本文在56例成年反复尿感病人进行了排尿期膀胱尿路造影(MCU)及同位索间接法膀胱输尿管造影(IVRC)检测膀胱输尿管返流(VUR)的比较,结果发现两者符合率为71.4%,而在RN病人中IVRC及MCU显示返流阳性率分别为59.4%及40.6(P<0.05)。在17只实验性VUR小猪共98侧输尿管进行了MCU及同位素直接法胱输尿管造影(DRC)检测VUR的研究。结果显示两者诊断VUR的总符合率为91.8%。DRC诊断VUR的特异性为92.3%。以上结果提示:IVRC是一种简单、易行、安全的VUR筛选方法;DRC作确诊VUR的新方法,有较大实用价值。  相似文献

We assessed the accuracy of voiding color Doppler ultrasonography (US) with echo enhancement for diagnosis or exclusion of vesicoureteral reflux (VUR) versus voiding cystourethrography (VCUG) and evaluated patient tolerance of the echo-enhancing agent. One hundred twenty-two patients (ages range, 1 month to 17 years) with 244 ureterorenal units underwent voiding color Doppler US with echo enhancement, which was followed by VCUG on the same day. After US of the urinary tract, the bladder was filled with saline solution via catheter. Then an echo-enhancing agent was instilled, and color Doppler US was performed. Reflux was diagnosed when microbubbles appeared in the ureter or the pelvicaliceal system. VUR was detected in 98 units. All grades of reflux were identified. In 13 units (13.3%), grade I reflux diagnosed at voiding color Doppler US was not observed at VCUG. No reflux was detected in 146 units by either method. In 231 (94.2%) of the 244 ureterorenal units, there was concordance between the two methods. The echo-enhancing agent was well tolerated by all patients. Voiding color Doppler US with echo enhancement is useful for the diagnosis or exclusion of VUR, being as good as VCUG. Therefore, it may reduce the number of patients exposed to ionizing radiation.  相似文献

Many complaints are self-limited, but some represent disorders that can threaten organ function. 目的探讨排尿性膀胱尿道造影(micturating cystourethrography,MCU)筛查儿童原发性膀胱输尿管反流(vesi-coureteric reflux,VUR)的意义。方法选择40例经99Tcm-二巯基丁二酸(DMSA)显像后肾脏放射性摄取缺损或减少的尿路感染(urinary tract infection,UTI)患儿行MCU检查,依据国际反流性肾病协会提出的VUR分级标准评价MCU诊断结果。结果 40例MCU检查诊断原发性VUR 16例,检出率为40.0%,其中双侧9例,单侧7例;男8例,女8例;年龄<1岁13例。MCU检查共检出VUR 25个肾输尿管单位,其中Ⅱ级反流2支(8.0%),Ⅲ级反流5支(20.0%),Ⅳ级反流12支(48.0%),Ⅴ级反流6支(24.0%)。结论对UTI患儿进行规范的MCU检查可为原发性VUR诊断和治疗提供客观依据。  相似文献

目的:探讨经腹膜外径路腹腔镜下行膀胱外输尿管膀胱再植术的临床应用可行性。方法:2005年9月—2007年3月用腹膜外腹腔镜下膀胱外黏膜下隧道式抗返流输尿管膀胱再植术(Lich-Gregoir改良术)治疗10例输尿管出口梗阻患者(男性4例,女性6例)。5例为单纯先天性输尿管末端狭窄;2例为巨输尿管症;3例为重复肾输尿管末端狭窄,其中2例为双侧性。结果:10例均获得成功,无中转开放手术。单侧手术时间(107±22)min,出血量(49±16)mL,住院时间(5.8±1.1)d,术后无一例发生并发症。随访3~12个月,B超、静脉肾盂造影和(或)磁共振尿路成像显示肾积水或输尿管扩张有不同程度好转,排泄性膀胱尿道造影示无一例发生膀胱输尿管返流。结论:腹膜外径路腹腔镜下行膀胱外黏膜下隧道式抗返流输尿管膀胱再植术的方法简单、创伤小、并发症少、恢复快、抗返流效果好,值得临床推广应用。  相似文献

PURPOSE: To determine whether sonographic examination of subureteral implants after endoscopic dextranomer/hyaluronic acid (DHA) injection would help to evaluate the efficacy of this method in the treatment of vesicoureteral reflux. MATERIALS AND METHODS: Thirty-six patients (49 ureters) who underwent endoscopic subureteral DHA injection were evaluated using voiding cystourethrography (VCUG) and bladder sonography for a mean duration of 2.1 years (range, 3 months to 6.5 years) after treatment. Patient records-including the results of VCUG, renal sonography, Dimercapto succinic acid (DMSA) scintigraphy, and periodic urinary analysis-were also reviewed to determine the outcome of treatment. RESULTS: Reflux was corrected in 43/49 (88%) ureters (complete cure in 38, downgrading of reflux in 5). No change was noted in 6 (12%) ureters on VCUG. The reflux was found to be corrected in 19/22 (86%) ureters, with the implants clearly identifiable on sonography. However, reflux was documented in only 3/27 (11%) ureters, around which no implant was seen on sonography. CONCLUSION: The identification of the implant in the subureteral region by bladder sonography correlated with resolution of reflux in the early postinjection period. On the other hand, the majority of the implants disappeared on sonography in the long term despite correction of reflux. Therefore, we believe that the use of bladder sonography is not useful in the evaluation of patients in long-term follow-up.  相似文献
