目的 探究注意缺陷多动障碍(ADHD)患儿血清25羟基维生素D[25(OH)D]、微量元素表达水平及临床意义。方法 选取2019年4月至2020年11月本院诊治的84例ADHD患儿进行研究(ADHD组),依据ADHD分型标准将患儿分为注意缺陷为主型组(28例)、多动为主型组(33例)、混合型组(23例),并选取同期84例体检健康儿童进行对照研究(对照组)。采用高效液相色谱-串联质谱法检测所有受试儿童血清25(OH)D水平;检测血清铁(Fe)、磷(P)、镁(Mg)、钙(Ca)、铜(Cu)、锌(Zn)、铅(Pb)水平;Pearson法分析ADHD患儿血清25(OH)D水平与Cu、Zn、Pb水平的相关性;Logistic回归分析ADHD发生的影响因素;受试者工作特征(ROC)曲线分析血清25(OH)D、Zn、Pb水平对ADHD的诊断价值。结果 ADHD组血清25(OH)D、Cu、Zn水平均显著低于对照组(P<0.05),Pb水平显著高于对照组(P<0.05);注意缺陷为主型组、多动为主型组、混合型组患儿血清25(OH)D、Fe、P、Mg、Ca、Cu、Zn、Pb水平比较,差异无统计学意义(P>0.05);ADHD患儿血清25(OH)D水平与Cu、Zn水平均呈正相关(P<0.05),与Pb水平呈负相关(P<0.05);25(OH)D、Zn是影响ADHD发生的保护因素(P<0.05),Pb是影响ADHD发生的危险因素(P<0.05);血清25(OH)D、Zn、Pb水平诊断ADHD的曲线下面积(AUC)分别为0.773、0.770、0.772,截断值分别为21.01 ng/mL、16.05 μmol/L、50.69 μg/L,相应灵敏度分别为82.1%、77.4%、63.1%,特异度分别为67.9%、71.4%、88.1%;三者联合诊断ADHD的AUC为0.883,其灵敏度、特异度分别为81.0%、81.0%。结论 ADHD患儿血清25(OH)D、Cu、Zn水平降低,Pb水平升高,25(OH)D、微量元素水平与ADHD患儿类型无关,25(OH)D、Zn、Pb联合有助于临床筛查ADHD患儿。  相似文献

Previous event-related potential (ERP) studies show that a salient lateral sound activates the visual cortex more strongly contralateral to the sound, observed as an auditory-evoked contralateral occipital positivity (ACOP). Studies showed that this activation enhances the early cortical processing of co-localized visual stimuli presented after, reflected by better detection rates, better discrimination, and sharper perceived contrast. We replicated the ACOP, using earphones, and tested whether auditory cuing can influence temporal order judgments (TOJ) for two visual stimuli (horizontal arrangement) as well as if the ACOP would predict the amplitude of this influence. A lateral salient sound was followed, after 150 or 630 ms, by the visual presentation of a pair of disks, one in left and one in right hemifield, with variable SOA. The TOJ task was to indicate which disk appeared first or which disk appeared second (controlling for response bias). We observed an ACOP at posterior electrode sites and confirmed our hypothesis that the lateral sound influenced TOJ by accelerating the perception of the disk presented on the cued side, even though the sound was irrelevant to the task. Furthermore, the ACOP amplitude was correlated to this visual perceptual change, indicating that a larger change in brain activity was associated with a faster processing of co-localized visual stimuli.  相似文献

Yoga is increasingly recognized for both its physical and mental health benefits, yet its central mechanisms of action remain unclear. In addition to benefits generally associated with physical exercise, yoga may also cultivate interoception, the sense of the body's internal state, the ability to notice and respond adaptively to physiological cues. Complicating matters, yoga training varies in the relative emphasis on physical movement and the cultivation of interoceptive awareness, yet few studies have explored whether differences in emphasis impact yoga's benefits. A randomized trial explored the effects of interoceptive emphasis in yoga training on attention and subjective wellbeing. Over a 10-week period featuring classes twice each week, community-dwelling adult participants with moderate depressive symptoms (N = 58) were randomized to attend either more Movement-Focused yoga or Interoception-Focused yoga. Assessments were conducted at baseline, post-intervention, and at a one-month follow-up, which included both self-reports of mood and interoception, and task-performance on a Sustained Attention to Response Task. Relative to Movement-Focused yoga, Interoception-Focused yoga practitioners showed greater improvements in sustained attention, but no differences in self-reported mood or interoception. Yet sustained attention improvements in the Interoception-Focused group were greatest for those endorsing high levels of interoception, an effect that was absent in the Movement-Focused group. Despite an exploratory sample size, these findings support the potential for an interoceptive focus to extend yoga's attentional benefits, particularly for those who report high interoceptive awareness.  相似文献

