BackgroundPost-operative elderly hip fracture patients require significant rehabilitation. Nandrolone is an anabolic steroid used to promote muscle growth. This study aims to examine the effect of nandrolone in improving rehabilitation and quality of life in elderly female patients with hip fractures undergoing hemiarthroplasty.MethodsThis is a double-blinded prospective randomized-controlled-trial consisting of female patients above the age of 65 with an isolated neck of femur fracture planned for a hip hemiarthroplasty. Participants were randomized into two arms of the study – 50 mg intramuscular nandrolone vs normal saline placebo administered on post-operative day 0, and weeks 2, 6 and 12. The participants were followed up across a 1-year period following the surgery. Clinical outcomes such as time taken to achieve rehabilitation milestones, distance of ambulation and thigh muscle girth, and functional scoring with SF-36 questionnaire were recorded at intervals of 2, 6 and 12 weeks, 6 months and 1 year following the surgery.ResultsThere were a total of 23 subjects with 11 in the steroid group and 12 in the placebo group. There was no significant difference in demographics and injury patterns between both groups. There was no significant difference for time taken to achieve various rehabilitation milestones and distance of ambulation. SF-36 scores on discharge and at 1-year follow-up mark were comparable. There was no difference in the complication rate between both groups.ConclusionIntra-muscular Nandrolone after hip surgery in elderly female patients does not result in short to mid-term improved rehabilitation or functional outcomes. 目的: 探讨采用大转子延长截骨钢丝固定加自体骨屑植骨进行全髋关节置换翻修的临床效果。方法: 自2010年12月至2018年12月,应用大转子延长截骨钢丝固定结合自体骨屑植骨,行全髋关节置换翻修术患者18例,其中男8例,女10例;年龄68~82(78.89±3.32)岁;初次置换术后至翻修时间9~22(16.33±2.93)年。术后定期随访记录患者截骨块愈合时间、完全负重活动时间、髋关节Harris评分及并发症等情况。结果: 纳入研究的18例患者均获得随访,时间16~38(25.78±6.65)个月。手术切口均Ⅰ期愈合。切口长度16~21(18.89±1.32) cm。手术时间105~128(115.44±6.59) min;出血量240~285(267.44±13.77) ml。截骨块愈合时间12~18(15.61±1.75)周;患者完全负重活动时间14~22(17.78±2.53)周。术前髋关节Harris评分(47.11±5.04)分,完全负重活动时髋关节Harris评分(76.39±3.85)分,末次随访时髋关节Harris评分(82.22±2.76)分,差异有统计学意义(P<0.05)。随访期间,患者均未发生患肢短缩、感染、切口愈合不良、假体松动及下沉、假体周围骨折等并发症。结论: 在全髋关节置换翻修术时,应用大转子延长截骨钢丝固定结合自体骨屑植骨,能够获得满意的临床治疗效果,但需要术者对翻修术前、术中及术后恢复等各个时期做好系统规划。  相似文献

ObjectiveDeterioration of the native aortic valve function by a late progression of rheumatic disease is not infrequent in patients who underwent rheumatic mitral valve surgery; however, this phenomenon has not been clearly quantified.MethodsA total of 1155 consecutive patients (age 52.0 ± 12.9 years; 807 female) who underwent rheumatic mitral valve surgery without concomitant aortic valve surgery from 1997 to 2015 were enrolled. The primary end point was the composite of progression to severe aortic valve dysfunction or a requirement of subsequent aortic valve replacements during follow-up. To determine the risk factors of the primary outcome, we performed the generalized linear mixed model.ResultsThe baseline severities of aortic valve were none to trivial in 880 patients (76.2%), mild in 256 patients (22.2%), and moderate in 19 patients (1.6%). The latest 1062 echocardiographic assessments (91.9%; median, 81.2 postoperative months; interquartile range, 37.3-132.1 months) demonstrated 26 cases (0.33%/patient-year) meeting the primary end point during follow-up. Cumulative incidence of the primary end point at 10 years was 0.4% ± 0.3% and 7.4% ± 2.5% depending on the presence of mild or greater aortic valve dysfunction at baseline (P < .01). In multivariable analyses, aortic valve peak pressure gradient (odds ratio, 1.14; 95% confidence interval, 1.10-1.20), aortic regurgitation degree (mild over none: odds ratio, 3.26; 95% confidence interval, 1.15-9.23), and time (odds ratio, 1.30; 95% confidence interval 1.19-1.41) were significantly associated with the occurrence of the primary end point.ConclusionsProgression of severe aortic valve dysfunction and the need for aortic valve replacement are uncommon in patients undergoing rheumatic mitral valve surgery. However, such events were relatively common among those with mild or greater aortic valve dysfunction at the time of mitral valve surgery.  相似文献

