ObjectivePrevious studies on glioblastomas (GBMs) have not reached a consensus on peritumoral edema (PTE)’s influence on survival. This study evaluated the PTE index’s prognostic role in newly diagnosed GBMs using a well-designed method.MethodsSelected patients were reviewed after a rigorous screening process. Their general information was obtained from electronic medical records. The imaging metrics (MTD, TTM, TTE) representing tumor diameter, laterality, and PTE extent were obtained by manual measurement in Syngo FastView software. The PTE index was a ratio of TTE to MTD. Multiple variables were evaluated using analysis of variance and Cox regression model.ResultsOf 143 patients, 62 were included in this study. MGMT promoter methylation and tumor laterality were both independent prognostic factors (p = 0.020, 0.042; HR = 0.272, 2.630). The lateral tumors’ index was higher than that of the medial tumors (57.7% vs. 42.6%, p = 0.027). Low-index tumors were located in relatively medial positions compared with high-index tumors (TTM, 4.9 vs. 12.8, p = 0.032). This finding indicated that the PTE index tended to increase with tumor laterality. Moreover, the patients with low-index tumors had a significant survival disadvantage in the univariate analysis but not in the multivariate analysis (p = 0.023, 0.220). However, further analysis found that the combination of tumor laterality and PTE statistically stratified the survival outcome. The patients with lateral high-index tumors survived significantly longer (p = 0.022, HR = 1.927).ConclusionsIn contrast with the previous studies, this study recommends combining PTE and tumor laterality for survival stratification in newly diagnosed GBMs.  相似文献

Music processing and right hemispheric language lateralization share a common network in the right auditory cortex and its frontal connections. Given that the development of hemispheric language dominance takes place over several years, this study tested whether musicianship could increase the probability of observing right language dominance in left‐handers. Using a classic fMRI language paradigm, results showed that atypical lateralization was more predominant in musicians (40%) than in nonmusicians (5%). Comparison of left‐handers with typical left and atypical right lateralization revealed that: (a) atypical cases presented a thicker right pars triangularis and more gyrified left Heschl's gyrus; and (b) the right pars triangularis of atypical cases showed a stronger intra‐hemispheric functional connectivity with the right angular gyrus, but a weaker interhemispheric functional connectivity with part of the left Broca's area. Thus, musicianship is the first known factor related to a higher prevalence of atypical language dominance in healthy left‐handed individuals. We suggest that differences in the frontal and temporal cortex might act as shared predisposing factors to both musicianship and atypical language lateralization.  相似文献

Introduction

The elbow joint is a compound joint made of articulations in between the humerus, ulna and the radius. The coupling areas (joints) are of prime importance from the kinetic-biomechanical perspective and of potential inter-ethnic significance. These articulations can be affected by several pathologies that may require medical and surgical interference. This experimental analysis aims to infer data in relation to the morphometry of the proximal segment of the ulna and its articular surfaces represented by the greater sigmoid notch (trochlear notch) and lesser sigmoid notch (radial notch).

Methods

A sample of fifty ulnae (n = 50, 27 right and 23 left) was studied in connection with; the surface area of the sigmoid notches (SA), weight of ulna, and the volume of proximal portion of ulna (including the olecranon process and reaching inferiorly to the lowest margin of the radial notch), the length of ulna (L). Longitudinal dimensional parameters were also studied including; the straight distance between the highest point (tip) of the olecranon and that of the coronoid process (OCD), and the mid-olecranon thickness in mediolateral (T1) and anteroposterior orientation (T2).

Results

It has been inferred that there were no significant differences in between right versus left ulnae and in relation to the majority of morphometric parameters with an exception for OCD (22.47 vs 20.75, p-value = 0.002). There was a positive correlation in between all the parameters, although the strongest associations were observed in between OCD, the area of the trochlear notch, and the weight of ulna.

Discussion

A precise conclusion was reached in relation to morphometry, volumetry and the pertinent biomechanics of the proximal segment of the ulna. Key findings are of value to biomedical engineers, medical professionals including orthopaedic surgeons and rheumatologists, evolutionary biologist, and physical anthropologist. Data from this study can be used to (reverse) engineer the perfect implant for the elbow joint.  相似文献

ObjectiveThis study assessed the degree of relationship between masticatory laterality and lateral asymmetry of masticatory performance using silicon pieces enclosed in a latex bag.DesignForty-two young adults with natural dentition participated in this cross-sectional, observational study. They performed four different masticatory assays, each consisting of five trials of chewing three pieces of silicon for 20 cycles. In one assay, they were asked to masticate unbagged silicon free-style, whilst in the three other assays they were asked to masticate bagged silicon free-style, unilaterally on the right-hand side and unilaterally on the left-hand side. The preferred chewing side was determined by calculating the asymmetry index for both the free-style assays. Masticatory performance was determined by sieving the silicon particles and the cycle duration was also recorded. Data were analysed using independent samples or paired t-test and linear regression.ResultsMasticatory function using the bagged silicon was similar to that using the unbagged silicon. A significant and positive relationship was observed between the preferred chewing side expressed as the asymmetry index and the side with better masticatory performance. Alternate unilateral chewers demonstrated better masticatory performance than unilateral chewers. However, when free-style and unilateral chewing were compared for each subject, unilateral chewing was found to be as efficient as – or even more efficient than – free-style chewing.ConclusionsThere is a positive association between the preferred chewing side and the more efficient side. Alternate unilateral mastication per se does not promote better masticatory performance than consistently unilateral mastication.  相似文献

