Value of contrast-enhanced MRI of breast after silicone implant

Summary Early recognition of recurrence and work-up of clinically indeterminate lesions may be impaired after reconstruction with silicone implants due to superimposition of the implant or to scarring. This study was undertaken to evaluate the use of contrast-enhanced MRI in patients with silicone implant after breast cancer. Contrast-enhanded MRI was offered to 169 patients. Comparative two- to three-view mammography was also performed in 169 patients, as well as comparative sonography in 144 patients. Conventional imaging and clinical examination detected only 8/13 recurrences, whereas 12/13 were detected by MRI. One recurrence had been visible as a strongly enhancing 2-mm dot in a previous examination (2 years before), but was not called. It was therefore counted as false negative. In addition, multicentricity was detected by MRI alone in two of three cases. MRI correctly diagnosed scar tissue in all cases with indeterminate findings. However, due to false-positive calls caused by enhancing granulomas specificity could not be improved. Contrast-enhanded MRI allowed decisive additional information in our study group and improved the sensitivity significantly (concerning all diagnoses). Contrast-enhanded MRI is recommended in patients with diagnostic problems or high risk of recurrence after silicone implants.      

人发角蛋白桥接周围神经缺损的形态学观察

目的 :了解人发角蛋白在神经再生中的作用 ,为临床桥接周围神经缺损寻求新的替代材料。方法 :将 18只新西兰兔的双侧坐骨神经切断 ,造成 10mm缺损 ,一侧用人发角蛋白桥接 (实验组 ) ,另一侧用空硅胶管桥接 (对照组 ) ,术后 1、 2、 3个月通过肉眼观察 ,光镜、电镜和有髓神经密度测定 ,观察、分析神经再生情况。结果 :实验组再生神经均通过 10mm缺损 ,对照组有 2例无神经生长 ;实验组再生神经排列较紧密、有序 ,髓鞘形成早于对照组 ;神经纤维密度明显高于对照组 (P <0 0 1)。结论 :人发角蛋白可促进周围神经再生 ,是桥接周围神经缺损的理想材料。     

维甲酸硅油的视网膜毒性实验研究

目的：了解维甲酸硅油对视网膜是否产生毒性。 方法：12只新西兰白兔的24只眼，随机分为3组，行气体压迫玻璃体手术3天后，分别向玻璃体腔内注入硅油(4只眼)、5μg/ml维甲酸硅油(10只眼)、10μg/ml维甲酸硅油(10只眼)各
0.5m1，用检眼镜、光漳和电镜检查来观察视网膜变化情况。 结果：注入硅油28天后，未发现各浓度的维甲酸硅油对视网膜产生毒性作用。结论：浓度为5、10μg/ml的维甲酸硅油注入玻璃体腔4周，对视网膜不产生毒性作用。 （中华眼底病杂志，1997，13：81-82）     

奥美定注入小香猪体内后生殖毒性的观察

目的　观察奥美定注入小香猪体内后的生殖毒性。方法　注入奥美定后观察小香猪母代及子代的生长、健康情况 ,抽出奥美定检测丙烯酰胺单体含量以及抽血查染色体及微核试验观察。结果　 6只母代小香猪健康 ,2只与公猪交配 ,4个月及 5个月后生出 9只及 4只子代小猪 ,健康情况亦良好。在半年及 1年后分别抽出奥美定检测丙烯酰胺单体没有增加 ,共检测 9只小香猪的染色体包括母代及子代 ,均未发现数目畸变及结构畸变 ,微核试验有细微的变化 ,并初步反映出与用药量大小呈正相关 ,但超量注射的一只动物 (18.5ml/kg)变化的千分值仍在正常范围内。结论　从目前几项观察与检测指标看 ,实验量奥美定注入小香猪体内尚不具生殖遗传毒性     

Breast augmentation with silicone implants: the role of surgical drainage—report on 502 consecutive patients

Breast augmentation is one of the most frequently performed cosmetic surgery procedures worldwide. We report on a series of 502 consecutive cosmetic breast augmentations by a single surgeon, using both the submuscular and submammary approaches. A vacuum drain was placed in the surgical pocket at the end of the procedure. The drain fluid collection was recorded immediately after the surgery and the following day before removal. A tenfold increase in the amount of fluid collection was noted in the drain’s reservoir. Thirteen patients presented with minor wound healing problems at the second postoperative follow-up, which were treated conservatively; two of them had the implant removed and replaced after 2 months. There were only two capsular contractions at 1-year follow-up. We focus our discussion on the drainage in the early postoperative period and conclude that the routine use of a surgical drainage reduces the risk for complications. Based on our findings, we advocate that short-term routine use of a surgical drain is advisable.     

