目的 了解成都市非EV - A71手足口病患者住院费用及其影响因素。方法 通过成都市公共卫生临床中心病案管理系统,收集2018—2021年手足口病住院病人信息。采用秩和检验、多元线性回归分析不同血清型患者费用的影响因素。结果 共纳入2 310例手足口病患者,直接医疗费用中位数为4 187元。秩和检验结果显示,CV - A10患者的住院天数、重症的费用差异有统计学意义(U = - 4.40, - 3.59; P<0.05);CV - A16患者的住院天数、高热的费用差异有统计学意义(U = - 4.90, - 2.51; P<0.05);CV - A6患者的性别、住院天数、并发症、重症、支付方式的费用差异有统计学意义(U = - 2.81, - 13.54, - 4.00, - 4.28;H = 13.65; P<0.05);其他肠道病毒患者的住院天数、并发症、重症、支付方式的费用差异有统计学意义(U = - 7.46, - 3.25, - 2.45;H = 13.14;P<0.05);混合感染患者高热的费用差异有统计学意义(U = - 2.40, P<0.05)。多元线性回归结果显示,除混合感染外,住院天数是所有患者费用的影响因素。在CV - A10、CV - A6和其他肠道病毒中重症患者的费用更高;在CV - A6和其他肠道病毒中有并发症的患者费用更高。高热是混合感染患者费用的影响因素。结论 成都市非EV - A71感染手足口病患者的病毒血清型不同,费用的影响因素也有所不同。  相似文献

目的 为了解慢阻肺患者住院费用的主要影响因素,减缓住院费用过快增长,减轻患者、家庭及社会的经济负担,优化医疗服务价格体系提供科学依据。方法 收集贵州省某三甲医院2015—2019年的9 223例慢阻肺患者病案首页信息,对慢阻肺患者的住院费用和住院天数进行描述性分析,并采用多元回归分析拟合通径分析模型的方法,研究慢阻肺患者住院费用的影响因素。结果 慢阻肺患者的人均住院天数为12.4 d,人均住院费用为18 598.81元,60岁以上患者占比达87.3%,医保患者占73.5%,且住院天数是影响住院费用的主要因素,总效应系数为0.719,危重型患者总效应系数为0.181排第二位。通径分析研究结果显示,年龄、医保、公费、危重、手术、药物过敏、医嘱离院不仅对住院费用存在着直接影响的效应,同时还通过住院天数间接影响着住院费用。结论 在不影响医疗质量的情况之下,按照临床路径的程序,合理缩短住院天数,并加强对于可控因素的限制,采取有效的措施,最大可能的减轻患者住院费用的负担。  相似文献

目的 了解流动人口住院费医保报销现状及其影响因素,为减轻流动人口医疗经济负担提供参考。方法 利用2018年全国流动人口卫生计生动态监测数据,运用χ2检验和logistic回归分析流动人口住院费医保报销现状及其影响因素。结果 3 364名调查对象中,2 346名(69.7%)流动人口报销住院费,其中有18人重复报销,报销总人次为2 364次。2 346名报销者中,66.5%在流入地报销住院费。从报销总人次来看,不同参保类型的流动人口住院费报销情况不同,参加城镇职工医疗保险的报销率最高,为82.2%;而报销地点上,参加城乡居民医保、城镇居民医保、城镇职工医保的选择在流入地报销分别占76.1%、79.9%和89.8%。多因素结果显示,年龄为35~44岁(OR = 1.474,95%CI:1.083~2.007)、45~54岁(OR = 2.223,95%CI:1.587~3.115)、55~64岁(OR = 2.320,95%CI:1.562~3.446)、65岁及以上(OR = 4.179,95%CI:2.576~6.778),受教育层次为高中(OR = 1.397,95%CI:1.077~1.811)、大专(OR = 1.887,95%CI:1.390~2.562)、本科及以上(OR = 2.427, 95%CI:1.660~3.548),流动时间6~10年(OR = 1.282,95%CI:1.058~1.554)、10年以上(OR = 1.264,95%CI:1.018~1.570),省内流动(OR = 1.483,95%CI:1.234~1.781)、市内流动(OR = 1.883,95%CI:1.502~2.360),异地养老(OR = 2.305,95%CI:1.052~5.052),流入西部(OR = 1.283,95%CI:1.061~1.551)是流动人口报销住院费的保护因素;从事商业及服务业 (OR = 0.429,95%CI:0.301~0.611)、生产及运输业(OR = 0.645,95%CI:0.431~0.966)、其他(OR = 0.421,95%CI:0.295~0.600)是流动人口报销住院费的危险因素。结论 流动人口住院费医保报销仍有较大的提升空间。应提高医保区域统筹层次,优化医保报销流程,加强对流动人口医保政策宣传,做好流动人口医保报销工作。  相似文献

