背景 人口老龄化趋势下,罹患多种慢性病的失能或半失能老年人对居家医疗护理服务需求迫切,但基层医务人员提供居家医疗护理服务存在较大的医疗风险和安全隐患,加强居家医疗护理服务的安全管理,有利于提高基层医务人员提供居家医疗护理服务的意愿。 目的 调查基层医疗卫生机构对居家护理服务的安全支持情况和医务人员的服务意愿,为进一步推动居家医疗护理服务发展提供参考依据。 方法 2021年8—10月,采用便利抽样法在四川省五大经济区抽取49所基层医疗卫生机构的1 131例医务人员。自行设计调查问卷,以"问卷星"电子问卷的形式收集资料。问卷内容包括医务人员的一般情况(8个条目)、提供居家医疗护理服务的经历(2个条目)、医疗机构对医务人员的安全支持(7个条目)、医务人员的居家医疗护理服务意愿(1个条目)。比较不同特征医务人员提供居家医疗护理服务的意愿,采用二元Logistic回归分析医务人员提供居家医疗护理服务意愿的影响因素。 结果 1 131例基层医务人员中,692例(61.18%)报告有过居家医疗护理服务的经历,193例(17.06%)报告在服务过程中发生过不良事件/安全事件,531例(46.95%)报告其所在医疗机构未评估过患者接受居家医疗护理服务的安全风险,199例(17.60%)表示所在机构评估了医务人员提供居家医疗护理服务的安全风险,299例(26.44%)在居家医疗护理服务过程中使用APP定位系统,273例(24.14%)由机构配备报警、延迟预警等设备,807例(71.35%)表示居家医疗护理服务过程中没有获得所在机构内其他医务人员的支持措施,303例(26.79%)表示机构曾组织居家医疗护理服务的专项研讨活动,352例(31.12%)表示接受过有关居家医疗护理服务专项培训,853例(75.42%)表示愿意提供居家医疗护理服务。二元Logistic回归分析结果显示,就职的基层医疗卫生机构类型,目前受聘的专业技术职称,医疗机构是否评估过患者接受居家医疗护理服务的安全风险,在居家医疗护理服务过程中是否配备报警、延迟预警等设备,是否参加过有关居家医疗护理服务的专项培训,对医务人员提供居家医疗护理服务意愿有影响(P<0.05)。 结论 现阶段居家医疗护理服务的安全支持不足,多种因素影响基层医务人员提供居家医疗护理服务的意愿。基层医疗卫生机构应加强对患者接受居家医疗护理服务安全风险的评估,为提供居家医疗护理服务的医务人员配备报警、延迟预警等设备,积极举办居家医疗护理服务相关的专项培训并鼓励医务人员参加,以降低医务人员提供居家医疗护理服务的风险,保障其人身安全。  相似文献

PurposeTo describe the range of occupational badge dose readings and annualized dose records among physicians performing fluoroscopically guided interventional (FGI) procedures using job title information provided by the same 3 major medical institutions in 2009, 2012, and 2015.Materials and MethodsThe Radiation Safety Office of selected hospitals was contacted to request assistance with identifying physicians in a large commercial dosimetry database. All entries judged to be uninformative of occupational doses to FGI procedure staff were excluded. Monthly and annualized doses were described with univariate statistics and box-and-whisker plots.ResultsThe dosimetry data set of interventional radiology staff contained 169 annual dose records from 77 different physicians and 698 annual dose records from 455 nonphysicians. The median annualized lens dose equivalent values among physicians (11.9 mSv; interquartile range [IQR], 6.9–20.0 mSv) was nearly 3-fold higher than those among nonphysician medical staff assisting with FGI procedures (4.0 mSv; IQR, 1.8–6.7 mSv) (P < .001). During the study period, without eye protection, 25% (23 of 93) of the physician annualized lens dose equivalent values may have exceeded 20 mSv; for nonphysician medical staff, this value may have been exceeded 3.5% (6 of 173) of the time. However, these values did not account for eye protection.ConclusionsThe findings from this study highlight the importance of mitigating occupational dose to the eyes of medical staff, particularly physicians, performing or assisting with FGI procedures. Training on radiation protection principles, the use of personal protective equipment, and patient radiation dose management can all help ensure that the occupational radiation dose is adequately controlled.  相似文献

ObjectiveWe conducted a realist review to understand how (mechanism) and in what circumstances (context) evidence-based practices are sustained in rehabilitation (outcome).Data SourcesMEDLINE, Embase, reference lists, and targeted websites.Study SelectionTwo independent reviewers calibrated study selection; then 1 reviewer screened all titles and abstracts, while the second reviewer screened a random 20%. We repeated this process for full texts. We included 115 documents representing 61 implementation projects (8.9% of identified documents). Included documents described implementation projects in which physical therapists, occupational therapists, and/or speech-language pathologists were the target users of an evidence-based practice.Data ExtractionTwo reviewers repeated the independent process described in study selection to extract basic study and sustainability characteristics as well as context, mechanism, outcome, and strategy text.Data SynthesisUsing basic numerical analyses, we found that only 54% of evidence-based practices in rehabilitation are sustained. Furthermore, while authors who reported sustainability planning sustained the practice 94% of the time, sustainability planning in rehabilitation is rare (only reported 26% of the time). Extracted text was synthesized using the realist technique of inductive and deductive retroduction in which context, mechanism, outcome, and strategy text are combined into narrative explanations of how sustainability works. To inform these explanations, we applied normalization process theory and the theory of planned behavior. Collectively, the 52 identified narratives provide evidence for 3 patterns: (1) implementation and sustainability phases are interconnected, (2) continued use of the evidence-based practice can be interpreted as the ultimate sustainability outcome, and (3) intermediate sustainability outcomes (ie, fit/alignment, financial support, benefits, expertise) can become contextual features influencing other sustainability outcomes.ConclusionsImplementation teams can use the narrative explanations generated in this review to optimize sustainability planning. This can sustain practice changes and improve quality of care and patient outcomes. Future research should seek to iteratively refine the proposed narrative explanations.  相似文献

To overcome the extensive polymorphism found in human Plasmodium antigens and to avoid the lengthy characterization of their 3 dimensional structure and subsequent production of the native proteins we have been concentrated in large unstructured, non-or low-polymorphic fragments present in the blood stage of P. falciparum. Three fragments derived from the 2 family-specific and constant regions of merozoite surface protein (MSP2) and PFF0165c protein were previously selected for evaluation as potential single vaccine candidates. In order to increase and optimize their potential efficacy against P. falciparum infection the 3 antigens were combined in a single DNA recombinant product (FusN) and compared its antigenicity with that of single antigens in sera of volunteers living in endemic countries. Immunogenicity of the FusN was then compared with that of the mixture of 3 antigens in 3 strains of mice. Antigen specific, affinity purified human antibodies were then tested in antibody dependent cellular inhibition and merozoite opsonization assays. In addition, the antigen specific antibody response and its association with protection from malaria infection were determined. The data collected indicate that the recombinant product is an equal or better antigen /immunogen than fragments used either alone or as a mixture for vaccination in combination with adjuvant. In addition, antibody response to FusN shows a stronger association with protection than single fragments. The use of a single construct as vaccine would drastically reduce the cost of manufacturing and development of the GMP product.  相似文献
