目的 探讨低温等离子腺样体切除联合双侧扁桃体方案治疗儿童鼾症的疗效及其影响因素研究。 方法 选取鼾症患儿110例作为研究对象,按照随机数字表法分为对照组与观察组,各55例。观察治愈率及总有效率,对比术前、术后1个月、6个月的免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白G(immunoglobulin G,IgG)、免疫球蛋白M(immunoglobulin M,IgM)。评估术前、术后7 d的呼吸暂停低通气指数(apnea hypopnea index,AHI)、夜间最低氧饱和度[minimum blood oxygen saturation,LSa(O2)]及儿童睡眠问卷(pediatric sleep questionnaire,PSQ)评分。 结果 观察组的总有效率高于对照组,且治愈率高于对照组(P<0.05)。观察组手术时间、术中出血量、术后疼痛时间及症状缓解时间均低于对照组(P<0.05)。2组IgA、IgG、IgM均随着时间增加,术后1个月、6个月,观察组的IgA、IgG、IgM高于对照组(P<0.05)。2组IgA、IgG、IgM在组间,时点间,组间时点间交互作用比较差异有统计学意义(P<0.05)。简单效应LSD-t成对比较显示,术前2组IgA、IgG、IgM、AHI、LSa(O2)及PSQ评分比较,差异无统计学意义(P>0.05);术后1个月、6个月,观察组的IgA、IgG、IgM高于对照组(P<0.05)。术后7 d,观察组的AHI低于对照组,而LSa(O2)、PSQ评分高于对照组(P<0.05)。治愈/显效组与有效/无效组在性别、年龄、病程、体重、术前IgA、术前IgG、术前IgM、术前AHI、术前LSa(O2)、术前PSQ评分比较中,差异无统计学意义(P>0.05);而在手术方法比较中差异有统计学意义(P<0.05)。logistic回归分析显示,动力切割系统切除腺样体联合常规剥离双侧扁桃体手术是影响儿童鼾症治疗效果的独立危险因素(P<0.05)。 结论 低温等离子腺样体切除联合双侧扁桃体切除治疗小儿鼾症效果显著,不仅对免疫功能影响小,并且能改善患者通气功能。  相似文献

We read with interest the commentary by Skeldon and Dijk about our article “Weekly, seasonal and chronotype-dependent variation of dim light melatonin onset.” The discussion points raised by Skeldon and Dijk are currently among the most hotly debated in human circadian science. What external factors determine human phase of entrainment? How great is the contribution of natural versus artificial light and sun time versus social time? Our intra-individual data add to the still limited evidence from field studies in this matter. In their commentary, Skeldon and Dijk formulate two either-or hypotheses, postulating that humans entrain either solely to the natural light-dark cycle (sun time referenced by midday) (H1) or solely to the light selected by local clock time and social constraints (H2). Neither hypothesis accounts for the effect of season on human light exposure. We interpreted our findings along more complex lines, speculating that the 1-h earlier melatonin rise in summer found in our sample is likely the combined result of daylight saving time (DST)-induced behavioral advances and a stronger natural zeitgeber in summer (light exposure determined by social and seasonal factors, Horiginal). Here, we show how the criticism by Skeldon and Dijk is based on two sentences quoted out of context (misrepresenting our hypothesis as H1) and that their hypothesis H2 leaves out important seasonal components in light exposure.  相似文献

IntroductionThe association between the intensity of obstructive sleep apnea and skeletal alterations in the face and hyoid bone is still scarcely addressed in the literature.ObjectiveTo evaluate whether the intensity of obstructive sleep apnea is associated with craniofacial alterations and the position of the hyoid bone in children with mixed dentition.Methods76 children aged 7 to 10 years old were examined by otorhinolaryngological evaluation, polysomnography, and orthodontic assessment, including cephalometry. The participants were divided in 3 groups: primary snoring, mild obstructive sleep apnea and moderate to severe obstructive sleep apnea. Cephalometric measures of the face and hyoid bone were assessed. These measures were compared among the different groups by unpaired Student's t test. Moreover, these measures were correlated with the patient's obstructive apnea and hypopnea index variable using Pearson's correlation test.ResultsOf the 76 children, 14 belonged to group 1, with primary snoring; 46 to group 2, with mild obstructive sleep apnea; and 16 to group 3, with moderate-severe obstructive sleep apnea. There was no difference between the groups regarding the craniofacial variables. Children with obstructive sleep apnea showed a longer distance from the hyoid bone to the mandibular plane when compared to the primary snoring group (p < 0.05). Between the two obstructive sleep apnea subgroups, patients with moderate or severe disease showed significantly shorter horizontal distance between the hyoid bone and the posterior pharyngeal wall (p < 0.05), when compared to the groups with mild obstructive sleep apnea. We also observed a significant positive correlation between obstructive apnea and hypopnea index and the distance from the hyoid to the mandibular plane (p < 0.05) as well as a significant negative association between obstructive apnea and hypopnea index and the horizontal distance from the hyoid to the posterior pharyngeal wall (p < 0.01).ConclusionWe did not observe any association between obstructive sleep apnea and linear lateral alterations of the face. In contrast, there is a direct association between obstructive sleep apnea severity and the inferior and posterior position of the hyoid bone in children aged 7 to 10 years old.  相似文献