Comfort of fenestrated hydrogel lenses

A controlled, double-masked, randomized study was conducted on ten subjects to determine the effect of fenestration size on the initial comfort of hydrogel contact lenses. Four fenestrated lenses were tested, each lens containing four mid-peripheral fenestrations of the same size. The diameter of the fenestrations used in the four lenses ranged from 0.39 to 0.96 mm. An unfenestrated lens was also tested. All lenses were made of HEMA and were ordered with the following specifications: -3.00 D, 14.0 mm diameter, 8.4 mm back central optic radius and 0.06 mm centre thickness. There was a significant negative correlation between comfort and fenestration size, indicating that larger fenestrations are less comfortable. Even the lens with the smallest fenestrations (0.39 mm) was significantly less comfortable than the unfenestrated lens. The implication of this finding is that fenestrations may not be clinically efficacious in view of the poor comfort (and presumably increased mechanical effect of the fenestration edges on the tarsal conjunctiva) of fenestrated lenses.     

聚丙烯酰胺水凝胶取出同期假体隆乳术分析

目的:介绍一种聚丙烯酰胺水凝胶注射隆乳后取出同时置入假体隆乳的新方法。方法:18例聚丙烯酰胺水凝胶注射隆乳术患者在取出注射物后同期于胸大肌下置入硅凝胶假体。结果:18例患者均取得了良好的效果,外观形态好,无1例出现血肿和感染。结论:聚丙烯酰胺水凝胶注射隆乳后取出注射物同期胸大肌下置入硅凝胶假体是一种可行且效果良好的隆乳矫治方法。     

Chest Wall Implants: Their Use for Pectus Excavatum,Pectoralis Muscle Tears,Poland's Syndrome,and Muscular Insufficiency

Solid customized and prefabricated silicone implants have been used by the author for 15 years in a wide range of chest wall deformities. Chest wall implants are often used in males seeking to augment a muscularly deficient or underdeveloped chest; however, their greatest use has come in a variety of deformities both congenital and acquired, such as pectus excavatum, Poland's Syndrome, and pectoralis muscle tears. The implants can be either customized using a moulage technique or are prefabricated, manufactured implants which can be modified on the operating table to repair the contour deformity. The immediate postoperative problem of seroma and subcutaneous implant ``show' has been minimized by careful planning, gentle technique, deep insertion, improved patient positioning on the operating room table, and the use of oral anti-inflammatory medications. The long-term results of these implants seem very satisfactory. The patients are usualy physically active, and the implants show no long-term sequelae such as seroma, infection, displacement, or rupture.     

Alloplastic injectable biomaterials for soft tissue augmentation: a report on two cases with complications associated with a new material (DermaLive) and a review of the literature

A wide variety of biological and alloplastic injectable biomaterials are available for soft tissue augmentation, but the ideal material has not yet been discovered. Biological materials such as collagen and hyaluronan yield temporary results, while injectable alloplasts are apt to cause varying degrees of foreign body reactions that may result in lumps and chronic inflammation.We present two cases (one is the first filed case in the world) of migratory subcutaneous inflammatory masses secondary to injection of acrylic hydrogel (DermaLive), which is an alloplastic biomaterial recently introduced into the market in Europe. Histology revealed foreign body reaction to acrylic hydrogel with granuloma formation containing multinucleated giant cells. Following this, further reports on complications have been reported elsewhere in Europe. The use and development of injectable materials, as well as alternative methods and future directions are reviewed.     

Aspects of silicone rubber as an encapsulant for neurological prostheses

Silicone rubber is not, at first sight, a promising material with which to encapsulate a piece of implantable microelectronics expected to last ten or twenty years. It is well known to be very permeable to water. In fact, if correctly applied, it performs very well for ten years, possibly longer. The paper considers the part player by osmosis in achieving satisfactory performance combined with simple technology.
