Diabetic foot ulcer is a devastating complication of diabetes mellitus and significant cause of mortality and morbidity all over the world and can be complex and costly. The development of foot ulcer in a diabetic patient has been estimated to be 19%-34% through their lifetime. The pathophysiology of diabetic foot ulcer consist of neuropathy, trauma and, in many patients, additional peripheral arterial disease. In particular, diabetic neuropathy leads to foot deformity, callus formation, and insensitivity to trauma or pressure. The standard algorithms in diabetic foot ulcer management include assessing the ulcer grade classification, surgical debridement, dressing to facilitate wound healing, off-loading, vascular assessment (status and presence of a chance for interventional vascular correction), and infection and glycemic control. Although especially surgical procedures are sometimes inevitable, they are poor predictive factors for the prognosis of diabetic foot ulcer. Different novel treatment modalities such as nonsurgical debridement agents, oxygen therapies, and negative pressure wound therapy, topical drugs, cellular bioproducts, human growth factors, energy-based therapies, and systematic therapies have been available for patients with diabetic foot ulcer. However, it is uncertain whether they are effective in terms of promoting wound healing related with a limited number of randomized controlled trials. This review aims at evaluating diabetic foot ulcer with regard to all aspects. We will also focus on conventional and novel adjunctive therapy in diabetic foot management.  相似文献

BackgroundPassive and hybrid passive ankle foot orthoses (AFOs) are often prescribed in post stroke drop foot; however, the effects of these AFOs on balance related parameters in these patients seem unclear. Accordingly, the aim of current study was to evaluate the role of the newly designed hybrid passive and Posterior Leaf Spring (PLS) AFOs on balance related parameters including: self-reported balance confidence (ABC), Timed Up and Go Test (TUG) and Berg Balance Scale (BBS) in post stroke drop foot patients.MethodsFifteen post stroke drop foot patients were recruited in current study. Then, ABC, TUG and BBS were assessed with newly designed AFO and PLS AFO.ResultsThe results of this study were shown a significant improvement in ABC, TUG and BBS scores with the newly designed AFO than PLS AFO (p < 0.05).ConclusionThis study suggested that the newly designed AFO was improved the balance related parameters than PLS AFO.  相似文献

Background and aimsDiabetes mellitus (DM) is a chronic metabolic disease associated with long-term multisystem complications, among which nonhealing diabetic foot ulcers (DFUs) are recognized as major cause of morbidity and mortality. Treating DFUs with surgical procedures such as synthetic or biological skin grafts or skin substitutes has several limitations, where none of the currently available skin substitutes is ideal.MethodsOVID/Medline and PubMed databases were searched using the Medical Subject Heading (MeSH) or Title/Abstract words (“diabetic foot ulcers”, “skin substitutes”, and “nanofibers”), to identify published research studies on DFUs and nanofibers.ResultsElectrospinning nanotechnology is being used in the biomedical field to produce polymeric nanofibers impregnated with drugs for wound healing, burns and diabetic ulcers. Those nanofibers also enable seeding of cells into them and culturing them in vitro to synthesize tissue-like structures. Knowing the advantages of generating patient-specific induced pluripotent stem cells (iPSCs) and organoids in three-dimension (3D), including skin organoids, it is worth mingling these technologies to develop tissue-engineered biological skin substitutes.ConclusionNanofiber-skin substitutes hold promise for treatment of patients suffering from DFUs and inspire novel strategies that could be applied to other organ systems as well, introducing a new era of “regenerative and personalized medicine”.  相似文献

BackgroundClinical tests of foot posture and mobility are not strongly related to the dynamic kinematics of the foot during gait. These measures may be more directly related to foot and ankle kinetics. The supination resistance test (SRT) is a clinical test that may more directly measure forces acting on the weightbearing foot to provide clinicians with insight about the loading of foot structures.Research QuestionWhat is the relationship between the SRT in relaxed calcaneal stance and in single-leg-stance and the kinetics and kinematics of the foot and ankle during gait?Methods10 healthy adults between the ages of 18 and 65 were recruited to participate in this study. Three-dimensional motion analysis was performed using the Oxford Foot Model during gait. The results of the SRT were compared with peak midfoot and ankle joint moments, power generation and absorption, joint angles, and peak angular velocities and accelerations. Correlation coefficients were calculated to assess the strength of relationships between these variables and the SRT.ResultsThe SRT demonstrated significant relationships with several variables. In relaxed calcaneal stance, the SRT was inversely related to maximum midfoot pronation moments (r = −0.51), maximum midfoot plantarflexion moments (rho = −0.71), and peak midfoot power generation (r = −0.61). In single-leg-stance, the SRT was significantly related to maximum midfoot plantarflexion moments (rho = −0.55) and peak midfoot power generation (r = −0.47).SignificanceThe SRT is significantly associated to several kinetic variables that quantify midfoot loading during gait. Interventions that decrease supination resistance may have the potential to increase midfoot power generation.  相似文献