Understanding the relationship between cerebral laterality and intelligence is important in elucidating the neurological underpinnings of individual differences in cognitive abilities. A widely used, behavioral indicator for cerebral laterality, mainly of language, is handedness. A number of studies have compared cognitive abilities between groups of left- and right-handers, while others have investigated the handedness prevalence between groups of different cognitive abilities. The present study comprises five meta-analyses of studies that have assessed the handedness prevalence in (a) individuals with intellectual disability (ID) of unknown/idiopathic nature compared to typically developing (TD) individuals, and (b) individuals with intellectual giftedness (IG) compared to TD individuals. Nineteen data sets totaling 16,076 participants (5795 ID, 8312 TD, and 1969 IG) were included in the analyses. Elevated levels of atypical handedness were found to be robust only for the ID to TD comparison. Findings constrain the range of acceptable theories on the handedness distribution for different intelligence levels.  相似文献

In temporal lobe epilepsy (TLE), determining the hemispheric specialization for language before surgery is critical to preserving a patient's cognitive abilities post‐surgery. To date, the major techniques utilized are limited by the capacity of patients to efficiently realize the task. We determined whether resting‐state functional connectivity (rsFC) is a reliable predictor of language hemispheric dominance in right and left TLE patients, relative to controls. We chose three subregions of the inferior frontal cortex (pars orbitalis, pars triangularis, and pars opercularis) as the seed regions. All participants performed both a verb generation task and a resting‐state fMRI procedure. Based on the language task, we computed a laterality index (LI) for the resulting network. This revealed that 96% of the participants were left‐hemisphere dominant, although there remained a large degree of variability in the strength of left lateralization. We tested whether LI correlated with rsFC values emerging from each seed. We revealed a set of regions that was specific to each group. Unique correlations involving the epileptic mesial temporal lobe were revealed for the right and left TLE patients, but not for the controls. Importantly, for both TLE groups, the rsFC emerging from a contralateral seed was the most predictive of LI. Overall, our data depict the broad patterns of rsFC that support strong versus weak left hemisphere language laterality. This project provides the first evidence that rsFC data may potentially be used on its own to verify the strength of hemispheric dominance for language in impaired or pathologic populations. Attempts to switch left-hand preferences towards the right side are socialisation practices found in many countries (Perelle & Ehrman, 1994). Although researchers acknowledge that pressures against left-hand use contribute to the cross-cultural fluctuations in the prevalence of left- versus right-hand preference, there has been little systematic cross-cultural study of how these pressures are applied, and who are the major agents applying the pressures to change. Our study explored specific rightward conversion practices and the results of these practices among individuals from two countries. One sample of participants was from a culture categorised as formal, Brazil, and the second sample was from Canada, a nonformal culture (Hofstede, 2001). Researchers have argued that prevalence rates of right-handedness should be higher in cultures that value conformity, called formal cultures, and lower among members of nonformal cultures where conformity pressures are lax (Medland, Perelle, De Monte, & Ehrman, 2004). The socialisation practices used to foster a change from the left to the right side are also predicted to differ in frequency and in kind in formal versus nonformal cultural settings. These cultural differences are assumed to result in differences in success rates of the conversion attempts, with formal cultures producing higher numbers of successfully converted right-handers. Our findings indicate that the formal versus nonformal cultural explanation for cross-cultural fluctuations in the prevalence of hand preference types does not account for the characteristics of the rightward conversion experiences reported by participants in the two groups. We propose that hypotheses concerning the effects of cultural differences on hand preference formation be expanded to include possible biological or genetic variance between groups.  相似文献

Bilateral saccadic eye movements enhance episodic memory retrieval; however, this usually only occurs for consistent-handed, not inconsistent-handed, individuals. It was hypothesized that inconsistent-handers begin closer to the peak of a Yerkes–Dodson-type inverted-U curve and increasing activation pushes them along the curve eventually decreasing performance, while consistent-handers start at a lower baseline and therefore increasing activation increases their performance. The current study tested this hypothesis by using hand clenching (grip strength) to increase activation at 5 different levels for both consistent- and inconsistent-handers. A total of 316 participants were presented with a list of 36 words after which they squeezed a hand dynamometer to induce cortical activation, and then recalled as many of the words as they could. Results showed that, as predicted, both inconsistent- and consistent-handers demonstrated an inverted-U pattern of memory performance as the strength of squeeze increased with inconsistent-handers peaking at a lower level of grip strength than consistent-handers. These results may help explain past findings, not only with episodic memory but also a variety of other cognitive tasks. They may also have interesting theoretical and real-world implications, which are discussed.  相似文献