BackgroundThis study aimed to assess inpatient prevalence, characteristics, outcomes, and resource utilization of hospitalization for methanol intoxication in the United States.Materials and MethodsA total of 603 hospitalized patients with a primary diagnosis of methanol intoxication from 2003 to 2014 were identified in the National Inpatient Sample database. The inpatient prevalence, clinical characteristics, treatments, outcomes, resource utilization, were investigated. Multivariable logistic regression was performed to identify factors independently associated with in-hospital mortality.ResultsThe overall inpatient prevalence of methanol intoxication among hospitalized patients was 6.4 cases per 1,000,000 admissions in the United States. The mean age was 38±18 (range 0–86) years. 44% used methanol for suicidal attempts. 20% of admissions required mechanical ventilation, and 40% required renal replacement therapy. The three most common complications were metabolic acidosis (44%), hypokalemia (18%), and visual impairment or optic neuritis (8%). The three most common end-organ failures were renal failure (22%), respiratory failure (21%), and neurological failure (17%). 6.5% died in the hospital. Factors associated with increased in-hospital mortality included alcohol drinking, hypernatremia, renal failure, respiratory failure, circulatory failure, and neurological failure. The mean length of hospital stay was 4.0 days. The mean hospitalization cost per patient was $43,222ConclusionThe inpatient prevalence of methanol intoxication in the United States was 6.4 cases per 1,000,000 admissions. The risk of in-hospital mortality mainly depended on the number of end-organ failures.  相似文献

BACKGROUND Infections and associated morbidity and mortality may be more frequent in children who have undergone liver transplant than in healthy children.Immunization strategies to prevent vaccine-preventable infections(VPIs) can effectively minimize this infection burden.However,data on age-appropriate immunization and VPIs in children after liver transplant in Asia are limited.AIM To evaluate the immunization status,VPIs and non-VPIs requiring hospitalization in children who have undergone a liver transplant.METHODS The medical records of children who had a liver transplant between 2004 and 2018 at King Chulalongkorn Memorial Hospital(Bangkok,Thailand) were retrospectively reviewed.Immunization status was evaluated via their vaccination books.Hospitalization for infections that occurred up to 5 years after liver transplantation were evaluated,and divided into VPIs and non-VPIs.Hospitalizations for cytomegalovirus and Epstein-Barr virus were excluded.Severity of infection,length of hospital stay,ventilator support,intensive care unit requirement,and mortality were assessed.RESULTS Seventy-seven children with a mean age of 3.29±4.17 years were included in the study,of whom 41(53.2 %) were female.The mean follow-up duration was 3.68±1.45 years.Fortyeight children(62.3%) had vaccination records.There was a significant difference in the proportion of children with incomplete vaccination according to Thailand's Expanded Program on Immunization(52.0%) and accelerated vaccine from Infectious Diseases Society of America(89.5%)(P 0.001).Post-liver transplant,47.9% of the children did not catch up with ageappropriate immunizations.There were 237 infections requiring hospitalization during the 5 years of follow-up.There were no significant differences in hospitalization for VPIs or non-VPIs in children with complete and incomplete immunizations.The risk of serious infection was high in the first year after receiving a liver transplant,and two children died.Respiratory and gastrointestinal systems were common sites of infection.The most common pathogens that caused VPIs were rotavirus,influenza virus,and varicella-zoster virus.CONCLUSION Incomplete immunization was common pre-and post-transplant,and nearly all children required hospitalization for non-VPIs or VPIs within 5 years posttransplant.Infection severity was high in the first year post-transplant.  相似文献

