目的: 建立并验证一种人全血中氯喹、羟氯喹、阿比多尔的检测方法,并将其用于临床检测。方法: 全血样本经氯化铵裂解和乙腈蛋白沉淀后,采用液相-串联质谱联用仪测定。色谱柱为BEH C18,以含0.5%甲酸乙腈溶液-0.5%甲酸水溶液为流动相梯度洗脱,流速为0.4 mL·min-1,电喷雾正离子模式,多反应监测。结果: 氯喹、羟氯喹、阿比多尔在8~1 600 ng·mL-1线性关系良好;批内和批间精密度(RSD)均在12.71%以内,批内和批间准确度(RE)均在-7.12%~13.51%;提取回收率均在76.50%~92.49%;内标归一化的基质因子变异系数(RSD)均小于15%;在多种储存条件下,处理前后的样品稳定性良好。结论: 该方法专属性强,准确度高,重复性好,可用于3种药物治疗药物监测及中毒检测。  相似文献

目的 探讨腹腔镜直肠癌手术患者术中不同模式间歇充气加压装置(intermittent pneumatic compression,IPC)预防下肢深静脉血栓(deep vein thrombosis,DVT)的效果。方法 2020年4月—2021年4月,在温州市某三级甲等医院结直肠外科连续选择符合纳入标准的130例拟行腹腔镜直肠癌根治术患者作为研究对象。采用随机数字表法分为对照组、试验组Ⅰ、试验组Ⅱ、试验组Ⅲ。对照组围手术期采用常规护理方法预防下肢DVT。各试验组在对照组基础上,手术全程采用对应的IPC模式预防下肢DVT。试验组Ⅰ采用IPC双足模式,足底气囊加压130 mmHg(1 mmHg=0.133 kPa),持续6 s,放松12 s。试验组Ⅱ采用IPC双小腿模式,气囊于小腿序贯加压80 mmHg,持续12 s,放松24 s。试验组Ⅲ采用IPC双小腿+大腿模式,气囊分别从足踝、小腿、大腿部位序贯加压80、70、60 mmHg,持续24 s,放松24 s。比较4组麻醉诱导前和拔管后双下肢静脉血流动力学变化及术后血栓发生率。结果 与对照组相比,试验组Ⅰ和试验组Ⅱ:麻醉诱导前和拔管后,双侧股总静脉、股浅静脉、腘静脉的血流速度变化率差异有统计学意义(P<0.05),管径变化率差异无统计学意义(P>0.05)。术后第1天下肢DVT发生率明显下降,差异有统计学意义(P=0.039,P=0.042)。与对照组相比,试验组Ⅲ:麻醉诱导前和拔管后,双侧股总静脉血流速度变化率差异无统计学意义(P>0.05),各静脉管径变化率差异均无统计学意义(P>0.05)。术后第1天下肢DVT发生率无改善,差异无统计学意义(P=0.820)。4组术后第7天血栓发生率差异均无统计学意义(P=0.125)。结论 术中IPC足模式和IPC小腿模式均能改善下肢血流动力学,预防腹腔镜直肠癌患者术后第1天血栓发生,且效果相近。  相似文献

ObjectiveThe objective of this study was to estimate the antibacterial activity of three different herbal extracts against oral bacteria and their bioactive composition.MethodsUsing the disk diffusion technique, the antibacterial activities of three different extracts (lemongrass, sage, and guava leaf) were evaluated against oral bacteria (Streptococcus mutans, Staphylococcus aureus, and Enterococcus faecalis). Additionally, the bioactive components of the herbal extracts were assessed by employing the gas chromatography-mass spectrometry technique.ResultsThe sage, lemongrass, and guava leaf extracts suppressed the proliferation of all three tested bacterial strains at different rates. The phytochemical analysis revealed that sage extract possessed the highest content of antioxidants, phenols, and flavonoid compounds. The gas chromatography-mass spectrometry analysis of the tested plants revealed the presence of vital bioactive compounds.ConclusionsLemongrass, sage, and guava leaf extracts have potent antibacterial activities, are rich in bioactive compounds, and could be utilized as natural remedies for the prevention of oral diseases.  相似文献

目的 建立一种联合表面增强拉曼散射(SERS)与卷积神经网络(CNN)的方法,用于耐甲氧西林金黄色葡萄球菌(MRSA)和对甲氧西林敏感金黄色葡萄球菌(MSSA)的准确鉴定。方法 合成带正电的纳米银颗粒(AgNPs+)作为SERS基底,测量MRSA和MSSA的SERS指纹谱并制作数据集,构建浅层一维CNN网络并在该数据上训练生成MRSA/MSSA二分类模型。结果 合成的AgNPs+能够通过静电引力紧密地吸附在细菌菌体表面并产生显著的SERS效应,在654、731 cm-1等7个波段均有明显的拉曼峰被增强出。提出的SERS-CNN方法用于MRSA和MSSA检测的准确率超过94.5%,重现性小于5%,检测灵敏度达到102 cells/mL。结论 建立的联合SERS与CNN的方法可用于MRSA的准确检测。  相似文献