What is important to think about in surgical education and technical skills training? Technical skills training is grounded in social cognitive theory and the concepts of modeling and self-efficacy. Cognitive and nontechnical learning is critical to supplement the overall proficiency of the surgical learner in performing an operation. Technical learning is cemented by deliberate practice and there is benefit to productive struggle and failure. BackgroundChildren with visual impairments (VIs) face challenges in social play activities, which limits their opportunities to practice social skills.AimsWe investigated whether augmented toys were effective to facilitate play in 52 children with VIs who attended special schools for students with visual impairments and blindness.Methods and procedures52 children with VIs (mean age: 9.22 years, SD = 2.07) played three times with both an augmented and a non-augmented toy. A Playmobil® knight’s castle was augmented with Radio Frequency Identification (RFID) technology, such that each play figure produced audio feedback during play. The RFID-technology could be activated and deactivated. Social and cognitive aspects of play were coded from video and data were analyzed using multilevel logistics.Outcomes and resultsChildren showed less disengagement and more parallel play, but less cooperative play when they used the augmented versus the non-augmented castle. This pattern persisted after repeated play sessions with both toys.ConclusionsThe addition of sounds to physical toys increased shared attention between children with VIs during the exploration of play materials, yet it interfered with social interaction during peer play.  相似文献

Background:The results of published articles on the relationship between the Val158Met polymorphism in the (Catechol-O-methyltransferase) COMT gene and the susceptibility of attention-deficit hyperactive disorder (ADHD) are controversial. We conducted an updated meta-analysis of case-control studies to assess the relationship between Val158Met polymorphism in COMT gene and ADHD susceptibility.Methods:A comprehensive literature search was conducted to identify all the case-control studies on the relationship between the COMT gene Val158Met polymorphism and ADHD susceptibility. According to the heterogeneity test results among studies evaluated with I2, the fixed effect model or random effect model was selected as the pooling method. Meta-regression as well as sensitive analysis were used to explore possible causes of between-study heterogeneity. The funnel plot and Harbord test were used to estimate publication bias.Results:Finally, seventeen studies that met the inclusion criteria were included. The Val158Met genotype distributions of COMT gene in controls were in Hardy–Weinberg equilibrium in all studies. In general, there was no significant association between the COMT gene Val158Met polymorphism and ADHD susceptibility in dominant, recessive, and codominant models. The recessive genetic model (I2 = 60.8%) showed strong heterogeneity among studies, and still no significant association was found after sensitivity analysis. Subgroup analysis stratified by ethnicity (Asian and Caucasian) also showed that there was no significant association in the above-mentioned three models.Conclusions:This updated meta-analysis indicated that the Val158Met polymorphism in the COMT gene may not be related to the risk of ADHD. Further researches are needed to confirm these results.  相似文献

【】目的:探究奥卡西平治疗良性癫痫伴中央颞区棘波患儿的临床疗效及对注意网络功能的影响。方法:选取2013年5月~2015年10月我院神经内科门诊及住院部接受治疗的90例良性癫痫伴中央颞区棘波患儿,采用随机数字法将其分为对照组和观察组,每组各45例,其中观察组给予奥卡西平进行治疗,对照组给予卡马西平进行治疗,治疗结束后比较两组患者治疗疗效和各类不良反应发生率,同时使用注意网络测试工具(ANT)对观察组患者患儿治疗前后的注意网络功能进行比较。结果:观察组患儿治疗后临床疗效优于对照组,但无显著差异(χ2=2.196,P=0.138);治疗后两组患儿脑电图异常放电情况得到明显改善,同时观察组患儿明显优于对照组(χ2=5.415,P=0.020);观察组患儿治疗后注意网络功能明显优于治疗前(P<0.05);观察组患者的各类不良反应发生率显著低于对照组(P<0.05)。结论:虽然使用奥卡西平和卡马西平的治疗效果不存在显著差异,但奥卡西平对良性癫痫伴中央颞区棘波患儿却有明显的疗效,可改善患儿脑电图异常放电,降低不良反应发生率,同时能显著改善患儿的注意网络功能,在临床治疗中值得推广。  相似文献