目的 分析双水平气道正压双水平气道正压通气(bi-level positive airway pressure,BiPAP)对阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者血管内皮功能及预后的影响。 方法 将100例OSAHS患者纳入研究,随机分为观察及对照组,各50例。观察组行BiPAP ,对照组行常规治疗,比较两组多导睡眠监测(polysomnography,PSG)检查指标(低通气指数、呼吸暂停指数、呼吸暂停及低通气时间、90%以下血氧饱和度次数、90%以下血氧饱和度时间)、心肺功能[脑钠肽(brainnatriureticpeptide,BNP)、右心室Tei指数、左心室Tei指数、一秒用力呼气容积占用力肺活量的百分比(forced expiratory volume in one second/forced vital capacity,FEV1/FVC)、一秒用力呼气容积占预计值的百分比(FEV1占预计值%)]、血管内皮功能[内皮素1(endothelin-1,ET-1)、一氧化氮(nitric oxide,NO)、凝血酶Ⅲ(antithrombin Ⅲ,AT-Ⅲ)]及生活质量。 结果 观察组低通气指数、呼吸暂停指数、呼吸暂停及低通气时间、90%以下血氧饱和度次数及时间指标均显著低于对照组(P<0.05)。观察组治疗后BNP以及Tei指数显著低于治疗前及对照组治疗后,FEV1/FVC、FEV1占预计值%显著高于治疗前及对照组治疗后(P<0.05)。观察组治疗后ET-1、AT-Ⅲ低于治疗前及对照组治疗后,NO高于治疗前及对照组治疗后(P<0.05)。观察组治疗后生活质量各项目评分高于对照组(P<0.05)。 结论 BiPAP治疗能够改善OSAHS患者的预后情况及血管内皮功能,效果确切。  相似文献

Because yoga is increasingly recognized as a complementary approach to cancer symptom management, patients/survivors and providers need to understand its potential benefits and limitations both during and after treatment. The authors reviewed randomized controlled trials (RCTs) of yoga conducted at these points in the cancer continuum (N = 29; n = 13 during treatment, n = 12 post-treatment, and n = 4 with mixed samples). Findings both during and after treatment demonstrated the efficacy of yoga to improve overall quality of life (QOL), with improvement in subdomains of QOL varying across studies. Fatigue was the most commonly measured outcome, and most RCTs conducted during or after cancer treatment reported improvements in fatigue. Results also suggested that yoga can improve stress/distress during treatment and post-treatment disturbances in sleep and cognition. Several RCTs provided evidence that yoga may improve biomarkers of stress, inflammation, and immune function. Outcomes with limited or mixed findings (eg, anxiety, depression, pain, cancer-specific symptoms, such as lymphedema) and positive psychological outcomes (such as benefit-finding and life satisfaction) warrant further study. Important future directions for yoga research in oncology include: enrolling participants with cancer types other than breast, standardizing self-report assessments, increasing the use of active control groups and objective measures, and addressing the heterogeneity of yoga interventions, which vary in type, key components (movement, meditation, breathing), dose, and delivery mode.  相似文献

目的研究鲜天麻对睡眠干扰(sleep interruption,SI)诱导的小鼠学习记忆障碍的改善作用。方法 HPLC法测定鲜天麻中天麻素、对羟基苯甲醇的含量,苯酚硫酸法测定多糖的含量。60只ICR雄鼠随机分为对照组、睡眠干扰模型组、阳性药(莫达非尼)组和鲜天麻低(3 g/kg)、高(9 g/kg)剂量组。睡眠干扰造模14 d后,依次进行自主活动、新物体识别、水迷宫和避暗等动物行为学检测实验,并测定小鼠血清和海马组织超氧化物歧化酶(SOD)、丙二醛(MDA)水平及海马组织乙酰胆碱(Ach)、谷氨酸(Glu)和去甲肾上腺素(NE)水平。结果自主活动实验中,各组小鼠运动功能无显著性差异。与对照组比较,模型组小鼠在新物体识别实验中的相对辨别指数(DI)显著性下降,水迷宫寻台潜伏期明显延长,避暗实验错误次数增加、入暗潜伏期缩短;血清和海马组织MDA水平升高,海马组织的SOD水平降低;海马组织Ach、Glu和NE水平均显著降低。与模型组比较,莫达非尼和鲜天麻各剂量能不同程度增加小鼠新物体识别DI,提高新物体辨别能力;增强空间学习获得和保持能力,缩短水迷宫潜伏期;减少避暗错误次数、延长避暗潜伏期;提高血清和海马组织中SOD、Ach、Glu和NE水平,降低MDA水平。结论鲜天麻能改善睡眠干扰引起的学习记忆障碍,改善氧化应激和神经递质水平,是一种有潜力的改善学习记忆中药。  相似文献

目的通过对2019年2月-2019年6月在西安交通大学第二附属医院实习的大学四年级及大学五年级的临床医学专业的医学生就业意向及就业情况的分析,研究住院医师规范化培训制度对医学生就业的影响。方法运用自制的问卷进行匿名填写的方式对大学四年级、大学五年级的临床医学本科生及5+3模式的规范化培训医学生进行调查。结果大部分医学生选择继续升学,选择就业的学生多愿意留在大城市就业,选择住院医师规范化培训的学生只占极少数。结论做好住院医师规范化培训政策的宣传,落实各项优惠政策,鼓励学生进行住院医师规范化培训及到基层工作,引导学生理性考研,是目前社会及医学院校就业指导中的重要内容。  相似文献