目的探讨不同的康复方式对老年全膝关节置换术患者术后焦虑、镇痛效果及生活质量的影响。 方法选取2017年11月到2020年6月于海南西部中心医院骨科门诊收入院的退行性膝关节炎老年患者100例,年龄≥65岁,均为初次行单侧全膝关节置换术,排除存在既往膝关节手术史、下肢其他关节功能障碍、凝血功能异常、下肢深静脉血栓(DVT)以及依从性差的患者。所有患者由同一高年资手术医师完成手术。按随机数字法分为两组,其中实验组给予加速康复措施,对照组给予常规康复措施,记录包括手术时间、住院时间、术中出血量,评估住院指标。于术前、术后1个月、术后6个月评估患者疼痛视觉评分(VAS)、焦虑自评量表系统评分(SAS)、膝关节综合评分(AKS)及生活质量评分(SF-36)。记录患者自手术至术后6个月内发生的手术相关性感染、DVT、假体松动、关节返修、骨关节炎进展等发生情况。计量资料采用独立样本t检验,重复测量资料比较采用球形检验,若不满足球型分布,需采用Green house-Geisser方法对自由度进行校正后计算;计数资料采用卡方检验或Fisher精确检验。 结果实验组较对照组手术时间(t=8.618)、住院时间(t=4.116)较短,首次下床时间(t=16.360)较早,术中出血量及住院总花费较少(t=16.359、5.839,均为P<0.05)。经治疗,两组VAS评分、SAS评分均降低,AKS评分、SF-36评分均升高。术前、术后1个月、术后6个月两组患者SAS、VAS、AKS及SF-36评分均不满足球型分布,实验组SAS、VAS评分均低于对照组(F=5.201、9.018),AKS评分(F=7.231、10.205)、SF-36评分(F=20.102、7.118)均高于对照组,差异有统计学意义(均为P<0.05),且无交互作用(均为P>0.05)。实验组并发症发生率为10.0%,对照组为28.0%,差异有统计学意义(χ2=5.263,P=0.022)。 结论加速康复外科可优化老年全膝关节置换术患者的住院指标,减少术中出血及术后并发症,缓解焦虑,提高镇痛效果、膝关节功能及生活质量。  相似文献

目的观察基于出院准备服务的康复指导在全膝关节置换术患者中的应用效果,探讨最佳护理措施。 方法采用前瞻性分析的研究方法,选择2019年4月至2020年9月在南方医科大学南方医院首次行全膝关节置换术患者共210例,按照随机数字表法分组。105例设为对照组,实施常规护理与康复干预;105例设为观察组,在常规护理基础上实施基于出院准备服务的康复指导的延续性护理模式。入院时、出院前、出院后1个月及3个月采用膝关节功能评分量表、Bathel指数评价患者的关节功能、日常生活能力;采用出院准备度量表对比两组患者出院准备度情况。根据不同数据类型,采用独立样本t检验、秩和检验、卡方检验和重复测量方差分析进行统计分析。 结果通过对全膝关节置换术患者实施出院准备服务的康复指导,患者从入院至出院后3个月时间里,在膝关节功能及日常生活能力方面逐步得到改善,出院前观察组患者出院准备度评分显著高于对照组(t=36.020,P<0.01)。入院时和出院前两组患者的膝关节功能评分差异无统计学意义(F=1.127、0.936,均为P>0.05);出院后1个月及3个月观察组患者的膝关节功能评分高于对照组(F=21.069、10.476,均为P<0.01)。两组患者入院时和出院前的日常生活能力评分差异无统计学意义(F=0.979、1.158,均为P>0.05),在出院后1个月及3个月观察组患者的日常生活能力评分显著高于对照组(F=19.463、33.458,均为P<0.01)。 结论对全膝关节置换术患者实施基于出院准备服务的康复指导,可有效改善患者膝关节功能,提高其日常生活能力,促进患者早日康复。  相似文献

