目的:探讨舌三针联合康复训练对脑梗死后假性延髓麻痹所致构音障碍疗效,以期提高脑卒中后患者生命质量。方法:选取2016年12月至2019年1月海南省人民医院收治的脑梗死后假性延髓麻痹所致构音障碍患者78例进行回顾性分析,按照治疗方法不同分为对照组和观察组,每组39例。对照组常规西医+康复训练治疗,观察组在以上基础上加用舌三针治疗,均连续治疗6周。观察2组治疗前、完成治疗后血脂、Frenchay构音评分、血液流变学、言语空气动力学指标变化并比较。完成治疗后总结疗效情况。结果:1)2组治疗完成后反射、呼吸、唇、颌、软腭、喉、舌、言语评分和治疗前比较均显著上升(P<0.05),且观察组上升幅度显著高于对照组(P<0.05)。2)2组治疗完成后三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)较治疗前均显著下降,高密度脂蛋白胆固醇(HDL-C)较治疗前比较均显著上升(P<0.05),且观察组上升幅度显著高于对照组(P<0.05)。3)2组治疗完成后全血高低切黏度、血浆黏度、红细胞压积较治疗前均显著下降(P<0.05),且观察组优于对照组(P<0.05)。4)对照组治疗前、完成治疗后各指标比较差异无统计学意义(P>0.05),观察组治疗完成后平均声压级(MSDV)、音调(P)、最长发声时间(MPT)、平均气流率(MFR)、声门下压(SGP)、声门阻力(GR)、发声效率(VE)较治疗前均显著升高(P<0.05),且观察组优于对照组(P<0.05)。5)观察组治愈率38.46%、总有效率89.74%,显著高于对照组治愈率23.08%、总有效率79.49%(P<0.05)。结论:舌三针联合康复训练能通过促进脑微循环,改善血脂,促进言语功能来改善脑梗死后假性延髓麻痹所致构音障碍症状,提高生命质量。 相似文献

目的:观察“风三针”针刺疗法在治疗中风后吞咽障碍的临床疗效。方法:随机收集中风后吞咽障碍患者60例,按照不同的治疗方法分为对照组和治疗组各30例。两组均先进行吞咽康复训练,后治疗组予风三针针刺治疗,观察两组治疗后洼田饮水试验评分,VFSS吞咽难度评分,并进行对比分析其临床疗效。结果:对照组及治疗组在治疗4个疗程后,治疗组总有效率93.33%高于对照组70.00%(P<0.05); 两组洼田饮水试验评分治疗后均低于治疗前(P<0.05),治疗组的评分低于对照组(P<0.05); 两组的VFSS吞咽难度评分治疗后均高于治疗前(P<0.05); 治疗组的VFSS难度评分高于对照组(P<0.05)。结论:“风三针”针刺疗法在治疗中风吞咽障碍中临床疗效确切。 相似文献

Fabrication of nanofibrous biomaterials composed of natural and synthetic materials that incorporated with antibiotic and growth factors with controlled release manner is an attractive topic in wound healing. The purpose of this study was to prepare optimal composite of materials as biomimetic nanofibrous mats for application in wound healing. The mat was prepared of polycaprolactone (PCL) in the bottom, chitosan/poly ethylene oxide (Cs/PEO) in the middle, and PCL/collagen (PCL/Coll) in the top layer. A panel of standard characterization tests of nanofibrous mat was performed and its compatibilities in strength and integration were confirmed. Middle layer was loaded with epidermal growth factor (EGF) and basic fibroblast growth factor (bFGF), and silver sulfadiazine (SSD) was incorporated in the bottom layer as an anti‐infection factor. Then, on the dorsum of rats, a 400‐mm2 wound was created and surrounded by a silicone ring to control the usual tissue contractions. Nanofibrous mats with or without growth factors were applied as wound dressings and at day 14, the healing process was evaluated. At day 14, the treated group by designed mat showed faster epithelialization and angiogenesis. Silicone ring in the test group was desirable in wound closure compared to the control group. Reformation of skin tissue was manifested in a shorter time. This composite nanofibrous mat could be introduced as a dynamic and effective candidate for wound dressing. 相似文献