In a recent meta-analysis the prevalence of coronavirus disease 2019 (COVID-19)-associated hyperglycemia was 25%, and that of COVID-19-associated new-onset diabetes was 19%. An association between hyperglycemia or new-onset diabetes and COVID-19 has been suggested. In a recent relevant study of critically and non-critically ill patients with COVID-19, we found that indeed beta-cell function was compromised in critically ill patients with COVID-19 and that these patients showed a high glycemic gap. Nevertheless, one quarter of critically ill patients with no history of diabetes have stress hyperglycemia, a finding which could obscure the prevalence of hyperglycemia or new-onset diabetes that could be attributed to COVID-19 per se.  相似文献

IntroductionInfluenza remains a clinically heavy burden worldwide. It is well known that some populations are at high risk of complications from influenza, whereas, even previously healthy people might suffer from severe influenza.The objective of this study was to clarify clinical manifestations of hospitalized patients without risk factors infected with influenza.MethodsThe clinical data for patients who were severely ill with influenza, and required hospitalization were gathered and analyzed between November 2014 and August 2020 (6 influenza seasons) using an internet-surveillance system. Among them, the patients who had no risk factors of complications from influenza were extracted.ResultsFinally, a total of 91 patients (9.0% of all influenza-related hospitalizations) without risk factors were analyzed. The no risk group was younger than the risk group, though other significant differences of clinical characteristics were not recognized between the groups. Pneumonia was the most common cause of hospitalization in the no risk group, and primary influenza viral pneumonia was the most common pneumonia. Antiviral drugs were administered in 96.7% of the no-risk group, and artificial ventilation was performed in 18.7%. In-hospital death was recorded for 3 patients without risk factors.ConclusionsSevere complications of influenza which required hospitalization may occur in a certain degree of patients with no risk factors. Efforts are needed to diagnose and treat influenza appropriately even in previously healthy younger patients. Continuous nationwide surveillance will be required to clarify risk factors for severe influenza even in previously healthy younger patients.(UMIN000015989).  相似文献

IntroductionRespiratory syncytial virus (RSV) is one of the most common causes of lower respiratory tract infections in children aged <5 years and is associated with long-term respiratory morbidities such as recurrent wheezing and asthma, decreased lung function, and allergic sensitization. The objective of this review was to evaluate the epidemiology and burden of RSV infection in the pediatric population in Japan.MethodsStudies indexed in PubMed and ICHUSHI databases during January 2010–December 2020 were manually reviewed. Data on proportion of RSV infections, seasonality, length of stay (LoS), mortality, medical expenses, and palivizumab use were extracted from the selected articles.ResultsNinety-three articles were included (PubMed, 64; ICHUSHI, 29). The proportion of patients/samples with an RSV infection was 5.5%–66.7%, and 6.0%–29.9% in the inpatient and outpatient departments, respectively. RSV infections generally occurred during autumn/winter; however, recently the peak has shifted to summer. The LoS was variable and depended on factors such as age, infection severity, wheezing, and RSV subgroups. Mortality rates varied from <1% to 19% depending on the infection severity. The average daily hospitalization and intensive care unit cost was JPY 34,548 while intensive care unit incurred an additional cost of JPY 541,293. Palivizumab was indicated for high-risk infants and 0%–3% of patients required hospitalization despite palivizumab use.ConclusionsRSV imposes a significant burden on the Japanese healthcare system, suggesting a need to create awareness among caregivers of children, pregnant women and healthcare professionals to ensure early recognition of infection and adequate treatment or prophylaxis.  相似文献