Given that the global population of elderly individuals is expanding and the difficulty of recovery, hip fractures will be a huge challenge and a critical health issue for all of humanity. Although people have spent more time at home during the coronavirus disease 2019 (COVID-19) pandemic, hip fractures show no sign of abating. Extensive studies have shown that patients with hip fracture and COVID-19 have a multifold increase in mortality compared to those uninfected and a more complex clinical condition. At present, no detailed research has systematically analyzed the relationship between these two conditions and proposed a comprehensive solution. This article aims to systematically review the impact of COVID-19 on hip fracture and provide practical suggestions. We found that hip fracture patients with COVID-19 have higher mortality rates and more complicated clinical outcomes. Indirectly, COVID-19 prevents hip fracture patients from receiving regular medical treatment. With regard to the problems we encounter, we provide clinical recommendations based on existing research evidence and a clinical flowchart for the management of hip fracture patients who are COVID-19 positive. Our study will help clinicians adequately prepare in advance when dealing with such patients and optimize treatment decisions.  相似文献

目的通过有限元分析,初步评估股骨头部分置换术后股骨与内植物模型的应力分布和位移。 方法选择一名成年健康志愿者,获取股骨全长CT扫描数据,利用Mimics 20.0软件、Geomagic软件及UG NX 12.0软件建立股骨头缺血坏死模型,骨隧道切除股骨头坏死区域后装配设计开发的股骨头假体,模拟单腿站立环境进行有限元仿真模拟,获取股骨与内植物模型的应力分布和位移数据。 结果股骨应力分布主要集中在股骨颈下方和股骨干皮质骨两侧,最大应力为48.25 Mpa,最大位移为10.98 mm。内植物模型的应力主要分布在金属内植物主体结构下方,最大应力为147.2 Mpa,最大位移为9.58 mm。 结论股骨头部分置换后,假体头与股骨头曲率一致,应力传导模式与正常侧髋关节一致,但在假体头与茎部连接处发生应力集中,应重点考虑选择更大强度的材料。  相似文献

BackgroundAccurate glenoid component positioning is an important determinant of outcome in a shoulder arthroplasty surgery. Optimal glenoid placement is determined using bony landmarks of the scapula. The Glenoid Vault Outer Cortex (GVOC) has been recently described as a new, more accurate radiological reference. This has, however, only been evaluated against current standard references in young patients. Further investigation of the GVOC in older patients is therefore warranted. We, therefore, evaluated the effect of aging on the accuracy and stability of the GVOC, in determining glenoid anatomy as compared to the commonly used Scapular Border (SB) plane.MethodsComputed tomography imaging of 129 individual scapulae was obtained retrospectively from a cohort of patients who had undergone either total body or region-specific computed tomography imaging which included the shoulder region. This comprised of 35 males and 33 females (64 and 65 scapulae respectively) who were aged from 30 to 92 years. Imaging of 54 scapulae was from patients aged ≥60 years. The accuracy of the GVOC plane was then assessed against the SB plane.ResultsIn all patients, the mean difference between estimates using the GVOC plane and the GR (actual) was 2.2° (standard deviation [SD], 4.2) for version, and 1.8° (SD, 4.9) for inclination (P < .001). This contrasted with values of 7.6° (SD, 7.6) for version, and 22.9° (SD, 10.8) for inclination when using the SB reference plane (P < .001). Within the group aged ≥60 years, the mean difference between estimates using the GVOC plane and the glenoid rim (actual) was 3.2° (SD, 4.7) for version and 1.9° (SD, 3.1) for inclination, (P < .001). This contrasted to 10.0° (SD, 7.2) for version, and 23.4° (SD, 10.7) for inclination when using the SB plane (P < .001).Separately it was noted that the GVOC’s relationship to the glenoid rim remained constant throughout aging as opposed to the SB which changed significantly over time effecting estimates of glenoid retroversion.ConclusionsThe GVOC is a new plane of reference developed specifically for the use in shoulder arthroplasty. It is shown to be more accurate and stable in the aging scapulae than the currently used SB plane. The future development of guides and planning softwares that utilize the GVOC may provide an important opportunity for improved accuracy and outcome in shoulder arthroplasty.  相似文献
