;科室缺乏支持缩减约束的组织氛围,难以引起护士对执行约束缩减行动的重视,进而削减执行的动力。结合理论域框架将上述因素分别归属为知识、自我效能、环境背景和资源、社会影响、行为规范5个领域。结论:知识、自我效能、环境背景和资源、社会影响及行为规范为ICU护士执行身体约束缩减行动的障碍因素。科室要在明确障碍因素的基础上,加强领导层对执行缩减约束的重视和支持。制定缩减约束评估决策规范化流程以及培训方案,倡导团队协作,创造支持身体约束缩减行动的组织氛围,坚定ICU护士执行缩减约束的信心,促进身体约束缩减行动的持续实施。  相似文献

The discussion paper will focus on continuity of care relating to previous NZ research, specifically to transitioning complex preterm infants from NICU to home based on parent experiences, and on the practice developments that have occurred, to ensure optimal health outcomes. Previous NZ research discovered parent desire a consistent service delivery for the entire transition journey from NICU and at home.An informative and comprehensive opportunity has occurred for reflective professional practice, evaluation, development and implementation which have transpired in positive change through innovative practice developments and support change implementation in Wellington, NZ. This has resulted in the articulation of a model of care that has both embraced and integrated parental desires for a continuity of care process for complex preterm infants. This has been achieved by having the same Discharge Facilitator/Key Case Manager present within the NICU and external to the NICU for Home-based infants for the entire transition journey.The paper will focus and emphasis additional practice development changes and furthermore, will present a real purpose, for other countries to learn of such practice developments that have exemplified a celebratory success for families of Wellington, NZ.  相似文献

目的 探讨精神科护士夜班经历创伤事件后的成长体验,以期为护士不同阶段心理情况进行针对性干预和指导,并为护士职业生涯早期心理建设提供方向。方法 采用扎根理论的研究方法,2020年3月—7月对广东省某三级甲等医院12名夜班经历创伤事件的精神科护士进行深度访谈,采用Strauss和Corbin扎根理论分析资料,分为开放式登录、轴心式登录和选择式登录3个阶段,持续比较分析资料。结果 发现精神科护士夜班经历创伤事件后经历了应激期、调适期、成长期3个不同阶段。结论 精神科护士夜班经历创伤事件后的成长为动态过程,管理者或干预者可根据不同阶段进行针对性干预和指导,并在护士早期培训中促使其提前做好相关知识、技能的储备和必备心理素质的塑造。  相似文献

BackgroundA randomized controlled study was conducted on the effects of laughter yoga in reducing the perceived stress and burnout levels in nurses during the pandemic and in increasing their life satisfaction.Materials and methodsThe study was conducted with 101 (51 in the experimental group, and 50 in the control group) nurses providing care for patients with COVID-19. A total of eight sessions of laughter yoga were applied to the nurses in the experimental group for four weeks, twice a week. The Introductory Information Form, Perceived Stress Scale, Maslach Burnout Inventory, and Life Satisfaction Scale were used during data collection.ResultsThe difference between the pre- and posttest Perceived Stress Scale, Maslach Burnout Inventory, and Life Satisfaction Scale score averages of the nurses in the experimental group were found to be statistically significant (p < 0.05), whereas the difference between the pre- and posttest score averages of the nurses in the control group was not significant (p > 0.05).ConclusionLaughter yoga is an effective method to reduce perceived stress and burnout while also increasing life satisfaction.Clinicaltrials.gov idNCT05317091.  相似文献

IntroductionNew and flexible multidisciplinary workforce models are needed to address unnecessary medication regimen complexity in residential aged care facilities (RACFs). This study will investigate the feasibility of a nurse practitioner-pharmacist telehealth-based collaborative care model to simplify complex medication regimens.MethodsThis is a pragmatic, non-randomized pilot and feasibility study of up to 30 permanent residents from 4 RACFs in Western Australia. Simplification will be conducted in accordance with a validated 5-step implicit process. Nurse practitioners will identify residents potentially interested in and who may benefit from simplification, including any regulatory or safety imperatives that might preclude simplification. Medication regimens will be assessed by an off-site clinical pharmacist to identify opportunities for simplification in terms of drug–drug, drug–food, or drug–time interactions, and the availability of alternative formulations. The pharmacist will communicate simplification opportunities to nurse practitioners via video case conferencing. Nurse practitioners will then discuss simplification opportunities with the resident, caregiver and the health and care team, including any unintended consequences for the resident or RACF. The primary outcome measure will be feasibility (stakeholder acceptability, protocol adherence, recruitment and retention rates). Secondary outcomes include change in the number of medication administration times per day, medication and behavioral incidents, falls and fractures, hospitalization and mortality at 4 months.Ethics and disseminationEthical approval has been obtained from the Monash University Human Research Ethics Committee. 目的 了解江苏省省级专科护士工作投入现状及影响因素,为针对性管理提供参考。方法 采用一般资料调查问卷与专科护士工作投入量表对2 472名省级专科护士进行调查分析。结果 专科护士工作投入总均分为4.06±0.46;多元线性回归分析显示,性别、聘任方式、专科类别、工作类型、出任专科护理门诊、论文发表是专科护士工作投入的影响因素(P<0.05,P<0.01)。结论 专科护士工作投入水平较高,其影响因素较多,护理管理者应关注专科护士专业开展现况,采取针对性措施支持专科护理的有效开展,从而进一步提高专科护士的工作投入水平。  相似文献

目的评价基于多学科协作的加速康复外科(ERAS)护理专业组管理模式在临床实践中的效果。方法将接受常规ERAS方案的500例外科患者设为对照组,将接受ERAS护理专业组管理(在外科13个病区共设置6个专业小组,在常规ERAS方案的基础上,以专业组为依托,推动多学科协作,实现ERAS护理精准化)的750例患者作为干预组。比较干预前后护士的专业知识、职业认同水平以及两组患者ERAS相关指标。结果实施后护士专业知识水平及职业认同感均显著高于实施前(均P0.01);干预组术前禁食水时间、术中低体温发生率、早期下床时间、留置尿管时间以及术后中重度疼痛、营养不良和深静脉血栓发生率显著低于对照组,术后住院时间、住院费用显著少于对照组(P0.05,P0.01)。结论多学科ERAS护理专业组管理模式可有效提升护士的专业能力,保障ERAS的实施,改善患者结局。  相似文献

IntroductionThere is insufficient understanding of diagnosis of etiologic dementia subtypes and contact with specialized dementia care among older Americans.MethodsWe quantified dementia diagnoses and subsequent health care over five years by etiologic subtype and physician specialty among Medicare beneficiaries with incident dementia diagnosis in 2008/09 (226,604 persons/714,015 person-years).ResultsEighty-five percent of people were diagnosed by a nondementia specialist physician. Use of dementia specialists within one year (22%) and five years (36%) of diagnosis was low. “Unspecified” dementia diagnosis was common, higher among those diagnosed by nondementia specialists (33.2%) than dementia specialists (21.6%). Half of diagnoses were Alzheimer's disease.DiscussionAscertainment of etiologic dementia subtype may inform hereditary risk and facilitate financial and care planning. Use of dementia specialty care was low, particularly for Hispanics and Asians, and associated with more detection of etiological subtype. Dementia-related professional development for nonspecialists is urgent given their central role in dementia diagnosis and care.  相似文献