Three‐dimensional (3D) printing is a digital rapid prototyping technology based on a discrete and heap‐forming principle. We identified 53 articles from PubMed by searching “Hip” and “Printing, Three‐Dimensional”; 52 of the articles were published from 2015 onwards and were, therefore, initially considered and discussed. Clinical application of the 3D printing technique in the hip joint mainly includes three aspects: a 3D‐printed bony 1:1 scale model, a custom prosthesis, and patient‐specific instruments (PSI). Compared with 2‐dimensional image, the shape of bone can be obtained more directly from a 1:1 scale model, which may be beneficial for preoperative evaluation and surgical planning. Custom prostheses can be devised on the basis of radiological images, to not only eliminate the fissure between the prosthesis and the patient's bone but also potentially resulting in the 3D‐printed prosthesis functioning better. As an alternative support to intraoperative computer navigation, PSI can anchor to a specially appointed position on the patient's bone to make accurate bone cuts during surgery following a precise design preoperatively. The 3D printing technique could improve the surgeon's efficiency in the operating room, shorten operative times, and reduce exposure to radiation. The main objective of this preliminary study was to evaluate the feasibility and safety of 3‐D laparoscopic living donor left nephrectomy (LDLN). The secondary objective was to compare intraoperative and postoperative outcomes between 3‐D and 2‐D laparoscopic LDLN. All patients who underwent a laparoscopic LDLN from January 2015 to April 2018 in a university center were included. All surgeries were performed by three experienced surgeons. Seventy three patients were included the following: 16 underwent a 3‐D laparoscopic LDLN (3‐D group), and 57 underwent a 2‐D laparoscopic LDLN (2‐D group). Operative time and warm ischemia time (WIT) were significantly lower in the 3‐D group (operative time: 80.9 ± 10.2 vs 114.1 ± 32.3 minutes in the 3‐D and 2‐D groups, P = .0002) (WIT: 1.7 ± 0.6 vs 2.3 ± 0.9 minutes in the 3‐D and 2‐D groups, P = .02). No conversion to open surgery occurred in both groups. Length of hospital stay was significantly shorter in the 3‐D group. No major postoperative complications (Clavien ≥ III) occurred. One‐year postoperative GFR was similar to 3‐D and 2‐D groups. Our preliminary study demonstrates that 3‐D laparoscopic LDLN is a feasible and safe surgical procedure. Intraoperative and postoperative outcomes were similar in both 2‐D and 3‐D vision systems, but 3‐D vision systems allow reduction in WIT and operative time. 相似文献

Three‐dimensional (3D) printing technology, virtual reality, and augmented reality technology have been used to help surgeons to complete complex total hip arthroplasty, while their respective shortcomings limit their further application. With the development of technology, mixed reality (MR) technology has been applied to improve the success rate of complicated hip arthroplasty because of its unique advantages. We presented a case of a 59‐year‐old man with an intertrochanteric fracture in the left femur, who had received a prior left hip fusion. After admission to our hospital, a left total hip arthroplasty was performed on the patient using a combination of MR technology and 3D printing technology. Before surgery, 3D reconstruction of a certain bony landmark exposed in the surgical area was first performed. Then a veneer part was designed according to the bony landmark and connected to a reference registration landmark outside the body through a connecting rod. After that, the series of parts were made into a holistic reference registration instrument using 3D printing technology, and the patient's data for bone and surrounding tissue, along with digital 3D information of the reference registration instrument, were imported into the head‐mounted display (HMD). During the operation, the disinfected reference registration instrument was installed on the selected bony landmark, and then the automatic real‐time registration was realized by HMD through recognizing the registration landmark on the reference registration instrument, whereby the patient's virtual bone and other anatomical structures were quickly and accurately superimposed on the real body of the patient. The precise cause of the bands of Fontana, striations on peripheral nerves visible to the naked eye, has been the subject of debate for hundreds of years. Some researchers have described them as reflecting the sinuous course of nerve fibres passing through nerves, and others have proposed that endoneurial collagen and sheaths surrounding nerves play a role in their appearance. We hypothesised that the bands are caused exclusively by reflection of light from the surfaces of nerve fibres travelling in phase in sinusoidal waveforms through peripheral nerves. We aligned images of obliquely illuminated nerves with confocal images of axons in those nerves, and the numbers and positions of the bands precisely matched the axonal waves. We also developed three‐dimensional models of nerves with representations of the sinusoidal path of axons at their surface. We observed patterns resembling the bands of Fontana when these models were obliquely illuminated. This provides evidence that the bands of Fontana can be caused by light reflected sinusoidal path of axons alone. We subsequently describe a mechanism of band production based on our observations of both nerves and models. We report that smaller diameter nerves such as phrenic nerves and distal branches of sciatic nerves have shorter band intervals than larger nerves, such as proximal trunks of sciatic nerves, and that shorter band intervals correlate with longer axons per unit length of nerve, which suggests a greater tolerance to stretch. Inspection of banding patterns on peripheral nerves may permit prediction of axon length within nerves, and assist in the interpretation of nerve conduction data, especially in diseases where axon path has become altered. 相似文献

目的 探讨中度稳定期慢性阻塞性肺疾病(慢阻肺)患者疲劳与营养指标、生活质量指标、炎症指标、精神情绪指标的关系。方法 采用前瞻性研究,根据一般疲劳定义,从慢阻肺患者中随机选取中度稳定期慢阻肺疲劳患者30例(疲劳组)与中度稳定期慢阻肺非疲劳患者26例(非疲劳组)。收集两组一般资料、营养指标、生活质量指标、炎症指标、精神情绪指标,并进行比较。结果 两组性别、年龄、吸烟指数等方面差异无统计学意义(均P>0.05);与非疲劳组相比,疲劳组白蛋白、血红蛋白、体重指数、第1秒用力呼气容积占预计值的百分比(percent predicted forced expiratory volume in one-second, FEV1%)、6 min步行距离(6-minute walk distance, 6MWD)均显著降低(P<0.05),慢性阻塞性肺疾病评估测试(chronic obstructive pulmonary disease assessment test, CAT)评分、C反应蛋白(C-reactive protein, CRP)、降钙素原(procalcitonin, PCT)、白细胞(WBC)计数、贝克抑郁和焦虑量表评分, 心理身体紧张松弛测试量表(PSTR)评分均较高(P<0.05)。 结论 疲劳与中度稳定期慢阻肺患者营养、生活质量、炎症、精神情绪有关,是建立慢阻肺患者多维度疲劳测量的重要部分。 相似文献