BackgroundLymphedema is a serious complication of axillary lymph node dissection (ALND) with an incidence rate of 20%. Simplified Lymphatic Microsurgical Preventing Healing Approach (SLYMPHA) is a safe and relatively simple method, which decreases incidence of lymphedema dramatically. Our initial study showed an 88% decrease in clinical lymphedema rate. In the initial study, we used arm circumference measurement for the diagnosis of lymphedema and median follow up was 15 months. The aim of this study was to confirm these results after a long-term follow up period and by using bioimpedance spectroscopy (L-Dex) technology in detecting lymphedema.Study designAll patients, undergoing ALND with or without SLYMPHA between January 2014 and November 2020 were included in the study. Patients with no postoperative L-Dex measurements were excluded. A L-Dex score outside the normal range (±10 L-Dex unit) or ≥10 L-Dex unit increase above patient's baseline was considered as lymphedema. The incidence of lymphedema was compared between patients with and without SLYMPHA.Results194 patients were included in the study. 57% of cohort underwent SLYMPHA. Mean follow-up time was 47 ± 37 months. Patients, who underwent SLYMPHA, had a significantly lower rate of lymphedema (16% vs 32%; p = 0.01; OR 0.4 [0.2–0.8]).ConclusionSLYMPHA is a safe and relatively simple method, which continued its efficacy after a long-term follow up period. It should be considered as an adjunct procedure to ALND for all patients during initial surgery.  相似文献

目的:研究脑、肠、肾区域血氧饱和度(rSO2)预测小儿先天性心脏病术后急性肾损伤(AKI)发生的价值。方法:选择2020年1—12月在上海交通大学医学院附属上海儿童医学中心进行体外循环先天性心脏病纠治术、体重大于2.5?kg且年龄在1岁及以下的患儿57例。采用近红外光谱连续监测患儿术后48?h内脑、肠、肾rSO2。比较AKI组与非AKI组,以及发生与未发生2级及以上AKI患儿脑、肠、肾rSO2的变化差异,并采用ROC曲线分析肠、肾rSO2对术后发生AKI及严重程度的预测价值。结果:57例患儿中,38例(66.7%)发生AKI,其中AKI 1级18例(47.4%),AKI 2级9例(23.7%),AKI 3级11例(28.9%),AKI总发生率约为66.7%。AKI组与非AKI组脑rSO2的变化差异无统计学意义(F=0.012,P>0.05),但AKI组肠rSO2和肾rSO2明显低于非AKI组(F=5.017和5.003,均P<0.05)。发生与未发生2级及以上AKI患儿脑rSO2的变化差异无统计学意义(F=0.311,P>0.05),但发生2级及以上AKI患儿肠rSO2和肾rSO2均低于未发生2级及以上AKI患儿(F=6.431和14.139,均P<0.05)。对术后AKI的预测效能进行分析发现,24?h内肠rSO2的诊断效能较好,其中术后3?h肠rSO2诊断AKI的曲线下面积(AUC)为0.823,以术后3?h肠rSO2低于85%预测AKI的敏感度为66.7%、特异度为89.5%;24~48?h肾rSO2的诊断效能较好,其中术后31?h肾rSO2诊断AKI的AUC为0.918,以术后31?h肾rSO2低于84%预测AKI的敏感度为72.2%、特异度为84.2%。对术后发生2级及以上AKI的预测效能进行分析发现,24?h内肠rSO2的诊断效能较好,其中术后3?h肠rSO2诊断2级及以上AKI的AUC为0.829,以术后3?h肠rSO2低于84%预测2级及以上AKI的敏感度为62.2%、特异度为90.0%;24~48?h肾rSO2的诊断效能较好,其中术后34?h肾rSO2诊断2级及以上AKI的AUC为0.826,以术后34?h肾rSO2低于71%预测2级及以上AKI的敏感度为91.9%、特异度为55.0%。结论:肠、肾rSO2不仅对术后AKI发生有较好的预测作用,同时对AKI的严重程度也有较好的预测效能。  相似文献

Cognitive impairment has recently attracted researchers as one of the possible neuropsychiatric manifestations of COVID-19, although how the infection perpetuates impairment of cognitive functions is still obscure. We presented a 29-year-old male patient with COVID-19 who developed new-onset transient attention deficit and memory problems following a SARS-CoV-2 infection. Structural neuroimaging was normal. MR-spectroscopy (MRS) of the bilateral DLPFC revealed significant for decreased levels of N-acetylaspartate (NAA), glutamate, and glutamate/glutamine ratio. After a follow-up without any medical treatment but with suggestions of memory exercises for three months a control MRS screening of DLPFC showed improved levels of NAA, glutamate, and glutamate/glutamine ratio. This report may suggest that cognitive deficits in SARS-CoV-2 infection can result from glutamatergic dysfunction with decreased NAA and glutamate levels in bilateral DLPFC.  相似文献

BackgroundAlthough various studies have been conducted to demonstrate the possibility of Raman spectroscopy (RS) as a diagnostic tool for prostate cancer (PC), it is difficult to use it in the real clinical area because of imitations in various research processes. Therefore, we did a systematic review and meta-analysis about the accuracy in diagnostic use of RS for PC.MethodsA literature search was done using PubMed, Embase, and Cochrane library databases in March 2019 to analyze the accuracy of RS for diagnosis of PC. The accuracy of RS for diagnosis of PC was evaluated by means of pooled sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratio (DOR), and summary receiver operating characteristic (SROC).ResultsFive studies were included for qualitative analysis by screening the remaining articles according to the inclusion and exclusion criteria by means of a systematic review. The pooled sensitivity and specificity of RS were 0.89 (95% CI: 0.87–0.91) and 0.91 (95% CI: 0.89–0.93), respectively. The overall PLR and NLR were 9.12 (95% CI: 4.15–20.08) and 0.14 (95% CI: 0.07–0.29), respectively. The DOR of RS demonstrated high accuracy (73.32; 95% CI: 18.43–291.73). The area under the curves (AUCs) of SROC curves was 0.93.ConclusionsRS is an optical diagnostic method with high potential for diagnosis and grading of PC and has advantages of real-time and convenient use. In order to consider real-time use of RS in an actual clinical setting, more studies for standardization and generalization of RS performance and analytical method must be conducted.  相似文献
